CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF LEASE, made and entered into this ____ day of ______________
2012 at Muntinlupa City, by and between:

_____________________________________, of legal age, married, Filipino and with


residence and postal address at __________________________________________________,
hereinafter referred to as the LESSOR,
-And-

_________________________of legal age, Filipino, single/married to


___________________________and with residence and postal address at
_________________________________________hereinafter referred to as the LESSEE,

WITNESSETH: That

WHEREAS, THE LESSEES hereby accepts to lease from the LESSOR a


commercial/residential space located at ____________________________________________,
subject to the following terms and conditions to wit:

1. TERM OF CONTRACT - That the terms of this lease shall be One (1) year; to
commence on ___________________, _______ and will expire on _______________, _______;
renewable upon the option of the LESSOR and subject to terms and conditions accepted to both
parties;

2. RENTALS- That the LESSEE shall pay the LESSOR a monthly rental of
______________THOUSAND PESOS (Php __________ .00), Philippine Currency every 5th day
of the month;

a. Upon the execution of this contract of lease, LESSEE shall pay the LESSOR an
amount equivalent to two (2) months advance and one (1) month deposit or the total
amount of _____________________ THOUSAND PESOS (Php
_____________.00) upon signing of this contract;

b. That in case the LESSEE fails to pay the above monthly rentals, the lease shall be
deemed terminated and the LESSEE shall peacefully and voluntarily vacate leased
premises and surrender the same with LESSOR without further need of demand,
otherwise, LESSOR shall be constrained to eject LESSEE from the leased premises
and padlock same terminated/disconnect the water and electric (Meralco) supply. Any
personal property left behind by the LESSEE/S should be turned over to the Barangay
for proper disposition. Should the LESSEE leave behind unpaid rentals and the
balance of his/her deposit after deduction will be sold by public auction and the
proceeds thereof to be applied to the unpaid rentals;

c. That the said monthly rental shall increase Ten percent (10%) every year;

4. That the premises leased have been received by the LESSEE in good, healthy and
habitable condition and shall maintain the same during the term of this lease;

5. That all ordinary repairs within that arise in the daily use of the facilities therein shall be
for the sole expense of the LESSEE, without right to reimbursement;

6. No any permanent structures shall be removed or replaced by the LESSEE;

7. That the LESSEE shall use the leased premises exclusively for the purpose leased and not
for any other purposes;
8. That the LESSEE is expressly prohibited to sublease the leased premises to any third party
and is likewise prohibited in storing any inflammable or combustible chemicals as well as prohibited
drugs therein;

9. That all charges for water, light and telephone shall be to the sole account of the LESSEE
during the term of the lease;

10. That LESSEE shall be responsible for the observance of sanitary and electrical
regulations imposed by the city government for the use and habitations of the leased premises;

11. That the LESSEE shall notify the LESSOR at least sixty (60) days in advance should he
decide to terminate the lease during the lifetime of this lease contract and if the lessee shall
unilaterally terminate the lease contract while lease contract is in effect ,the deposit and advance
shall be forfeited, further the advance and deposit of the lease is consumable and the LESSOR has
no obligation to refund said advance and deposit ;

13. LESSEE shall allow the LESSOR or their unauthorized representative to inspect the
leased premises at any reasonable time;

14. Should LESSEE fail to pay two (2) consecutive monthly rentals LESSOR shall have the
legal right to eject the LESSEE from the leased premises without the necessity of a court order, and
in any violation on the terms and conditions set forth in this contract and LESSEE shall immediately
vacate the leased premises and surrender peacefully possession unto LESSOR.

IN WITNESS WHEREOF, the parties have hereunto set their hands this _____ day of
___________, 2012 at Muntinlupa City, Philippines.

BEFORE ME, a Notary public for and in the Muntinlupa City this ____ day of
__________, 2012, personally appeared the parties, known to me to be the same persons who
executed the foregoing instrument and they acknowledge to me that the same is their free act and
deed.
This instrument related to a contract of lease consisting of two (2) pages including the page
where this acknowledgement is written and signed by the parties and their instrument witness at the
first page hereof.
