General Information
All athletic field rental requests must be submitted to Jason Kukkola via email at jkukkola@crpd.com or by
appointment at the Mather Sports Center.
Submitting a Field Rental Request form by no way guarantees your reservation. Only after you receive an
approved permit is your request finalized.
Athletic field rental requests will be allocated on a quarterly basis. The schedule is as follows:
o Requests for January, February, and March must be submitted by October 15 of the previous year.
o Requests for April, May, and June must be submitted by January 15.
o Requests for July, August, and September must be submitted by April 15.
o Requests for October, November, and December must be submitted by July 15.
Rental requests made after the quarterly allocation period must be submitted at least 10 business days in
advance of date requesting. Requests made within 10 business days may not be accepted or permitted.
Mather Sports Center is available to rent Monday Friday 8am-10pm and Saturday & Sunday 8am-6pm. Other
Athletic Fields not located at the Mather Sports Center opening and closing times may vary.
Rental times must include set-up and clean-up time.
Renters are responsible for all clean-up and waste removal.
A CRPD employee(s) will be on duty during rental activities at the Mather Sports Center Only.
Premise inspection will be conducted immediately following all rentals.
The District and Mather Sports Center is not responsible for lost or stolen items.
Soccer nets and corner flags are not included or guaranteed to be provided.

Rental Fees
Please refer to facility/fields rental fees sheet for prices.
All rental fees are due at least 30 days in advance of rental date. No exceptions.
Failure to pay all fees in full at least 30 days prior to scheduled event will result in cancellation of event and
retention of deposit.
Renters who arrive earlier or stay later than the reserved time will be charged for the additional time.
Fees are not refunded for reserved time not used.
Extra costs (above facility/field rental fees) may be assessed to compensate for additional staff required for
large/special events.
CRPD reserves the right to adjust fees at any time
Changes to your rental or rentals set up within 10 business days may be subject to additional fees as outlined in
the facility/field rental fees sheet.
Security Deposit
A security deposit is required for all facility rentals and must be paid to secure a reservation for a facility/field.
Security deposits are due in full with application and processed immediately upon receipt.
Security deposits will be returned after the event date if no damages or violations occur.
Deposits made with a credit card payment will be refunded approximately 7-10 business days after the rental is
complete. Deposits made with cash or check will be refunded approximately four to six weeks after the rental is
complete.

Insurance
Rentals must obtain insurance coverage in the minimum amount of $1,000,000 and name the Cordova
Recreation and Park District as additionally insured.
If you are unable to obtain insurance via home owners or independent policy, insurance is available for purchase
through the district. Please contact Gail Bair at 842-3313 gbair@crpd.com.

Cancellation/Refund Policy
30 days or less cancellation prior to rental date = 100% retention of deposit
14 days cancellation prior to rental date = 100% retention of deposit & all fees paid
A full refund for weekend field rentals will be issued due to severe inclement weather or catastrophic
emergency with cancellation made by 12noon on Wednesday of the week leading up to the rental.
Field rental cancellations due to extenuating circumstances with little or no advance notice may/may not merit a
full refund as determined by CRPD staff.
All cancellations are required to be in writing by the person(s) listed on request form. Written cancellations can
be emailed or sent general mail. A follow-up/supporting phone call is recommended.
Failure to comply with all the Cordova Recreation and Park Districts facility/field rental policies and procedures
may result in loss of your deposit and/or facility use permit, scheduled date(s) and/or event being cancelled
early or entirely.
Any false information regarding details of your event may lead to immediate termination of your facility use
permit, scheduled date(s) and/or event, the possible loss of fees and denial of your request for future use
Cordova Recreation and Park District has the right to cancel, change or postpone any of the scheduled dates, with
72-hour notice.
MSC Artificial Turf Rules & Regulations

NO food allowed on any playing surface or in the dugouts.


NO seeds or nuts with shell or hull are allowed ANYWHERE on synthetic fields. Only previously shelled
nuts/seeds may be consumed in designated areas.
NO gum allowed on playing field/dugout areas.
NO tobacco use of any kind on or near the fields and/or the gated complex area.
Fields must be returned to their original condition
NO metal cleats on synthetic fields.
NO gas/propane powered heathers; fires, or grills of any kind are allowed.
NO outside beverages of any kind are allowed in the gated complex area. Exception: Team water container.
Penalty: Items confiscated. If person(s) refuse to give up their unauthorized beverages/container, they will be
escorted from the premises.
NO outside food or ice chests of any kind are allowed in the gated complex area. Penalty: items confiscated. If
person(s) refuse to give up their unauthorized food/containers, they will be escorted from the premises.
Turf shoes, 3/8 or less rubber molded cleats, or athletic running shoes are recommended for the synthetic
fields.
NO concessions are allowed to be sold on any district property.
NO sports drinks/sugar based drinks allowed on playing surfaces/dugout areas of synthetic fields. Only water is
allowed.
NO hitting/kicking fences with baseballs, softballs, soccer balls, bats, feet, or other objects.
NO chairs with sharp ends allowed on fields. If spectators are allowed on the fields, only wide or flat bottom
chair supports may be used on fields.
NO stakes or sharp anchoring devices allowed.
Spectators must remain in designated areas
NO glass containers of any kind.
NO bicycles, skates, skateboards, in-line skates, or scooters.
NO animals or pets allowed. Exception: Assistance dogs.
NO spitting on the fields.
NO alcoholic beverages.
Please remove all trash and debris after use of the facility and dispose in provided receptacles.
NO picking or pulling on the grass fibers or infill granules.
NO digging into synthetic turf with feet, hands, or any other object.
NO moving portable fencing from their designated places.
NO climbing fences.
Renter is responsible for all guests behavior. Violence, excessive drinking, loud behavior and unsupervised
children are not permitted and will not be tolerated. Guests must adhere to all District policies and procedures.
CRPD may cancel any event for violations of disturbing the peace laws.

The Cordova Recreation and Park District reserves the right to search all bags, containers, and any other personal
belongings carriers brought onto the Mather Sports Center grounds. Any items found in violation will be confiscated.
The facility rules and regulations are meant for all persons in/on/around the Mather sports fields. Violation of any of
the rules by any person, regardless of whether they are a spectator, coach, participant, official, tournament sponsor,
etc. will result in immediate termination of the rental and no refunds issued.

"Matinee" Fee = Weekdays before 4pm

"Primetime" Fee = Weekdays after 4pm and Weekends all day

Primetime Fee $85 / hour per field

Softball Tournament Fee $1680 per day for all 3 fields

Fall/Winter Season: November 1 - March 31 Matinee Fee $30 / hour per field

Agreement, Waiver, and Release

In consideration for being permitted by Cordova Recreation and Park District (CRPD) to participate in the above activity, I
hereby waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which
may have, or which may hereafter accrue to me as a result of participation in said activity (ies). This release is intended to
discharge in advance CRPD (its officers, employees, and agents) from any and all liability arising out of or connected in any
way with my participation in said activity, though that liability may arise out of negligence or carelessness on the part of
the persons or entities mentioned above. It is understood that this activity involves an element of risk and danger of
accidents and knowing those risks I hereby assume those risks. It is further agreed that this waiver, release and assumption
of risk is to be binding on my heirs and assigns. I agree to indemnify and to hold the above persons or entities free and
harmless from any loss, liability, damage, cost, or expense which they may incur as a result of my death or any injury or
property damage that I may sustain while participating in said activity.

Photograph Note: By signing this waiver, I understand that CRPD staff may photograph me and/or my minor children and
that the District may use such photographs to promote District programs now and in the future. I expressly allow, and
herby waive any objection to, CRPD photographing of me and/or my minor children when I and/or my minor children are
participating in a CRPD recreation program. I understand that all photos will remain the property of Cordova Recreation
and Park District. I do not approve of having me and/or my minor child(ren) photographed by CRPD staff or its affiliates.
I HAVE CAREFULLY READ THIS AGREEMENT, WAIVER AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE
