1. I declare the information provided to be trueand
honest.
2. I have read and agree to comply with Chartered
Accountants Australia and New Zealands (CA ANZ)
Code of Professional Conduct and Regulations.
3. I have read and agree to comply with CA ANZs
policies and procedures in respect of the Chartered
Accountants Program (CA Program) as may be
notified to me from timeto time.
4. If I have completed the candidate assistance section
of the enrolment form, I have read and accepted
CAANZs Assistance for Candidatespolicy.
5. I accept that CA ANZ may notify the following of any
actual or suspected breach by me of any of the Royal
Charter, By-Laws, Code of Professional Conduct or
Regulation:
a. the organisation appearing in its records as my
current employer; and
b. the individual appearing in its records as my
current mentor.
6. I accept that CA ANZ may notify the following of
my progress and performance in the CA Program
modules:
a. the organisation appearing in its records as my
current employer; and
b. the individual appearing in its records as my
current mentor.
7. I agree that CA ANZ may, at its sole discretion, retain
the original of any module assessment item submitted
by me.
8. I agree that the CA ANZ will not be obliged to return
any assignments, examination answer books or other
materials lodged by me with CA ANZ. If I require access
to these, I understand that, due to the confidential
nature of the examination papers and related
materials, Iwill only be permitted an opportunity to
view them under supervision, and in accordance with
the Access to Assessment Materials Policy.
9. I agree that I will notify CA ANZ promptly if any of my
details change.
10. I understand I may withdraw my CA Program
enrolment, however there is no option to defer,
provide credit or transfer registration to another
provisional member. Upon a cancellation, the refund
amount is dependent on the notification period and
circumstances resulting in cancellation:
a. I will be entitled to a full refund less an
administration fee of AUD$124 in Australia, or
NZD$150 in New Zealand if withdrawing from
a module on or before thedate the module
commences.
b. I will be eligible for a 70% refund of the module fee
if withdrawing from the module on or before the
module Census Date.
c. Requests for refunds outside of these
timeframes will only be considered in exceptional
circumstances and must be supported by medical
or other documentation.
d. No refund is applicable for supplementary exams.
e. The late enrolment fee is a non-refundable
administration fee and is not refundable under
anycircumstance.

13. Privacy Statement

We collect, use and disclose your personal information

for the administration of the CA Program. If you do not
provide your personal information, you will not be able
to participate in the CA Program. We may disclose
your information to agents, contractors and service
providers such as where we outsource functions. By
completing this form, unless you opt-out, you consent
to us also using and disclosing your information for
promotional and marketing purposes. You can opt-out
