There was an 8 years old child admitted to the in hospital room, he was admitted to the hospital
at 8 pm, the doctor diagnosed that the patient was exposed to DHF, the doctor asked the nurse to
conduct an assessment of the patient.

In the patient's room ...

Nurse : Assalamualaikum maam, I am nurse A. Is this true with a patient named Intan?

Family : Waalaikumsalam, yes true, I am the parent of patient.

Nurse : Maam, according to the doctor's instruction I will do the assesment to the
patient to know what illness is being suffered by your child, I will do the action in
this place at 08.00 WIB. Its about 10 minutes, do you agree?

Family : Yeah ners I agree.

Nurse : Okay if you agrees I will prepare the tool first, then 5 minutes to come I will
come back again to do the assessment to the patient, excuse me!

Family : Okay ners.

The nurse went back to the nurse's room to prepare the equipment for the TTV assessment.

In the patient's room ...

Nurse : Morning maam, As we promised I will do TTV assessment to mother's child.

Family : Yes ners, please.

Nurse : Morning Intan, according to our promise that I here will do the TTV assessment
to you, Are you ready?

Patient : Yeah, I'm ready.

The nurse took TTV action directly to Intan. And the result was blood pressure 100/60 mmHg,
patient temperature 380 C, respiration 28 x / min, and pulse 100x / minute. After that, the nurse
back to the doctor's room to report the results of the assessment, then the doctor instructs the
nurse to check a laboratory examination to the patient.

In the doctor's room ...

Nurse : Assalamualaikum, doc!

Doctor : Waalaikumsalam! Please come in!

Nurse : Doc, I am here to report the results of my assessment of patient named Intan in
the internal medicine room.

Doctor : Yes, please ners.

Nurses : After the assessment, the results of 100/60 mmHg dosage pressure, patient
temperature 380 C, respiration 28x / minute and pulse 100x / minute.

Doctor : All right, to make sure the patient should have a laboratory examination such as
hemoglobin, leukocyte and platelet examinations.

Nurse : Yes doc.

After laboratory tests obtained results hemoglobin 11.8, leukocyte 4000, platelets 133.000. After
the results obtained from the assessment of the doctor can conclude the medical diagnosis of
patient Intan. Patient Intan is DHF.

Doctor's Room ...

Nurse : Good afternoon doctor!

Doctor : Good Afternoon ners, please sit down. Do you handle patients Intan in the
internal medicine room with DHF diagnoses?

Nurse : Yeah right doctor.

Doctor : Well then do the water tapid sponge to reduce the fever of the patient.

Nurse : Yes doc.

After that the nurse immediately contract the time to the patient's family to perform the act of
nursing care.

Nurse : Good morning Maam, still remember with me?

Mother : Yeah I remember ners.

Nurse : Okay if you still remember, how is your child's situation?

Mother : From last night my child is still high fever ners.

Nurse : Okay then, as the doctor's instructions, I will do a hot water compress action
aimed at reducing the fever of your child, the place is here at 08.00 hrs, do you
agree?

Mother : Yes please ners.

Nurse : Good morning Intan, still remember with me?

Patient : Morning, yes still ners.

Nurse : How are you doing?

Patient : Still dizzy, my eyes are also hot.

Nurse : Okay, I will do a hot water compress action aimed to reducing your fever. The
place is here at 08.00, Yeah?

Patient : Yes ners.

Nurse : Well, then I will prepare the tool first.

A few minutes later...

Nurse : Intan are you ready? Before doing the act let's read bismillah first ...

Patient : Bismillahirrahmanirrahim.

Then the nurse direct action hot water compress. After that the nurse again measured the
temperature of the patient and the result of the patient's body temperature returned to normal
37.50 C, after performing the nurse's actions documenting the results of his actions. A few days
later the patient's condition began to improve and the doctor allowed the patient to go home.

Conclusion
A patient was exposed to DHF, during laboratory tests found the number of leukocytes, platelets
and hemoglobin decreased. The patient's body temperature is also high. The action given to the
patient is a warm water compress to keep the patient's body temperature back to normal.
Collaboration between health personnel has been established where nurses and doctors work
together to cure patients.
