SMART Goal
Specific: Create a 2-sided, easy-to-read pamphlet that answers the question: How can I prevent or decrease the risks of
acquiring colorectal cancer? Information comes from evidence-based research. Give a 4-6 minute presentation on handout to
audience on 11/30/17.

Measurable: Pamphlet meets the CDCs Clear Communication index/ guidelines for low to moderate literacy level.

Attainable: Scope of project reasonable within timeframe.

Relevant: Colorectal cancer is the second leading cause of cancer deaths in the United States. Screenings such as a colonoscopy
are associated with a 90% cure rate if abnormalities are found.

Timely: Project will be completed and presented to audience by 11/30/17.

Domains
Cognitive: The patient recognizes the potential risk factors of colorectal cancer such as excess red meat intake.
Affective: The patient feels comfortable or less nervous about colon cancer screening such as a colonoscopy and is willing to
receive one.
Psychomotor: The patient makes lifestyle changes that involve increased physical activity as well as diet changes such as
increased fruit and vegetable intake.
