Subject: Fundraising Campaign YOUR HEART CLINIC NEEDS YOU

Sir / Madam,

The Friends of the Stella-Maris-de-Kent Hospital foundation are seeking your support for a
cardiology clinic. Founded in 2009, the Friends of the Stella-Maris-de-Kent Hospital foundation
contributes to the improvement and development of services offered at the hospital and is
committed to creating a partnership to foster a sense of belonging to the three different cultures
of its rural community. The Stella-Maris-de-Kent Hospital was built by the Notre-Dame-du-
Sacré-Cœur Sisters in 1966. It serves a population of over 30,000. The hospital holds 20 beds
and offers an emergency service open 24/7 More than 26 doctors, 120 employees see the well-
being of our community. The emergency service receives more than 35,000 visits per year.

Many people from the region travel every week for a visit to the cardiology clinic at the Dr.
Georges-L.-Dumont University Hospital Center. Our project aims to welcome more than 1,200
patients per year to Stella-Maris-de-Kent Hospital, which will benefit patients in our region by
improving access to care and services.
The total cost of the project is approximately $ 550,000 and includes renovations, the purchase
of equipment and the hiring of new staff. The required investments will be shared between the
Vitalité Health Network and the Friends of the Stella-Maris-de-Kent Hospital foundation. The
Network will be responsible for paying the renovation costs, as well as those related to the
normal day to day operations, such as employee salaries. The foundation is mandated to acquire
the new equipment - an examination stretcher, a treadmill and a cardiac ultrasound machine. The
cots for this new equipment is nearly $ 200,000.

A fundraising campaign was announced on June 1, 2017 to raise funds for the purchase of new
equipment. "We are counting on the generosity of the population to be able to pay for the total
amount of equipment. I am confident that we will achieve our goal”, said Mr. Adélard Cormier,
President of the Friends of the Stella-Maris-de-Kent Hospital foundation. The outpatient
cardiology services will be in operation in early 2018 and will be offered to the population three
days a week, but with the possibility of adjustment as needed. Dr. Jean-François Baril, a
cardiologist, believes that "these new services will bring happiness to patients, since they will
have access to quality exams thanks to the new state-of-the-art devices, and this, closer to their
home.

You will find in the Annex the commitment protocol and the donor privileges.

Hoping for a favorable response to our request, please accept, Sir / Madam, our most
distinguished greetings.

Adélard Cormier Joey Caissie
