AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I, Laarni Joy J. Conadera, of legal age, Filipino, and a resident of Poblacion, Initao, Misamis
Oriental , after having been sworn in accordance with law, hereby deposes and states the following:

1. That sometime in June 2011, RAD TRAVEL AND TOURS have started its business
Operation in Bulua, Cagayan de Oro City. However, due to its inability to generate business profit or
very low profit, it has stopped sometime in September of the same year;

2. That I am the true and lawful attorney-in-fact of Vel Rad M. Jarales, Owner / Proprietor
of Rad Travel and Tours;

3. That being given the above-mentioned position, it is my responsibility to receive and

keep in my custody Official Receipt Booklets, Original Certificate of Registration, Book of Accounts of
RAD TRAVEL AND TOURS;

4. That, upon it’s closure, said documents have been turned-over to the business’
Bookkeeper;

5. That, I have recently discovered the said item/s is/are irretrievably lost and can no
longer be found despite diligent search and efforts to locate the same; that I now believe that it is
lost beyond recovery;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and
for whatever legal purposes that this may serve.

I am writing to request an abatement of penalties of Rad Travel and Tours. The reason why I failed
to report income was because the business have stopped its operation sometime in September 2011
due to very low profit.
