RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That We, ANDIE M. ILAO, VANDOLPH M. GARCIA, BASILIO

C. TUYOGON, JOSELITO M. GARCIA, AND ALVIN G.
BUSTAMANTE, all of legal ages, and formerly employed with KWM
INDUSTRIES PHILIPPINES, INC., acknowledges receipt of the sum
of _____________________________, Philippine Currency, from KWM
INDUSTRIES PHILIPPINES, INC. in full payment and final settlement
of any kind of compensation due to us or which may be due to us
under the law or under any existing agreement with respect thereto,
as well as any and all claims of whatever kind and nature which we
have or may have against KWM INDUSTRIES PHILIPPINES, INC.,
arising from our contract with (and the termination of my contract
with KWM INDUSTRIES PHILIPPINES, INC.

In consideration of said payment, We do hereby quitclaim,

release, discharge and waive any and all actions of whatever nature,
expected, real or apparent, which We may have against KWM
INDUSTRIES PHILIPPINES, INC., its directors, officers, employees,
agents and clients by reason of or arising from said contract with the
company. We will institute no action, whether civil, criminal, labor
or administrative against KWM INDUSTRIES PHILIPPINES, INC., its
directors, officers, employees, agents and clients. Any and all actions
which We may have commenced either solely in my name or jointly
with others before any office, board, bureau, court, or tribunal
against KWM INDUSTRIES PHILIPPINES, INC., its directors, officers,
employees, agents and clients are hereby deemed and considered
voluntarily withdrawn by us and We will no longer testify or continue
to prosecute said action(s).

We declare that we have read this document and have fully

understood its contents. We further declare that we voluntarily and
willingly executed this Release, Waiver and Quitclaim with full
knowledge of my rights under the law.
