Ted Deshotels, MS, LRC, CRC, CLCP, Deshotels and Associates


Ted Deshotels owns and operates his own vocational rehabilitation counseling and consulting company which
he began in 1996. Ted earned his Bachelors degree in Psychology and a Master of Science degree in Vocational
Rehabilitation from The University of Louisiana at Lafayette in 1993. Mr. Deshotels is a licensed and certified
vocational rehabilitation counselor. He is also a certified life care planner utilized in assessing catastrophic cases
such as traumatic brain and spinal cord injuries. Teds company provides case management for Louisiana state
workers compensation cases with over 23 years experience in case management in the Louisiana workers
compensation arena.

Luke Lee, M.D., Prime Medical Occupational Clinic


Dr. Lee received his Doctorate of Medicine from The University of Kansas Medical School in Kansas City, Kansas
in 1988. He also earned a Master of Public Health degree in Occupational Medicine from The Medical College
of Wisconsin. After graduation, he completed his occupational medicine specialty training at the University of
Pennsylvania. Dr. Lee now serves as a consultant for various federal and state government agencies as well as
occupational medical director for many prominent corporations. Dr. Lee served as Chief of an Army medical care
clinic, and Senior Field Surgeon in Desert Storm for the 1st Cavalry Division of the U.S. Army. He is also a Certified
Medical Review Officer, Certified Independent Medical Examiner and has also served as an active leader for the
American College of Occupational and Environmental Medicine.

John Rabalais, Rabalais Unland, LLP


John Rabalais is a founding member and managing partner of the law firm Rabalais Unland, LLP. He is a 1987
graduate of Louisiana State University, cum laude, and a 1990 graduate of Tulane University School of Law, magna
cum laude, where he was the 1990 recipient of the Deans Medal. Johns practice focuses primarily on the defense
of complex workers compensation claims in Louisiana. Mr. Rabalais is recognized by many in the insurance industry
around the nation as an authority on Louisiana Workers Compensation law and serves as local counsel for numerous
national entities. In doing so, he has become a forerunner in the development of the fraud defense in Louisiana
jurisdictions and has been a strong force in the development of pro-employer jurisprudence and legislation.

Phil Walls, myMatrixx


As Chief Clinical and Compliance Officer for myMatrixx, a leading workers compensation pharmacy benefit
management firm, Phil Walls oversees its clinical program, including drug regimen reviews, formulary management,
and mail service pharmacy operations. A clinical pharmacist with over 35 years of experience in pharmacy,
healthcare informatics and workers compensation, Phil has particular expertise in pain management, side effects of
medications and managing overutilization. He was instrumental in developing the Get Ahead of the Claim program
designed to mitigate risk earlier in the claim process and provide better outcomes for injured patients while reducing
costs for myMatrixxs clients. Previously, Walls held leadership positions with Health Information Designs and PMSI.
He is a published author and frequent speaker on pharmacy in workers compensation. Phil was the 2011 Pharmacist
Award Recipient in the national Dorland Health People Awards honoring excellence and dedication of healthcare
professionals. He received his pharmacy degree from Mercer University.

Pauline Williams, Second Injury Board, Louisiana Workforce Commission


Pauline M. Williams is the Executive Director of the Louisiana Second Injury Board and Manager of OWCA Finance
& Audit. She has been employed by the Louisiana Office of Workers Compensation since 2007 and has 25 years of
experience in the Insurance Industry. In her current position, she is responsible for all operations of the Second Injury
Board, self-insurance approval and compliance, third party administration provider approval and compliance as well
as WC assessments.
