Each Foundation Year 1 post will rotate through three individual specialties, spending a total
of four months in each specialty.
There are currently two Foundation Programme Directors, Dr Kim Sinnamon (Renal
Consultant) and Dr Duncan Field (Consultant Cardiologist). Part of their role is to ensure the
Foundation teaching programme is relevant to the current Foundation curriculum as well as
giving support and guidance to the Foundation Trainee.
This post, as a LAS, will not be recognised for training and will not lead onto a F2 post.
However, the post-holder will be included and receive the same opportunities as the F1
trainees.
2.0 Post and Job Summary
JOB TITLE: Whole-Time Foundation Year 1 LAS Doctor
Clinical
The post holder’s duties will be primarily at the Colchester General Hospital but their
presence may from time to time be required in other parts of the Trust. From August 2015
F1 trainees may have placements in Primary Care and in the North Essex Partnership NHS
Foundation Trust.
The Post
The Foundation Year 1 doctor will have the principle responsibility of treating patients under
the supervision of senior colleagues in a variety of specialties according to their rotation.
The post-holder will take part in the appropriate rota for each specialty. In addition, even
though the post is not recognised for training, the post-holder will be expected to attend
teaching sessions and will complete a log-book of specified competencies and experiences.

The Trust is divided into five Divisions, Medicine Division, Urgent Care, Women’s and
Children’s Division, Cancer and Clinical Support Services Division and Surgery Division.
Each Foundation Year 1 post will rotate through three different specialties on pre-determined
rotations with the post-holder spending four months in each specialty. The majority of posts
on the training programme are within the four Divisions but from August 2015, there are
some placements outside the Hospital Divisions.
4.0 Divisions and Departments on F1 Rotations
MEDICAL DIVISION
There is a Medical Assessment Unit based in the main Colchester General Building.
The post-holder will participate in a full shift rota within EAU. When on-take the team is
responsible for ALL acute medical admissions.
Paediatrics
This is a busy medium sized paediatric unit which provides high quality training and
experience in all aspects of child health. Trainees will get exposure to a variety of acute and
ambulatory paediatrics.
The consultant paediatricians all undertake general paediatrics and also carry out clinics in
the following areas: Rheumatology, Diabetes, Neonatology, Respiratory, Allergy, Oncology,
Cardiology, Nephrology, Child Protection, Child Development and Neurodisability.
The F1 will spend approximately 3 months in acute general paediatrics including the
children’s ward and assessment unit, one month in the neonatal unit and be responsible for
cover of the postnatal wards. The training aim is to provide experience suitable for a
subsequent rotational post at F2 level. Under supervision, the post-holder will be expected
to gain experience in the following skills:
Clerking and examination of children of different ages with particular attention to
establishing a rapport with the child, which is appropriate for their developmental age.
There is senior review of all children for admission/assessment
Assisting with the management of children on the ward and day unit. This will
include some experience of practical skills
Participation in ward rounds on the children’s ward, allowing the opportunity for case
presentations and discussion
Assisting with management of children in the Accident and Emergency Department,
including paediatric resuscitation and management of paediatric emergencies
Attend at least 6 general and specialist out-patient clinics in 4our months
Undertake an audit report
Opportunity to spend time on the neonatal unit, learning to examine neonates and
assisting in their management (dependent upon the career intentions and training
needs of the post-holder)
This is a Monday to Friday, day duties only post.
Obstetrics and Gynaecology
The department has a broad mixture of consultants in obstetrics and gynaecology with
interest in pre-natal diagnosis, ultrasound, gynaecological oncology, urogynaecology,
infertility and family planning.
The trainee will be exposed to a broad range of obstetrics and gynaecology. Day to day
duties include:
Obstetrics – daily assessment of antenatal and postnatal women under the care of
your team. Attendance at antenatal clinics
Gynaecology – daily assessment of gynaecological patients under the care of your
team. Care of booked gynaecology patient pre and post-operatively. Attendance at
theatre and clinics
Informal teaching – of junior medical and nursing colleagues as well as
undergraduate students from UCL/Whittington Hospital and Bart’s
Ensuring that electronic discharge summaries are written for patients discharged
from hospital

SURGERY
Critical Care/Anaesthetics
There is a combined ITU/HDU with 14 bed spaces. Consultant cover is provided at all times
and there is a broad range of patients. It is a busy unit and trainees will learn about all
aspects of critical care.
The Anaesthetic posts will give a good overview of the role of the Anaesthetics department
including theatre, CDS and ITU. Experiences and competences would include:
How to identify high-risk patients
Recognise organ impairment
Prioritise treatment and organ system support
Understand importance of communication between medical teams and other health
professionals
Learn when treatment becomes futile and limitations of therapy
Develop practical skills including venous cannulation, catherisation, basic airway
management
Rapid interpretation of data e.g. ECG, CXR, Blood Gases
Importance of infection control techniques and appropriate use of antibiotics
Speaking to relatives and breaking bad news
Good record keeping
Principles of acute pain management including epidurals and PCAs
The post-holder will participate in a full shift rota.
The post-holder will participate in a full shift rota. Whilst attached to Plastic and Breast or
Urology the F1 will take part in the general surgery rota.
Teaching will be based around the Foundation Programme curriculum and initially will take
place for one hour in the lunchtime of one day per week. This timing may change through
the course of the year. Through the course of the year assessment as outlined in the
curriculum will be introduced to give trainees opportunities to experience these techniques
which they will have to complete during F2.
Each F1 will be expected, during the course of their year’s training to have a logbook
completed with competencies and experiences. Progression onto the F2 Programme will
not be possible without satisfactory completion of the specified competencies and
experiences.
ADMINISTRATION
The post-holder will be jointly responsible for the accurate and timely completion of clinical
notes and GP communications.
A Foundation Year 1 trainee has a leave entitlement of 27 days per annum. This should be
divided evenly between each training placement (i.e. 9 days each four months).
Annual leave requests should be planned and advised within two weeks of initially
taking up your post
Six weeks’ notice of leave must be given therein – any request for leave without this
period of notice may be refused
Leave request forms must be submitted for approval to the appropriate Consultant,
Clinical Director and Service Manager.
Leave is not granted unless/until an annual leave request form has been approved by
the Consultant/Service Manager/Clinical Director
Leave requested for the first or last week of your appointment is not usually approved
If cross-cover arrangements exist within your specialty, it is the responsibility of the
doctor taking leave to discuss cover arrangements with their cross-cover colleague
and obtain appropriate agreement / signatures before requesting leave.

be absent at any one time, leave when on the on-call rota etc.

7.0 Training and Postgraduate Education
General
There is an excellent Postgraduate Medical Centre with a lecture theatre. The centre
includes seminar rooms and a small but extensive library with very helpful staff on a site
adjacent to Colchester General Hospital.
The post-holder will be required to participate in programmes for teaching medical students.
The post-holder will attend relevant mandatory training including the Trust’s ‘At Our Best’
programme.
Post Specific
Teaching is provided in all departments throughout the rotations. Although this post is not
recognised for Training the successful post-holder will take part in formal teaching.

Study Leave
Study leave will be granted in accordance with the Terms and Conditions of Service for
Hospital Medical and Dental Staff, subject to local conditions.
Private study leave will only be available for a few days before an exam, and then only at the
discretion of the relevant unit training director.
8.0 General Terms and Conditions and Information
On appointment as Foundation Year 1 LAS Doctor a medical practitioner shall be eligible for
full registration with the General Medical Council.
In addition to basic salary, a supplement will be paid for hours worked in excess of a 40 hour
basic week. This supplement is pre-determined by the rota worked within each specialty
and may vary with each specialty placement.
The post is covered by the Terms and Conditions of Service of Hospital Medical and Dental
Staff (England and Wales).
Candidates will be required to complete a Health Statement and the Trust may require an
officer to pass a medical examination as a condition of appointment.
Because of the nature of the work of this post it is exempt from the provisions of Section 4(2)
of the Rehabilitation of Offenders Act, 1974 by virtue of the Rehabilitation of Offenders Act
1974 (Exemptions) Order 1975. Applicants are, therefore, not entitled to withhold information
about convictions, including those which for other purposes are "spent" under the provisions
of the Act, and, in the event of employment, any failure to disclose such convictions could
result in dismissal or disciplinary action by the Trust. Any information should be sent to the
Trust Medical Director in an envelope marked for his/her personal attention and will be
completely confidential and be considered only in relation to an application for positions to
which the order applies.
‘The Trust is committed to providing safe and effective care for patients. To ensure
this, there is an agreed procedure for medical staff that enables them to report,
quickly and confidentially, concerns about the conduct, performance or health of
medical colleagues’ (Chief Medical Officer, December 1996). All medical staff
practising in the Trust should ensure that they are familiar with the procedure and
apply it.
Medical Negligence
Co-operate fully with The Trust and its Legal Advisors in the investigation of any Patient
Complaint/Incident involving but not limited to any allegation of negligence or misconduct
on the part of the doctor.
To provide The Trust, on request, with a full written statement concerning the said
Patient Complaint/Incident.
Clinical Governance
The post-holder will participate in clinical governance activities, including clinical audit,
clinical effectiveness, risk management, quality improvement activities as required by the
Trust, and external accrediting bodies.
Management
The post-holder will be required to work within the Trust's management policies and
procedures, both statutory and internal, accepting that the resources available to the Trust
are finite and that all changes in clinical practice or workload, or developments requiring
additional resources must have prior agreement with the Trust. He/she will undertake the
administrative duties associated with the care of his /her patients under the direction of the
consultant.
Infection Control
Infection Control is everyone’s responsibility. All staff, both clinical and non-clinical, are
required to adhere to the Trusts’ Infection Prevention and Control Policies and make every
effort to maintain high standards of infection control at all times thereby reducing the burden
of Healthcare Associated Infections including MRSA.
All staff employed by the Trust have the following key responsibilities:
Staff must wash their hands or use alcohol gel on entry and exit from all clinical areas
and/or between each patient contact.
Staff members have a duty to attend mandatory infection control training provided for
them by the Trust.
Staff members who develop an infection (other than common colds and illness) that may
be transmittable to patients have a duty to contact Health & Wellbeing Department.
It is expected that as the organisation develops and changes, it may be necessary to vary
the tasks and/or the responsibilities of the post-holder. This will be done in consultation with
the post-holder and it is hoped that agreement can be reached to any reasonable changes.
Confidentiality
The post-holder must at all times maintain complete confidentiality of the material and
information that they handle. Any matters of a confidential nature, or in particular, information
relating to diagnoses and treatment of patients and individual staff records must not, under
any circumstances, be divulged or passed on to any unauthorised person or persons. The
post-holder must respect patient named confidentiality in keeping with “Caldicott principles”.
Data Protection
The post-holder must at all times respect the confidentiality of information in line with the
requirements of the Data Protection Act. This includes, if required to do so, obtaining,
processing and/or using information held on a computer in a fair and lawful way, holding
data only for the specified registered purposes and using or disclosing data only to
authorised persons or organisations as instructed.
General
The duties and responsibilities described in this Job Description are intended to be indicative

requirements of the job may change and the post-holder is expected to adapt to these

Employees must share responsibility for abiding by health and safety policies and

for the prevention and control of healthcare associated infection (available on the Trust
