ACKNOWLEDGEMENT RECEIPT WITH AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

I, GENALYN CABRERA MENDEZ, hereby acknowledged the Receipt of

the amount of One Hundred Thousand Pesos (P100,000.00) in Philippine
Currency, in hand paid and receipt is hereby acknowledged given personally by
SIMPLICIA M. PUDE of Cortes, Surigao del Sur;

That the said amount is my debt which is payable within one (1) month or 30
days from the execution of this instrument;

That as my guaranty, I hereby pledge my ATM Card from RCBC with

Account number 9-011-22031-8 which is my Pension from Social Security System
(SSS).
