PDOS ADVISORY NO. ~4

TO ALL PDOS PROVIDERS:

In line with Special Order No. 180, series of 2014, on the assignment of
personnel in PDOS Development and Monitoring Unit (PDMU)/creation of PDOS
Monitoring Team (PMT) and Memorandum of Instruction No. 002, Series of 2015, on
the Procedure and Guidelines for Monitoring the Compliance of PDOS Providers, you are
instructed to submit the following:

1. your monthly PDOS schedule; and

2. PDOS reports using the prescribed form (see Annex A).

Submission must be done every 1st day of the month. If it falls on a holiday,
Saturday or Sunday, please submit the same on the following working day to
owwa pdos@yahoo.com. Transmittal letter shall be addressed to the Director, PPDO
through the Chief of ASMD/PDMU.
