Placement Information Medical Information

While NSLIY is an invaluable experience and a time of tremendous growth for participants, it is important for
candidates and their families to understand that by its nature, NSLIY can be emotionally and physically
demanding. Being immersed in a different culture, placed in an unfamiliar host family, school, and community
often involves emotional stress during an adaptation period that varies from person to person.
If a candidate is currently experiencing medical, emotional, psychological, or family difficulties, or has recently
recovered from such issues, the demands of a study abroad program can severely exacerbate such
difficulties or be cause for relapse. For this reason, NSLIY requests as much information as possible be given
below regarding your health status. Your responses to these questions willnot be consideredduring the
selection process. Collecting this preliminary medical information helps facilitate program placement should
you be selected as a finalist for the program. More information on the medical review process can be found in
the NSLIY Terms and Conditions, which all applicants and their parent(s)/legal guardian(s) must thoroughly
review and sign.

If selected as a semifinalist, you will be required to have a physician complete the official NSLIY Medical
Evaluation formand return it to NSLIY. This form will be made available at the time of semifinalist notification.
Failure to submit this form by the stated deadline may result in cancellation of your application.

Do not submit a medical evaluation form with your initial application. It will not be accepted.
