ESTADOS IB2, IIA2, IIB, III, IVA: Estos estados localmente avanzados se
caracterizan por ser irresecables, y por tanto, el tratamiento de eleccin sera la
RT-QT concomitante combinada con BT, ya que, esta ha demostrado un mayor
control local y un aumento en las tasas de supervivencia.

En caso de ser necesario sera posible realizar un boost ( IMRT, IGRT ) a nivel
de parametrios, ganglios afectos a unas dosis de entre 5- 20 Gy.

El diagnstico clnico debe incluir la exploracin clnica y ginecolgica con
citologa e histeroscopia con biopsia, as como la ecografa endovaginal que
aportar datos importantes sobre el tumor.
La RMN es la tcnica ms sensible y especfica para la estadificacin local, en
cambio para el estudio de extensin a distancia y ganglionar, la TC toraco-
abdominal, el PET y el PET /TC representan las pruebas de imagen de
eleccin.

El carcinoma de vagina es una neoplasia rara, constituye slo el 1-2% de los
tumores ginecolgicos. El 20% de los tumores vaginales se detectan durante el
screening del cncer cervical, y el 75% son metastticos al diagnstico.
El 80-90% son carcinomas de clulas escamosas, el 15-20%
adenocarcinomas, y el resto, carcinomas de clulas claras, enfermedad de
Paget, basocelular, sarcoma y melanoma.

La BT intersticial juega un papel muy importante cuando existe invasin
parametrial profunda, y debera de administrarse en estos casos con unas
dosis de entre 20-25Gy.
El Cisplatino semanal concomitante con la RTE estara indicado en los estados
ms avanzados de la enfermedad.

BT: Administrada como boost tras RTE ( HDR 4-6Gy x 2-3 Fx ), en estados precoces
con mrgenes <8mm o positivos, o bien, como tratamiento RADICAL en lesiones
pequeas con ganglios negativos. ( HDR 7Gy x 3 Fx a 05cm de superf. vaginal o 6Gy
x 5 Fx sb superf. vaginal ).
