I. INTRODUCTION
A. Background and nature of Internship

I conducted my 160 hours/20 days Internship in the Government Service Insurance


System as fulfillment of the internship program of the Accountancy Department of the College of
Business Management and Accountancy of Eastern Samar State University. I was assigned in
the claims unit of the said agency. My whole training in the GSIS required my ability to do clerical
works not only in the unit I am assigned in but also in some other areas of the agency.

B. Objectives of Internship

To be able to apply the knowledge that I have learned in school specifically in the
course that Im taking.
To learn and/or understand how it is like to work in the actual workplace.
To learn how to have a good rapport as a member of a group or organization;
and
To acquire skills that would be helpful when I finally work someday.

C. Job Description

I was personally assigned at the claims unit and was all around sometimes so mostly my
work involved:

a. Encoding transmittals for different kinds of documents


b. Making personal envelopes of new claims or claims that are still to be processed.
c. Stamping the different names of the heads of the agency into vouchers and some
other data sheet.
d. Analyzing datas to help/aid my supervisor
e. Arranging personal envelopes of the clients of the agency
f. Assisting my supervisor in anything that she needs to do

II. ANALYSIS OF AGENCY/BRIEF HISTORY OF AGENCY

Created by Commonwealth Act No. 186 and Republic Act No. 8291 (GSIS Act of JOHN VIANNEY D.
1997), GSIS is a social insurance institution that provides a defined benefit scheme under OCAA
the law. It insures its members against the occurrence of certain contingencies in Extension Offic
exchange for their monthly premium contributions. Head
GSIS members are entitled to an array of social security
benefits, such as life insurance benefits, separation or
retirement benefits, and disability benefits. LEO A. COTONER ALDER D. MADELL

GSIS is also the administrator of the General Insurance Fund
by virtue of RA 656 (Property Insurance Law). It provides MALEU Reconciliation U
insurance coverage to government assets and properties that have
government insurable interests.

The GSIS covers all government workers except:

Members of the Judiciary and Constitutional Commissions who are covered by separate
retirement laws;

Contractual employees who have no employee-employer relationship with their agencies;

Uniformed members of the Armed Forces of the Philippines and the Philippine National Police,
including the Bureau of Jail Management and Penology and the Bureau of Fire Protection.*

To date, the GSIS has 42 Branch Offices, 14 Extension Offices nationwide and 58 service desks.
One of its extension office is located here in Borongan which existed for the past __ years. The
GSIS Borongan extension office has 5 permanent employees and is headed by Mr. John Vianney
D. Ocaa. They work together to render the best services they could possible provide to cater the
needs of the government employees here in Borongan.

III. SUMMARY OF EXPERIENCES

A. Daily Journal

Daily Journal Entry #1 April 17, 2017

Today, we went to school early to comply with all the requirements needed for us to be deployed
in the agency we are to have our on the job training. Fortunately, together with my co interns we
were able to comply our needed documents before the day ends.

Right after all the papers were signed by the respective signatories, we immediately went to our
agency. For my team we were assigned at Government Service Insurance System (GSIS). We
were greeted by their Guard and was immediately instructed as to where Mr. John Vianney Ocaa
- the representative of the Host training establishment - was. Mr. Ocaa told us the different house
rules we should follow during our stay in the agency as interns. He also was very kind for letting
us choose to start our proper training tomorrow. We weren't able to have the signature of our
immediate supervisor because our proper training is yet to start. Because of such we were not
able to notarize the documents needed. We were required to always be on time, wear a name
tag and to always wear our OJT uniform.

So far, that's what happened today and I can't wait to start tomorrows training.

Daily Journal Entry #2 April 18, 2017

My first day of training went well. I went to GSIS early. Sir Ocaa told us a brief background
information of GSIS, introduced us to the staffs and made a brief description of the different work
stations in the agency. Afterwards we were immediately assigned to different personnel who
would direct us with our training. I was assigned in the Claims Unit of GSIS under Ms. Sigrid
Avenido. She was very kind and friendly. The first job she gave me was to sort some papers.
Separate the Letters, Vouchers, Request forms etc. and place it in its appropriate folders. Second,
she made me arranged some envelopes containing the documents of their clients according to
their respective claims (whether the claims are for a Retirement loan, Cash surrender value,
Maturity benefits, Employee compensation, Death, Funeral etc.). Third, she asked me to compile
the new documents of the newly received claims by getting a new envelope and writing the
appropriete notes outside the envelope and placing it again in its appropriate section or type of
claim. She repetitively asked me to do this tasks and I believe it would be permanent because of
the nature of the workstation I am in.

In the afternoon, because ma'am Sig said that I don't have anything to do yet. Ate Flor asked me
and Mike Rey to sort some papers which were Pension, Policy and Consolidated Loans of the
previous years and asked us to make some notes for its Identification. I also removed staple wires
of some papers that were needed to be scanned by Rey Anthony.

Also, Sir Ocaa gave us time to have our MOA and waivers notarized earlier this morning.

It's tiring but I Enjoyed it. I hope I get to learn a lot of things in this training.

Daily Journal Entry #3 April 19, 2017


I woke up early this morning. After grooming myself, I went straight to GSIS and I arrived at 7:50
in the morning. Ma'am Sigrid asked me to retrieve the personal envelopes of those who have had
Employee Compensation claims from the year 2011 to 2016. Ate Flor helped me find them
because I don't know where it is placed. After finding the said Personal envelopes, ma'am Sigrid
made me Identify the Name, Injury/Medical Condition, Physical therapy (yes or no), and the
doctor's fee of those who had claimed for an Employee Compensation and wrote it in the bond
paper she had provided. After, I arranged and compiled the 2016 Pension, Consolidated, and
Policy loans for the months of March, May, September, October, November and December. I
went home to have my lunch afterwards.

In the afternoon, I helped ate Flor to check the monitoring of the Pension, Consolidated and policy
loans we have compiled earlier. Also, ma'am Sigrid made me Identify and list the clients who had
a deficit in their balances in their respective claims from the Referrals of the months of January to
March 2017. Right after, she asked me to retrieve the contact numbers from the personal
envelopes of some of their clients who had a Retirement claim.

We were dismissed at 5:10 in the afternoon.

Daily Journal Entry #4 April 20, 2017

Today, I went to GSIS at exactly 7:30 in the morning. This day was a bit exhausting because we
did a lot of work. First, I removed the staple wires of 5 bundles of educational assistance
application documents. Second, I removed the name of ma'am Sig and stamped sir Ocaa's name
into some retirement and survivorship personal data sheets. Third, I retrieved some pre-need
plans personal envelopes and returned some documents into the personal envelopes of the life
insurance and employee compensation claimants. At 11:00 in the morning, I Identified some
information from the employee compensation data on physical therapy sessions for the calendar
year 2011-2016. I finished it later in the afternoon. After, I helped my co-intern Mike Rey compiled
the Pension, Policy and Condolidated loans for the year 2017 Jan-Mar. Before going home, I
encoded the September 2016 claims Treasury Copy.

I had a hard time identifying the specific information needed by ma'am Sig for her report because
there were missing documents, irregularities in the voucher and official receipts and some have
plenty of unorganized documents. Fortunately for me I was able to provide all the infromation
ma'am needed.

I didn't even notice the time from doing all the work and was just told by my co-interns that it was
already time to go home.

We left at 5:10 in the afternoon.

Daily Journal Entry #5 April 21, 2017

Today I went to GSIS as early as yesterday.

I arranged the 2016 vouchers for the months of September to December according to the dates
of the vouchers from the oldest to the latest vouchers. I also Compiled and put printed titles into
the check release forms for the years 2015-2017. Also, I made 6 Personal accounts or Personal
envelopes for the new claims, arranged and compiled them. Lastly, I checked the transmittals of
the vouchers for the months of september, october, november and december 2016 if they were
correct or if whether they conform with the vouchers included in that certain month. After doing all
those mentioned above, I took my lunch.
In the afternoon, I arranged the personal envelopes of the clients alphabetically in every claim.
After doing so, I encoded 85 vouchers for the month of september and 15 check release forms.

So far I'm getting more comfortable in the office. But we're kind of worrying because we're running
out of things to do. I was even told by ate Flor to slow down in doing all the work assigned to me.

I assisted ma'am Sigrid in analyzing the Employee Compensation Claims documents for the
Physical Therapy sessions. I reconciled the professional fees actually paid by the claimants and
the fees reimbursed to them.

We were dismissed at 5:05 in the afternoon.

Daily Journal Entry #6 April 24, 2017

Today I went to GSIS at exactly 7:45 in the morning. Because its Monday, we had a flag ceremony
first before we started our daily work. After the flag ceremony Sir Ocaa said that on Friday there
will be a Pre-retirement seminar. Afterwards we started to do our work.

First, I helped ma'am Sigrid analyze again the employees compensation claims as to the actual
doctor's fees paid by the claimants. Second, I checked the transmittals and arranged the vouchers
for the months of September and October 2016 and January to March 2017. I also arranged the
check release forms for the year 2015-2016. By the time I finished arranging it was already lunch
time so me and Rey went home first to eat our lunch.

In the afternoon, I, together with kuya James, counted the pensioners brochure, ballpen, folders
and notepads for the pre-retirement seminar on April 28, 2017. We also arranged it according to
sir Ocaa's instruction. After, I encoded the check release forms for the months of January to April
this year. I also compiled them afterwards. Lastly, Before going home I changed the labels in
some of sir Ocaa's folders.

We went home at 5:10 in the afternoon.

Daily Journal Entry #7 April 25, 2017

We all went to GSIS early this morning. We didn't have a lot of things to do today so most of the
time we were at the dining area.

This morning, I sorted out the 2017 forms from the 2016 GSIS member request forms and
compiled them. I also separated or sorted out the mailing transmittals from the rest of the
transmittals for the years 2016 and 2017. Also, I filed some vouchers, GSIS member request
forms and some documents into the clients' personal envelopes. I also made 5 personal
envelopes for todays new claims. I wrote 28 PTB remittances into the Logbook and lastly I
checked the transmittal of the February 2017 enrollment for the GSIS UMID-eCard.

In the afternoon, we just compiled all the GSIS UMID-eCard enrollment documents and labeled it
appropriately.

We were out at exactly 5:10 in the afternoon.

Daily Journal Entry #8 April 26, 2017

Same as the previous days, we went to GSIS early this morning. We, I and Mike were instructed
to identify the kinds or types of documents inside the 14 boxes near the dining area. After, I pasted
some retirement and death claims transmittals into a folder. I also arranged the Member request
forms for the current year according to its date. I put 14 letters into GSIS envelopes and sealed
them appropriately. Afterwards, I changed the envelope and labeled the certified instruction to
update records.

After all those, I took my lunch.

Upon arriving at GSIS earlier this afternoon, we immediately started our work. I checked the 13
transmittals of the Retirement, Survivorship and death claims for the year 2015. After, I, with the
help of Mike Rey, encoded and counted the number of pages of the documents inside the 28
personal envelopes.

It has been a tiring day. I can't wait to sleep later. Anyways, we were out at 5:05 pm.

Daily Journal Entry #9 April 27, 2017

I went to GSIS early as usual. Since tomorrow is the Pre-retirement seminar, most of our work
were about the things that will be needed tomorrow. For the whole morning, we cleaned the
folders to be used tomorrow and I compiled and/or prepared the things that are to be given to the
guests. In every folder, I put a brochure, ballpen, notepad and a program. I prepared 100 folders
all in all. Afterwards, I counted the number of participants from the different agency and was
instructed to put their attendance paper into a folder. I also prepared the certificate of appearance
for each agency depending on the number of participants from the different agencies. After doing
so I went home for lunch.

In the afternoon, I continued counting the certificates because there were new list of participants.
After doing so, I helped Ate Flor in labeling the folders of the different agencies. Also, I binded
bundles of Educational Assistance Application Forms that were already scanned by Rey and I
also removed the staple wires of the Educational Assistance Application Forms that are yet to be
scanned.

We were asked to go to GSIS tomorrow before 7:00 in the morning because we are going to
prepare for the event which will commence at 8:00 in the morning.

We went home at 5:10 in the afternoon.

Entry #10

April 28, 2017

Today, we went to GSIS early as what sir Ocaa said yesterday. When I got there mike was
already there and a little after I arrived Rey came. Before we went to the venue we prepared the
things that are needed to be brought. We also cutted some pieces of scratch papers for the open
forum.

When we got there, we immediately started to prepare because it was almost time. I helped ate
Flor prepare the table for the registration of the participants, I placed the appearance sheets in to
the table and organized the kits in the table as well. I also arranged the forms that were brought.
(Retirement forms, CSV and such). A few minutes later, participants were starting to arrive. Ate
Flor and Sir Alder asked me to write the names of the Participants who came in to the Certificate
of Appearance. So during the event, I was Writing until the end because their were like 111
participants. When the event was finished everyone took there lunch because the event was
catered. When the participants were eating I, together with Ate Flor, distributed the Certificates to
them. Afterwards, we ate our lunch as well.

After cleaning and organizing our things, we went back to the office. We rested a little, and
continued to work after. I made 10 personal envelopes for the new claims. I also stamped into 9
survivorship proceeds data sheet and 4 survivorship updating form. Lastly, I removed the staple
wire of a bundle of papers to be scanned and binded 4 papers to be scanned as well.

We went home afterwards. It was a long day.

Daily Journal Entry #11 May 02, 2017

I went to GSIS early this morning. After logging in, we immediately did our respective work. I was
asked to put 2 copies of vouchers to the personal envelopes of some processed claims. I also
gave another copy of the said voucher to Kuya James and Ate Flor as a treasury copy. After, I
piled the said personal envelopes behind sir Ocaa's table, alphabetically. I retrieved the
cellphone numbers of some specific employees compensation claimants that ma'am Sig needed.
When I didn't have anything else to do, I helped Rey scanned the educational assistance
application forms until lunch time. We went home afterwards.

In the afternoon, I stamped into some retirement and survivorship proceeds data sheets, letters
and Annuitants updating forms. After, I helped Rey again scanned the educational assistance
application forms until 4:30 in the afternoon.

We went home at 5:05 pm.

Daily Journal Entry #12 May 03, 2017

Today, I went to GSIS just in time because of some family matters. First thing in the morning I
stamped Sir Ocaa's name into 4 retirement proceeds data sheet. Afterwards, I arranged the
educational assistance application form that was scanned by Rey and Mike. I also stapled the
same documents before arranging them. Later in the morning, kuya James called me and gave
me a new work to do and that is to encode or make a transmittal of checks released from 2007
up to the present. So, I started to encode the checks released in 2007 in the morning.

In the afternoon, I continued to encode the remaining checks released in 2007 and started the
checks released in 2008.

We were out at 5:01 in the afternoon.

Daily Journal Entry #13 May 04, 2017

I went to GSIS early today. Most of the time I encoded transmittals for the year 2008. I wasn't able
to finish it today because there were a lot. Aside from encoding, I stamped sir Ocaa's name into
some letters and retirement proceeds data sheet. I also made personal envelopes for some new
claims that were to be processed.

I encoded checks released documents the whole afternoon.

We went home a little later than the usual because there were a lot of things to do, individually.

Daily Journal Entry #14 May 05, 2017

I went to GSIS as early as yesterday. From morning till afternoon, I encoded checks released for
the years 2008 and 2010. At times, ma'am Sig would call me to ask me to do something. Like, I
made personal envelopes for the new claims and I have arranged and filed documents.

Today was kind of tiring because there were a lot of check released documents that must be
encoded. I'm hoping I would be able to finish it all next week.

We went out together same time as the usual.

Daily Journal Entry #15 May 08, 2017

I went to GSIS just in time before the flag ceremony started. After the ceremony, sir Rommel
Ocaa made some announcements. First, he congratulated and thanked us for a job well done
on the Pre-retirement seminar last April 28. Second, he said that the Vice president will visit on
May 18 and that we must prepare for his/her arrival. He also asked us as when will be the last
day of our internship. He told ate Flor and kuya James to maximize the work that we could
contribute or help for the remaining days of our internship. Afterwards, we immediately started
our respective work. For the whole morning and afternoon, I encoded checks released for the
years 2009 and 2011. I was not able to finish encoding the checks released for 2011 because it
was already time.

We went out at 5:05 in the afternoon.

Daily Journal Entry #16 May 09, 2017

I went to GSIS as early as the usual. Upon arriving, Ma'am Sig immediately asked me to make
personal envelopes for claims to be processed, file some forms and records and stamp into
letters, proceeds data sheets and annuitants updating forms. Afterwards, kuya James called me
to assist some pensioners who were applying for UMID cards. I helped them in filling up their
enrollment forms and photocopied their respective Identification cards. I encoded the remaining
2011 and some of the 2012 released checks. After, I went home and took my lunch.

Early in the afternoon, I helped ate Flor and Mike to prepare the kits for the upcoming seminar for
beginners. I encoded the released checks for the remaining documents of 2012 for the rest of the
afternoon.

We went home at exactly 5:00 in the afternoon.

Upon arriving at GSIS early this morning, I immediately prepared the things I would use to encode
again the check delivery receipt forms. After encoding a few, I was called by ma'am Sig to do
some things. First, I made 10 personal envelopes for the claims to be processed and compiled
them appropriately. And second, I filed some GSIS members request forms. After doing so, I
continued to encode some of the check delivery receipt forms until lunch time.

After taking my lunch, I immediately went back to GSIS and for the whole afternoon I encoded
the same forms for the first half of 2013.

We logged out at 5:15 in the afternoon.

Upon arriving at GSIS early this morning, I immediately prepared the things I would use to encode

continued to encode some of the check delivery receipt forms until lunch time.

After taking my lunch, I immediately went back to GSIS and for the whole afternoon I encoded
the same forms for the first half of 2013 .

We logged out at 5:15 in the afternoon.

I arrived at the office at exactly 7:50 in the morning. Upon arriving, I immediately started to encode
again and I did that for the whole morning. I encoded the check delivery receipt forms for the
months of April to October 2015. After taking my lunch, I went back to the office. Before I started
to encode again, I went first to ma'am Sig and asked if she needed anything or if there is anything
she wants me to do. She asked me to make some personal envelopes for the new claims to be
processed and compiled them on the cabinet beside her table. I also helped her staple newly
processed vouchers. After doing what ma'am Sig asked me to do I went back to encode the
remaining check delivery receipt forms and that is for the months of November and December
2015.

We logged out at 5:15 in the afternoon.

Entry #20

May 15, 2017

Today's monday so I went to GSIS early because there will be a flag ceremony. When I got there,
3 of our bosses were absent for reasons I do not know aside from sir Ocaa who is on a sick
leave. After the flag ceremony, we took some photos for our portfolio before the clients started to
arrive. Last week I finished all the documents that I needed to encode so I didn't do much today.
Because I don't have anything to do and my immediate supervisor is not around as well, I just
helped my co-interns and kuya James with their tasks. Like grooming the kits that we did not use
during the Pre-retirement seminar and some other documents that needed to be kept in a box.

We went out at exactly 5:00 pm in the afternoon. When we got out, me and Rey prepared the
evaluation sheets/forms we are to give to our respective supervisors tomorrow.

Tomorrow's our last day.

Daily Journal Entry #21 May 16, 2017

I was really excited to go to GSIS early this morning because its the last day of our internship
though I was kinda pissed because I woke up late. Still I managed to be there early. When I got
there both Mike and Rey were already there. We immediately prepared the evaluation forms then
gave it to sir Ocaa. We put the forms in a folder before giving it to him. After, I edited the certificate
of completion and asked Rey to print it and to put it also on the folder which was already at sir
Ocaa's table at that time. As much as I was excited for this day, I was kind of dismayed because
we didn't do much. After assisting ma'am Sig, I didn't do anything until lunch time. Before going
home to take my lunch I went and asked kuya Robert for our DTR's and then I checked if it was
complete or okay. When I finished doing so. Me and Rey went home.

In the afternoon, we didn't have anything else to do as well but then kuya Robert told us that we
have to encode our DTR's and so that is what we did for the whole afternoon. We printed it there
in the office as well and after we placed it on the folder containing the forms and certificate. Also,
our Bosses prepared some food for us because its our last day. Before going home we took some
photos with our bosses and also thanked them for a wonderful time during our internship.

I'm glad that we were able to finish our internship altogether (me and my co-interns).
