unit, dated January 17, 2018, RE: Requesting for a resolution authorizing the
Hon. Mayor Erlinda D. Lim, to enter into and sign for and in behalf in the Municipal
Government of Lupon, Davao Oriental the Memorandum of Agreement (MOA) with
the University of the Philippines Manila (UPM) and its National Telehealth Center
(NTHC) for the implementation of the Electronic Medical Record System in Lupon
Municipal Health Station.

Philippines.”

the Davao Oriental Provincial Hospital – Lupon (as the Other Referral Facility of the
