JOINT AFFIDAVIT OF ARREST

We, PO2 and PO2 both of legal ages, bonafide members of the
Philippine National Police and presently assigned at Digos City Police
Station, after having duly sworn to oath in accordance with law, do
hereby depose and state:

1. That at about 11:00 pm of January 4, 2017, our office received

information from the victims, _______________ and
_________________ who asked to record the incident in the
blotter book that there was a shooting incident at Sweet DG
KTV Bar located at Brgy. San Jose, Digos City;

2. That with a PNP team led by PI ROBERT CARUMBA together

with SPO1, P03, and PO2 we acted on the said information,
after our Chief of Police Supt. _________________ directed us
to respond/proceed at the reported place of incident to verify
the veracity of the said information and if affirmative, to
conduct an ocular investigation;

3. That, upon arrival at the place of incident, we were personally

informed by the witness Carla that she knows the identity of
the suspects because they were regular customers in their
place and that we were informed likewise that the suspects
escaped after the incident with firearm and used two
motorcycles as get away vehicles;

4. That we immediately informed our officer/unit confirming this

incident; thereafter, in coordination with PNP elements of Sulop
Police Station, we together with Carla, who witnessed the
incident and volunteered to help us, proceeded at the
residence/ house of the suspects at Brgy. McKinly, Sulop Davao
del Sur; and that upon arrival thereat, our witness positively
identified the suspects who were in their residences/houses;

5. That immediately introduced ourselves to them (suspects) and

arrested the same; and that we likewise informed them the
reason of their arrest and of their constitutional rights; and the
 firearm used was recovered from the possession of the suspect
________________________;

6. That the suspects submitted themselves to us peacefully; and

further identified themselves as _______________________,
__________________________, and _______________ all
residents of McKinly,Sulop, Davao del Sur; and immediately
brought them to our office/unit for investigation and proper
disposition; and the firearm described as cal. 38 revolver with
one bullet in the cylinder and the Two motorcycles described as
SkyGo Wizard with no plate number attached and color red
Honda XRM with plate number LA70545. That I (affiant PO3
______) marked the confiscated cal. 38 revolver as “RLB’.

7. That we executed this affidavit to attest to the veracity of the

aforementioned facts and to support the filing of appropriate
charge/s in court against the arrested suspects.
