PETITION
COMES NOW, Petitioner Herminia R. Orcine, by the undersigned
counsel, and unto this Honorable Court, most respectfully avers:

1. Petitioner is the maternal grandmother of minors Scarlet Orcine

Descalaye and Sacred Orcine Descalaye, Filipino, of legal age, married
to __________, has the legal capacity to sue and be sued and a resident
of Brgy. May-ogob, Ocampo, Camarines Sur, where she may be served
with summons and other processes of this Honorable Court;

2. Petitioner is also the mother of Maricel Orcine Descalaye, the mother

of minors Scarlet and Sacred Descalaye, who died on 9 March 2017 at
Mandaluyong City Medical Center, Mandaluyong City, Philippines.

A and made an integral part of this Petition;

3. Minor Scarlet Orcine Descalaye is likewise a Filipino, born on 18 April

2007, at Brgy. May-ogob, Ocampo, Camarines Sur, and presently
residing with the Petitioner at Brgy. May-ogob, Ocampo, Camarines
Sur;

4. Minor Sacred Orcine Desclaye is likewise a Filipino, born on 23

September 2008, at Brgy. Salcedo II, Noveleta, Cavite, and presently
residing with the petitioner at Brgy. May-ogob, Ocampo, Camarines
Sur;
5.
