Ulkus traumatik mulut sering dijumpai dengan penampakan berupa lesi cekung
seperti kawah dengan lapisan eksudat pseudomembran (fibrinosa) dan dikelilingi oleh
lingkaran edema dan terasa nyeri. Pengobatan ulkus mulut secara umum bertujuan untuk
mengurangi rasa nyeri dan mempercepat penyembuhan dengan mempercepat proses
inflamasi. Proses inflamasi disertai dengan migrasi sel-sel inflamasi salah satunya adalah sel
leukosit polimorfonuklear (PMN). Plantago lanceolata L. memiliki komponen aktif yang
memiliki efek antiinflamasi yaitu berupa asam ursolat. Penelitian ini bertujuan untuk
mengetahui efektifitas aplikasi topikal ekstrak etanol daun Plantago lanceolata L. terhadap
jumlah sel leukosit polimorfonuklear pada model ulkus mukosa mulut.
Penelitian ini menggunakan 20 ekor tikus Wistar yang dibagi menjadi 5 kelompok
perlakuan, yaitu kelompok kontrol positif dengan aplikasi Kenalog in Orabase; 3 kelompok
perlakuan orabase ekstrak daun sendok (Plantago lanceolata) berbagai konsentrasi (5%,
15%, 30%); dan kelompok kontrol negatif tanpa aplikasi apapun. Dua ekor tikus dari tiap
kelompok perlakuan didekapitasi pada hari ke-3 dan 7. Kemudian dilakukan pengambilan
jaringan ulkus dan dibuat sediaan histologis dengan pengecatan Hemaktosilin Eosin. Sel
leukosit PMN pada preparat histologis diamati menggunakan mikroskop cahaya perbesaran
400X dengan OptiLab pada 3 lapang pandang oleh 3 pengamat.
Hasil penelitian menunjukkan terjadi penurunan jumlah sel leukosit polimorfonuklear
pada kelompok perlakuan dengan ekstrak Plantago lanceolata konsentrasi 30%. Hasil analisis
Two Way ANOVA menunjukan terdapat perbedaan signifikan (p<0,05) antara kelompok
perlakuan dan hari pengamatan. Uji Least Significant Differences (LSD) menunjukan
terdapat perbedaan jumlah sel leukosit PMN yang signifikan antara kelompok kontrol negatif
dan kelompok perlakuan dengan ekstrak P.lanceolata dengan konsentrasi 30%. Kesimpulan
dari penelitian ini adalah ekstrak etanol P.lanceolata konsentrasi 30% efektif dalam
menurunkan jumlah sel leukosit PMN pada model ulkus mukosa mulut.

 ABSTRACT

Traumatic oral ulcer is characterized by concave surfaced lesion like crater with a layer of
pseudomembranous exudate and surrounded by a circle of edema and painful feeling. The
treatment of mouth ulcer is purposed to decrease the pain by reducing the inflammation. The
inflammatory process is accompanied by the migration of inflammatory cells one of wich is
the polymorphonuclear leukocyte. Plantago lanceolata L. having active components that
have anti-inflammatory effect in the form of Ursolic acid. The study aims to determine of the
effectiveness of topical application of ethanol extract of leaves of Plantago lanceolata L. to
the number of polymorphonuclear leukocyte in traumatic oral ulcer models.
This research used 20 male Wistar rats that were divide into 5 groups; positive control
was treated with Kenalog in Orabase application, negative control was without treatment,
and 3 groups were treated with various concentration of P.lanceolata leaf extract (5%, 15%,
30%). Two male Wistar rats from each group were decapitated on 3rd and 7th day. Excisional
wound of tissue ulceration were perfprmed after for histology examination in which
specimens were stained with Hematoxylin-Eosin. Polymorphonuclear leukocyte were
observed by 3 observer using 400x magnification of light microscope with OptiLab on 3 field
of view.
The result of the research showed a decline number of polymorphonuclear leukocyte in
the group treated with 30% extract concentration of P.lanceolata. The result of Two Way
ANOVA analysis showed that there were a significant differences in the number of
polymorphonuclear leukocyte (p<0,05) between the treatment groups and observation period.
LSD Post Hoc test showed that there were significant differences in the number of
polymorphonuclear leukocyte between the negative control group and treated group with
30% extract concentration of P.lanceolata. In conclusion, leaf extract of P.lanceolata in 30%
concentrationeffective in decline the number of polymorphonuclear leukocyte on the
traumatic oral ulcer models.
