This is to inform you of some changes of the schedule of duties of the practicum/ intern in
different government agencies. This is in accordance with the objective of the internship and
University calendar of activities.

Please follow the schedule, so that the practicum will not have a delay in submission of the
needed requirements for the final grade and as graduating students.

January 13, 2017 End of duty at DOST

January 17 – 20, 2017
January 23, 2017
February 28, 2017
March 1 – 3, 2017
March 8, 2017
March 8, 2017
Preliminaries at the BFAR (Orientation, MOA, Interview, etc)
Start of duty at BFAR
End of duty at BFAR
Submission of OJT documents for adviser review
Submission of OJT documents to the office
Submit Grade of the Student

Documents to be submitted in the office

- Photocopy of OJT Requirements
- Original copy of Daily time record (DTR) in all agencies and academe with signature
- Original copy of OJT journal, documentation with adviser signature
- Original copy of Evaluation forms with signature
- Photocopy of Certificate of completion signed by the company (indicating the duty
assignment of the intern)
- DOST and BFAR Memorandum of Agreement Signed and Notarized

Failure to submit the necessary documents on time, the office will not approved the grade of
the students and will delay their graduation requirements.
