Praise and gratitude we pray for the presence of Almighty God, because thanks to
the abundance of His Mercy and Grace so that we can arrange this paper smoothly and on
time. This paper was prepared to fulfill the task "ADMISSION TO A HOSPITAL".
This paper has been made based on various sources and some help from various
parties to help resolve this paper. Therefore, we would like to thank all those who have
helped in the preparation of this paper.
We realize that there are still many fundamental flaws in this paper. Therefore,
we expect readers to give suggestions and criticisms to build. Criticisms and suggestions
readers so we expect to further refinement of paper.

CHAPTER I
INTRODUCTION

1.1 Beaground
Admission to the hospital can happen in various ways. You family member may be
treated in the Emergency Room (ER) and need additional treatment requiring a hospital stay.
Other times you may know that your family member will be staying in the hospital for at least
one night. This planned admission could be for elective (non-emergency) surgery, tests, or
special procedures. Whether it starts as an emergency or as a planned admission, a hospital
stay is often the first stage in a series of transitions, or moves to different health care settings.
You and your family member will feel more prepared and perhaps less anxious when you
know what to expect. If you are helping your family member plan for a hospital admission,
start by reading the first section of this guide

DISCUSSION

2.1 Understanding
Acceptance of new patients is a way of accepting the arrival of new patients in a
room. In the reception of new patients delivered several things about the orientation of the
room, care, medical, and room order.

2.2 Stages of New Patient Acceptance


1. Pre-admission phase of new patient
a. Preparing administrative completeness
b. Prepare room fittings as per order
c. Setting up a new patient admissions format
d. Setting up the review form
e. Prepare informed consent centralized / drug management.
f. Set up the nursing kit
g. Prepare the patient's order sheet and the room visitor
h. Preparing the patient's rights and obligations
i. Setting up the card

a. The patient came in the room received by the head of the room / team leader /
nurse who was given the delegation.
b. Nurses introduce themselves to clients and their families.
c. The nurse shows the client's room / bed and takes it to a designated place.
d. Nurses with other employees move the patient to bed (if the patient comes with a
branchard) and provide a comfortable position.
e. Nurses conduct assessment of patients according to the format.
f. Introduce new patients with new patients in the same room.
g. After the patient is calm and the situation allows the nurse to provide information
to the client and family about the orientation of the room, treatment (including
responsible nurse and drug centralization), medical (responsible doctor and visite
schedule), and room order.
h. The nurse asks again about the clarity of the information that has been submitted
i. If the patient or family is clear, then it is required to sign informedconcent of drug
centralization.
j. The nurse handed over the patient the patient's satisfaction questionnaire sheet.

2.3 The role of the nurse in receiving new patients

2.4 Analyze patient date

Patient data records are very important to record the identity, diagnosis of the
disease, therapy already given and know the progress of the disease. Data of note
include:
1. Patient identity, including :
 Patient's name: we recommend full name instead of nickname or alias.
 . Sex: as completeness must also be written data
 Age: especially important in pediatric patients as it is sometimes used to
determine drug doses. Can also be used to estimate the possibility of
illness suffered, some diseases typical for a certain age.
 Address: if the patient is frequently moved around then ask not only the
present address but also the address at the time the patient feels sick
for the first time. This data is sometimes necessary to determine the
occurrence of outbreaks, endemic diseases or to epidemiological data
diseases.
 Occupation: if a doctor suspects there is a connection between the
patient's illness and his job, then ask not only the present job but also
the previous jobs.
 Marriage: sometimes useful to know the patient's psychological
background.
 Religion: this information is useful to know what is allowed and should
not (abstinence) a patient according to his religion.
 Ethnicity: related to certain habits or diseases related to certain racial or
ethnic groups.

2. Family identity, including: position in the family, occupation and residence
address. Some diseases are closely related to occupation / residential location.
Make the patient's data in a special paper (patient status) and Patient Register
placed on the shelf to make it easier for the patient to control or continue
therapy. Make cards and registration numbers so that patients can order well.

The purpose of doing anamnesis (interview) is to know the purpose of treatment
patients, as well as deepen the disease and complaints experienced. A true and
complete history is able to diagnose the disease up to 80%. What are we asking?
1. The main complaint, namely the complaint that causes a person to seek
treatment for dibekam. Such as headaches,
2. Additional complaints (complaints accompanying), namely other complaints
that accompany the main complaint, such as headache complaints are
accompanied by stiffness in the neck, blurred eyes and so forth.
3. Past medical history, ie diseases that are still associated with current
complaints, such as 2 years ago had fallen and head bumped, or similar
headache complaints accompanied by hypertension, and others. Likewise, the
history of allergies and inherited diseases such as diabetes is also asked.
Conduct examination and determine Diagnosis of disease
This examination is useful to prove what the patient complained in accordance with
the existing physical abnormalities. Sometimes patients complain about something
but no physical abnormalities are found and vice versa. The physical examination is
as follows:
1. General examination, including: blood pressure, pulse, body temperature,
breathing, iridology, palmistry and others. The most important thing is to be
able to know the disease, may be by medical diagnosis or traditionally or a
combination of both.
2. Inspection (Observation), hearing and smell from the organs that complained
of the patient. Note the discoloration of skin, shape, texture or other visible
changes. Also observe facial expressions, forms and attitudes as well as how to
walk the patient.
3. Palpation (Tactile, compressing) or percussion (tapping) around the body of the
complaint. Check if there is a hard / soft lump, or with emphasis if pain
indicates the disease including hyper (over function) and if the patient's
pressure feels good means the disease includes hypo (malfunction). Likewise
with the organ tap on whether there is a change, such as the lungs that should
sound sonor, in certain circumstances turned into deaf because there is a lung
tumor. Sometimes we need to move sick parts of the body, whether there are

limitations of motion on the hands / feet, stiffness, pain when moved and
others.
4. Auscultation, the examination by using a stethoscope to determine the
abnormalities in the chest cavity (heart and lungs) and abdominal cavity
(stomach, intestine, etc.).
5. If necessary do the investigation, such as blood lab, urine and stool, X-ray
(radiology), ECG, CT-Scan, MRI and so on.
Having known the complaints through anamnesis and has been done the
examination it can be taken conclusions about the disease experienced by the
patient (diagnosis). Diagnose this disease as a basic capital to determine the
next step on what type of therapy is suitable to do, which cupping points will
be selected and what support herbs that are needed.

3.1 CONCLUTION

When patients come to the hospital, they must fill in the identity sheet and answer
some general questions from the nurse.
