For the Final assessment, each indicator is worth 5 pts. totaling 100 possible points. There is space for comments on pg. 2.

Mid-Term COMMENTS:

Ms. Evans has been doing an amazing job in my classroom so far. She has not only built great relationships with her 2
students but all of my students. They all feel comfortable going to her for help or to share news. She is extremely receptive
to criticism and always asking for feedback. She is always prepared and knows what she must do. She is not afraid to ask
for help or ideas. I suggested that she make sure she has something for the students who may finish their work early and
to make sure everyone is on task. I would like her to work on being more direct with the students when correcting
behavior, but the students do respond well to her.

Final COMMENTS:
Ms. Evans has been doing an amazing job in my classroom. She has built an amazing relationship with all of my students.
They all feel comfortable going to her for help or to share news. She has gained more confidence in her behavior
management and I know she will continue working on it. She definitely spoke up more during this lesson which was great
to see. She is always prepared and knows what she must do. She is not afraid to ask for help or ideas. Their culture lesson
went really well. The students were very engaged during the lesson. The lesson was connected to the standard and
incorporated a great story. She was able to pair up the students in appropriate partners, so the students worked really
well on the activity. The last sheet that they used as an assessment was a little difficult for some of my students. It included
a math skill they were not proficient in yet. Overall, it has been a pleasure having Ms Evans in my class and I wish her the
best of luck in the future. She will make a great teacher one day.
