Chemistry) 2nd Semester School Year 2016-2017 herein referred to as PROFESSOR and BUNGAG,
ENRICO ANTONIO BS Psychology, enrolled in SCI 113 (Organice Chemistry) 2nd Semester School Year
2016-2017 herein referred to as the STUDENT.

Gina Marie M. Cabuhal, signed his graduation clearance and wrote passed in the remarks since the
deadline for submission for their clearance is March 8th provided that he has to pass his final exam and
pass activities from Activity 10 to 15 on March 13, 2017

The terms and agreements are as follows:

The student will:

1. Pass the final exam with a grade of at least 75.

2. Pass the activities required from Activity 10 to 15 on March 13, 2017

In return, the professor will mark his grade as Passed.

The following conditions apply:

1. Should the student fail to comply with the above requirements, the professor will mark his grade as
Failed.

2. Should the student comply with the above requirements, the professor will mark his grade as Passed.
