2. Written and Verified Petition:

Petitioners date of birth, residence, office address,
phone number, professional tax receipt details, roll of
attorneys number and IBP membership number;
Petitioner is a citizen of the Philippines; over 21 years
of age;
Petitioner is a resident of the Philippines for at least one
(1) year and maintains a regular place of work or
business in the city or province where the commission
is to be issued;
Petitioner is a member of the Philippine Bar in good
standing, supported by clearances from the Office of
the Bar Confidant and the IBP;
Petitioner has not been convicted in the first instance of
any crime involving moral turpitude.
Petitioner has no other application for notarial
commission in any other court, that he will not file a
similar application for notarial commission before any
other Court.
In case of change of address, Petitioner undertakes to
inform the Executive Judge within 10 days of any
change in the previously given address.
3. Clearance from the Office of the Bar Confidant (not
earlier than October of renewal year);
4. Clearance from the IBP (not earlier than October of
renewal year);
5. Proof of payment of filing of the Petition (Php2,020.00)
6. NBI clearance (not earlier than October of renewal
