TUTORIAL 5

STEP 1
1. Thawaf
An activity of muslim people to walk around kabah
for 7 rounds
2. Fall
Condition in which a sudden loss of consciousness,
and usually temporary caused by lack of blood flow
and oxygen to the brain
3. Agitated
Feels worry, uncomfortable, can be caused by
environmental effect
4. Zam-zam water
Water resource from zam-zam well in arab

STEP 2
1. Why was the old man falling when he did the
thawaf?
2. Why did his body feel hot?
3. What was the purpose of the hajj officer water the
old man?
4. What are the factors of dehydration ?
5. In scenario, the man just drank a little water, is there
relation between drinking little water and fall after
he did physical activity ?
6. What is the relation between dehydration and
hyperthermia?
7. How to prevent the case of scenario?
8. What types of dehydration?
9. Is this scenario is happen hyperthermia, what is the
type of hyperthermia in this case?
10. Does the old man need a medication?
11. What is the difference between fever and
hyperthermia?

STEP 3 (ANALIZE)
1. Why was the old man falling when he did the
thawaf?
- The man falling because of the man lack of water
intake so he got dehydration
- Because he feels fatigue
- Because the mans body is weak and caused by
dehydration and increase of the mans body
temperature
- Caused by high temperature of environment that
makes body temp increased dilatation of
vascular and sweat excretiondehydrationloss
of electrolit (NaCl) make balance of liquid and
blood vena disturbmake disturbance of blood
supply and lack of energyso, the body will
synthesis ATPcollect too much lactate acid
make his body fatigue
Lactate acid :May be because there are many people
there, so the oxygen supply decreased

2. Why did his body feel hot?

- Because his body continuously affected by sun
light
- Caused by mans thawaf activity
- Theres so many people there
- Becaused of he exposed high temperature and
dont replace the fluid heat loss while the body
always produce sweat, so it can decrease the
temperature of the mans body, but the man just
drank little of water.
- The heat is trapped in his body, and he wasnt
sweating, so he got heat stroke not dehydration
- Dehydration caused heat stroke.

3. What was the purpose of the hajj officer water the

old man with zam-zam water?
- to make the old mans body temp decrease
- to comfort him
- to stimulate his consciousness
- because the man got delirium (is condition where a
person has a little consciousness)
- to recovery the mans water loss
- Zam2 content Fluoride and Calsium

4. What are the factors of dehydration ?

loss of water and mineral inside the body because
of sweating
- Hot temperature make sweating
- The man wasnt sweating because of genetic
factor
- So the man was sweating but the sweat evaporate
- Symptom of dehydration : thirst, dry mouth,
increasing body temperature, decreasing of saliva,
and decreasing urinate

5. In scenario, the man just drank a little water, is there

relation between drinking little water and fall after
he did physical activity ?
- In 1 number
6. What is the relation between dehydration and
hyperthermia?
- Dehydration is the symptom of hyperthermia
- Types of hyperthermia:
a. Heat cramp : caused by loss of salt from
sweating
b. Heat exhaustion : caused by dehydration
c. Heat stroke : is combination of released of temp
by evaporation, radiation and convection. It
caused by;
-exercise
-non-exercise
Symptom of heat stroke :
1. High temp
2. Coord. Problem
3. Dry skin
4. Nausea
5. Dark urine
6. Hallucination
d. Malignant hyperthermia
7. How to prevent the case of scenario?
- Consume enough water
- Enough exercise
- Enough nutrition
- Make sure if we do exercise, the room has good
ventilation

8. What types of dehydration?

a. Hypertonic : loss much water than sodium
b. Isotonic : loss both of sodium and water
c. Hypotonic : loss much sodium than water

9. Is this scenario is happen hyperthermia, what is the

type of hyperthermia in this case?
- Heat stroke
10. Does the old man need a medication?
- No. the man just loss water and sodium, so the
man just has to consume much water
11. What is the difference between fever and
hyperthermia?
- Fever :
a. Change in hypothalamic set point
b. Rarely exceed 41 C degrees
c. Complication : dehydration, conflation
d. Caused by pirogen
e. Usually shiver
- Hyperthermia :
a. Failure in thermoregulation
b. Can exceed more than 41 C degrees
c. Caused by the condition of environment
d. There is no shiver, because hyperthermia
caused by failure in thermoregulation not
caused by changed hypothalamic set point
