You have four options to resolve this civil penalty assessment notice:
1. Within ten calendar days of receipt of this Civil Penalty Assessment Notice, you may pay the reduced
total amount shown in the "Total Amount if Paid Within 10 Calendar Days" section. If you choose this
option, you will have acknowledged your liability for all of the violations listed in the Civil Penalty
Assessment Notice and the PUC will accept your payment as payment in full, with surcharge, for all the
violations. If you choose this option, the Civil Penalty Assessment Notice will have been resolved,
without further action by you.
2. Within ten calendar days of receipt of this Civil Penalty Assessment Notice, you may pay the reduced
amount shown in the "10 Calendar Day Penalty + 15% Surchargecolumn for some, but not all of the
violations. If you choose this option, you will have acknowledged your liability for the identified
violations, and this document will convert into a Notice of Complaint to Appear on the remaining
violations on the 11th calendar day after your receipt of the Civil Penalty Assessment Notice. Below,
you must identify the violation number(s) for which you are making payment, and the violation
number(s) that you are contesting. You must then contact the PUC to schedule the hearing on the
remaining violations that you are contesting [303.894.2000 option 4, or 800.888.0170 option 4].
3. You may contest all of the alleged violations identified in the Civil Penalty Assessment Notice or
Notice of Complaint to Appear. If you choose this option, this document will convert into a
Notice of Complaint to Appear on the 11th calendar day after your receipt of the Civil Penalty
Assessment Notice. You must then contact the PUC to schedule the hearing on all of the violations
[303.894.2000 option 4, or 800.888.0170 option 4].
NOTICE: If you fail to pay the prescribed penalty within 10 calendar days after your receipt of the
Civil Penalty Assessment Notice, this document will convert into a Notice of Complaint to Appear.
If you fail to contact the Commission to schedule the hearing by 5:00 p.m. of the 15th day after your
receipt of this Civil Penalty Assessment Notice, the Commission will set the hearing date and notify you
of the hearing date. Upon proof of any violation alleged on the preceding page(s), the PUC may order you
to cease and desist activities in violation of statutes and Commission rules.
4. After this document has become a Notice of Complaint to Appear, you may avoid a hearing in this
matter, at any time prior to the scheduled hearing, if you pay the full total amount shown in the
"Total Amount" section. The PUC will accept your payment as payment in full, with surcharge,
and as your acknowledgment that you are liable for the violations herein.
If you are under 18 years of age, you must be accompanied by a parent or guardian at the hearing.
