P E T I T I O N

UNDERSIGNED PETITIONER, unto this Honorable

Judge, most respectfully stated:

1. That he is a Filipino Citizen, of legal age having

been born on September 30, 1995, single, with residence and
office address at Poblacion, Ubay, Bohol within the court’s
jurisdiction. His personal and office number is 09109939370.

2. That he has been admitted to the practice of law in

the Philippines on May 23, 2017 under Roll of Attorneys No.
69355. Attached herewith as Annex “A” is a certified true copy
of his Certificate of Admission to the Philippine Bar as duly
issued by the Supreme Court of the Philippines;

3. That he is of good moral character and has not been

criminally charged, much more convicted, of any crime
involving moral turpitude and he has no pending

administrative case before the Supreme Court’s Office of the
Bar Confidant. A certified true copy of his Certificate of Good
Standing issued by the Bar Confidant is attached as Annex
“B”. In addition, two (2) executive officers of the local chapter
of the Integrated Bar of the Philippines (IBP) in Bohol have
vouched for his good moral character as confirmed by a
certified true copy of a certification encloses as Annex “C”;

4. That he has paid his IBP annual dues for 2017 as

shown by a certified true copy of the IBP receipt enclosed as
Annex “D”. He is a member of the IBP in good standing. A fact
which the IBP Confirmed by the enclosed certified true copy of
a certificate issued in his favor marked as Annex “E”. He has
also paid the required professional tax for the year 2017 as
shown by a certified true copy of Official Receipt No. 2566412
dated June 5, 2017 which is hereto attached as Annex “F”;

5. That he has all the qualifications and none of the

disqualification for the issuance of a commission as a Notary
Public and that he has no other commission as Notary Public
in any other jurisdiction;

6. That as further proof of his genuine desire to

become a Notary Public, he has attached as Annex “G” three
(3) specimen of his official signature;

7. Additionally, he has attached in a separate paper

marked as Annex “H” for the court’s ready reference three(3) of
his passport-size color photographs bearing signature at each
of the bottom parts;

 8. Finally, he has paid the required filing fee as shown
by the attached receipt marked as Annex “I”.

PRAYER

WHEREFORE, PETITIONER RESPECTFULLLY PRAYS THAT

A COMMISSION AS NOTARY PUBLIC BE ISSUED.

after being sworn in accordance with law, hereby depose and

That I am the Petitioner in the above-entitled case; That I

have caused the preparation of the above Petition and I have
read the same and knows the contents thereof; That the
allegations contained therein are true and correct of my own
personal knowledge.

That I further certify that: (a) I have not theretofore

commenced any other action or proceeding or filed any claim
involving the same issues or matter in any court, tribunal, or
quasi-judicial agency and, to the best of my knowledge, no such
action or proceeding is pending therein; (c) if I should thereafter
learn that the same or similar action or proceeding has been
filed or is pending before the Supreme Court, the Court of
Appeals, or any other tribunal or quasi-judicial agency, I
