J O I N T AFFI DAVI T

We, JOVITA V. PRIETO and RODILYN C. MUSICO both Filipinos, of legal ages, and
residents of Gloria, Oriental Mindoro, respectively, having been duly sworn in accordance
with law, hereby depose and state:

1. That we personally know the person of JAYPEE DELOS SANTOS DE BELEN for a
long period of time, as we live in the same town;

2. That we are not related in any way, either by consanguinity or affinity, to the said
JAYPEE DELOS SANTOS DE BELEN;

3. That we know that he was born on August 17, 1983 at San Fernando, Pampanga.

4. That we know that her parents are Adelo Ogbac De Belen and Rosario Mallari Delos
Santos.

5. That we know that the person JAYPEE DELOS SANTOS DE BELEN with his
parents DATE OF MARRIAGE is November 14, 1981 as indicated in the NSO
Certificate of Live Birth and in the Permanent School Records of San Pedro Central
School in Laguna refer to one and the same person.

6. That we know that her true father is ROBERT BECHABE CRUZ.

7. That from now on she will be using HANNA SUZENNE SORIANO as indicated in
the NSO Certificate of Live Birth on all her legal documents to avoid confusions.

8. As such, we execute this Affidavit to attest to the truth of the foregoing facts and to
explain the discrepancy and for whatever purpose this affidavit may serve.
