CNS MALFORMATION & INFECTION
Neurological examination of newborns and infants is based on the same scheme that is
used for older children and adults:
1) CN
2) Motor function
3) Reflexes
4) Sensation
5) Meningeal signs
It should be noted that neurological status of the newborns has some special features. The
mainstay of investigation is the analysis of congenital reflexes which reflect the maturity if CNS,
viability of the newborn and adaptation potential.
The following conditions should be fulfilled during examination: comfortable room T (25-26),
and it is not recommended to examine the child immediately postprandial or before feeding because
feeding determinant can change the level of his congenital reflexes.
It should be noted that congenital reflexes of newborns are very labile and are easily
fatigued. One should elicit reflexes quickly and turn the child only after cranial and facial skeleton
and cranial nerves have been already tested.
It is important to look for dysembryological stigmata, which can be frequently encountered:
abnormal earlobes (low amount of helixes, absence of earlobes, elongated upper part – rabbit's ear,
low set ears), wide nose bridge (hypertelorism), prognatism, low hair border.
Much attention should be paid to the head shape: dolicocephaly, brachiocephaly, tower-like
skull, left or right inclination are all variants of normal skull. Sometimes head deformities are caused
by cephalohematoma or birth swelling. Associated asymmetry of head and face bones suggests
intrauterine pathology most frequently of embryonic stage of development.
HC should is measured. HC varies significantly from 33 to 37.5 cm. Analyzing HC one
should take into consideration height, weight and HC to chest diameter ration. At birth HC should not
exceed chest diameter by more than 1-2 cm. Average HC in a newborn whose weight=3000-3500 g;
height=50-52 is 36 cm. In newborns HC may actually be less because of overriding of skull sutures.
HC should increase by 4 cm in first 3 month, i.e. HC at 3 month = 40 cm. In first month HC
increases by 2 cm, in second month by 1.0-1.5 cm and in third one by 1.0 cm. During the next 3

month HC increases by 3 cm and at 6 month = 43 cm and at 1 year =46 cm (i.e. +3 cm within the
next 6 month).
During the first year of life HC increases by 10 cm.

Frontal fontanel should be palpated, its size should not exceed 2.5 x 3.0 cm. Coronal and
saggital sutures should be palpated as well. Coronal suture is normally closed at birth. Saggital
suture is sometimes open at birth but should not be wider than 0.5 cm.
When tense and bulging anterior fontanel, or opening of the saggital suture is for more than 0.5
cm is present one should suspect intracranial hypertension. Other symptoms suggestive of
intracranial hypertension are:
a) Tower-like skull with high forehead (Socrat's head)
b) Dolichocephalic skull with prominent occipit. Child tends to hyperextend his neck (due to
increased tone of extensors)
c) Wrist tremor with tendency to finger extension
d) Grefe's sign
e) Setting sun sign
The latter two signs should be distinguished from each other in spite of the fact that they have
similar presentation: Grefe's sign is a band of white sclera above the iris when eyeballs are not
moving. In the absence of other symptoms Grefe's sign is not indicative of intracranial hypertension.
It is frequently seen in premature children with exophthalmosis and hypotrophia.
Setting sun sign is elicited by repeated head nods. It presents as a down deviation of eyeballs
and a white band of sclera appears above the iris. If intracranial hypertension is pronounced and
when hyperbilirubinemia is present (especially in premature babies) this sign sometimes appear
spontaneously without a head nod. It originates from tectum of mesencephalon and signifies local
hypertension in the 3rd ventricle and Sylvian aqeduct, irritation of the 3rd cranial nerve nucleus and its
connections to the 8th nerve.
Cranial nerves

1st: In response to any strong odor the child wrinkles his nose, shuts his eyes and
sometimes sneezes.
2nd: Light makes the newborn to shut his eyes. If this is not seen optic nerve damage should
be suspected and ophthalmologist consultation is essential.
3, 4, 6th: Width and symmetry of eyes should be checked. Bilateral ptosis present at birth
signifies embryonic defect (underdevelopment of oculomotor nucleus) which can have genetic
cause.
Posterior longitudinal fasciculus is not developed fully at birth, that is why newborns
frequently do not demonstrate coordinated eyeball movements and sometimes have short periods of
esotropia and nystagmic movements. Pronounced esotropia is a sign of intracranial trauma. Marked
nystagmus is a sign of poor vision or blindness especially if wide pupils and absence of eye closing
at light are seen.
By the end of the first month of life when cortex and its interconnections between frontal,
parietal and occipital lobes are present children start to concentrate in response to a sound or a light
sources following the object with their eyes. Insufficient of absent concentration reaction can be
caused by abnormal psychic development secondary to intrauterine or intranatal damage of cortex or
damage of visual or acoustic nerves. Exo- eso or microphthalmos can be noted (microphalmos is
characteristic of infectious fetopathies: toxoplasmosis, CMV, etc). Horner's sign can be present. It is
usually caused by irritation of cervical sympathetic ganglion in peripheral arm paralysis, TB infection.
Pupils react to light since birth. Examiner should note shape of the pupils. Normally pupils
are round and symmetrical. Irregular form should raise suspicion of coloboma of cataract (defect of
embriologic period).
Epipsoid pupils, especially if paradox reaction to light is present (Argyl-Robinson reaction)
can be a sign of congenital syphilis. Characteristic atrophy seen on fundi (salt and pepper) should
further increase suspicion of congenital syphilis even if VDRL is negative.
5th: Trigeminal nerve function is important for sucking (together with 7 th and 12th nerve).
Normally mandibule is in tight contact with maxilla and is symmetrical. Low position of mandibule,
food falling out, absence of smaking sounds indicates pathology (trauma or underdevelopment) of
the motor nucleus of the trigeminal nerve. Difference in sweating and color of half of the face
(Harlequin’s sign) should raise the suspicion about sensitive branch involvement. Combined
pathology of the motor branch of the 5th nerve and the facial nerve is characterized by a group of
congenital reflexes of oral automatisms, which will be discussed later.

7th nerve: one should differentiate congenital face asymmetry and nerve damage.
Asymmetrical face muscles are usually seen along with bone asymmetry: both facial and skull
bones. Nasolabial folds are notable only in hypotrophic children, therefore, to diagnose central facial
paresis (which is characterized by paresis of lower facial muscles) ipsilateral hemiparesis should be
sought.
Only the presence of hemiparesis (sometimes, mild) plus facial muscle asymmetry allows to
diagnose central facial nerve palsy. In central palsy the angle of the mouth is depressed and the lips
are thin at the side of damage. If the damage is extensive scout reflex is weak at the ipsilateral side,
when crying mouth is displaced to the undamaged side. Peripheral facial palsy is easy to diagnose.
This type of palsy can be caused by intrauterine pathology (genetic or exogenous) or more
frequently by birth trauma when newborn's head stays long in the birth canal, facial presentation or
obstetrician-assisted delivery can be other causes. Clinical picture is similar despite multiple
etiologies: eye does not close fully at sleep, sometimes lagophthalomosis is present; when crying
asymmetry of palpebral fissures with more opening at the injured side, deviation of the mouth to the
uninjured side are noted. At the injured side food falls out of the angle of the mouth and scout reflex
is weak. Sometimes newborns have alternating Miiar-Gubler's syndrome: peripheral palsy of the
facial nerve and contralateral hemiparesis. This syndrome is characteristic of the intranatal
hemorrhage in the pons.
8th. At birth the normal child reacts to sounds: loud noise cause him to shut his eyes. Eye
closing is a universal reaction when checking peripheral analyzers: olfaction, vision, and acoustic. If
there is now eye closing to loud sounds acoustic abnormalities due to embryologic or fetal period
injury should be suspected. Sometimes loud sounds cause the child to shudder or abduct his arms
(first phase of Moro's reflex). Abrupt shuddering (the so-called start reflex) can signify low seizure
threshold (irritation of brain-stem structures with reflex arc closing at the level of tectum of
mesenchaphalon.
Vestibular portion of the nerve can be assessed by several congenital reflexes: Moro's
reflex, cervical-tonic reflex, tonic labyrinth reflex, and protective reflex. All them involve connections
of vestibular nerve and reticular formation, accessory nerve and anterior horns of cervical and
lumbar intumescences.
9-10. One of the most important functions of these nerves is swallowing which is performed
by the muscles of pharynx and soft palate innervated by a common motor nucleus of these nerves.
Pathology of development of this nucleus makes child non-viable. As soon as feeding via NG tube
stopped the child dies because of absence of swallowing reflex and swallowing itself.

When different intrauterine abnormalities of brain are present due to circulation problems
intranatally, children sometimes demonstrate clinical picture of nuclear pathology (the so-called
bulbar palsy) or supranuclear damage of corticonuclear pathways leading to the above mentioned
nucleus (pseudobulbar palsy). It is very difficult to differentiate those conditions in newborns but the
correct diagnosis is very important because of different prognosis.
Bulbar and pseudobulbar palsy are characterized by difficulty swallowing and abnormal
phonation. The child cannot swallow milk, chokes and coughs, milk pours out through his nose.
Voice changes are present: child’s cry is muffled, low modulated and “nasal”.
Bulbar palsy also causes shortness of breath with abnormal rhythm with no (especially at the
early stages) changes in lungs. Arrhythmias are seen, periodic bradycardia is characteristic.
Underlying pathologic process of bulbar syndrome is damage to caudal parts of brain (medulla
oblongata) where nucleuses of vagus and hypoglossus are in vicinity of respiratory and vasomotor
centers. Damage to these structures is life threatening. Pseudobulbar palsy is not.
Pseudobulbar palsy, however, may the cause of aspiration pneumonia. Dx of pseudobulbar
palsy can be confirmed when central palsy of n. hypoglossus is present characterized by limited
motion of the tongue. Reflexes of oral automatism are normal for newborns but pathological for
adults and are included in the pseudobulbar syndrome in adults.
11th innervates neck and shoulder muscles performing head turning and head lifting in older
children. When child is lying on his abdomen this nerve is responsible for protective reflex. When this
nerve is damaged torticollis occurs causing redistribution of the tone and elements of neck-tonic
reflex appear. Not infrequently children demonstrate head tics (hyperkynesis caused by damage of
cortico-nuclear pathways. Sometimes torticollis associated with flaccid paresis of arms as a result of
spinal and brachial plexus trauma. The most frequent type of torticollis the intranatal trauma of
sternocleidomatoideus with resulting intramuscular hemorrhage (the muscle is indurated and tender).
This type of torticollis should be followed by orthopedist. Neurological status should be checked
carefully to rule out possible damage of the accessory
12th innervates tongue. In the normal awake child tongue is in active movement. Flaccid
paresis is rarely seen and is due to underdevelopment of hypoglossal nucleus. Central paresis is
much more frequent manifesting by limited motion of the tongue. It becomes tense and indurated
(folded in a wad), initiation of sucking is poor.

Motor activity
The examination of the motor activity starts with noting the child’s posture. Unrestrained
healthy newborn is in constant chaotic movement with predominant flexion. Arms and legs are flexed
and adducted. Wrists are flexed into fist with the thumb opposed, feet are moderately extended (180
degrees to the shins). This position is called “embryo’s posture”. In contrast to intrauterine posture
child extends his head because of extensors hypertonus. Limb movement should also be noted.
Pre- and intra-natal pathologies can cause changes in posture. Open wrist (seal’ paws) are
frequent in pathology of brain circulation with intracranial hypertension. If the examiner sees an
extended arm with internally rotated wrist he should suspect flaccid palsy. Legs in extension with
tendency to crossing, feet in varus position with plantar hyperflexion (sometimes they even reach the
shins) are characteristic for spastic paresis.
Extended arms (in internal rotation) and legs (with abducted hips and sometimes with
dropped feet) (frog’s posture) are frequently seen in low-neuron paresis. It is obvious that motor
function in all above cases is impaired. Not infrequently motor function is abnormal even in the
absence of posture changes. In this case one should suspect diffuse inhibition of motor function
which often caused by pathology of brain circulation (SAH or hypertension syndrome). Asymmetric
movements prompt for searching latent paresis (monoparesis, hemiparesis, etc.)
It is important to estimate muscle tone. Normally tone is greater in limb flexors and neck extensors
before 1.5-2 month of life that is why it is difficult to differentiate normal tone from pathological one.
Intrauterine pathology sometimes lead to hypertonicity in hip adductors. When passive abduction
meets resistance the examiner should differentiate between dysplasia, congenital dyslocation and
spastic muscles. This differentiation requires orthopedist consultation. Frequently newborns have
muscle hypotone caused by meningeal and cerebral hemorrhages due to diashise ? . Hypotone can
transform into hypertone. Diffuse hypotonia with severely limited ROM (especially in proximal limbs)
suggests congenitalneuro-muscular diseases or various congenital metabolic abnormalities.
Tendon reflexes are very labile in newborns. The can be elicited only in 25-30% of children.
It is advised to fix child’s knee in a kind of lock: the thumb stabilizes the shin, the index fixes the hip
and the middle finger is put in the patellar fossa. Knee is bend 120 degrees (Fig. 1).
By using the light vibration the pediatrician can relax the child’s leg and find the most
productive position. The stroke should be made by index or middle finger but not by a hammer.
Sensation: the newborns react to hot and cold stimulus by changing their motor activity, sometimes
they show undifferentiated verbal reaction.
Meningeal signs in newborns and in several month old infants have some characteristic features.

There is no nuchal rigidity because due to open fontanels and sutures there is no tension of
meninges. It is necessary to remember that nuchal rigidity checking requires stabilization of the
thorax simultaneously with neck flexure to overcome physiologic hypertonicity of neck flexors.
Positive Kernig and lower Brudzinsky are normal until 4-6 month old.
Involvement of meninges can be suspected with tense, pulsating fontanel, general
hyperestesia, opisthotonus, hanging symptom increasing opisthotonus and leg flexion when
vertically lifting the child.
Congenital reflexes
Congenital reflexes are the hallmarks of maturity of CNS, viability and adaptation resources.
Arcs of these reflexes end at different levels of the spine and brain stem and reflect function of
cranial nerves and motor systems. It is useful to know only those reflexes, which are the most
representative since first hours of life. For maximum convenience we list reflexes not by anatomy but
by child’s posture.
Reflexes elicited when child is lying on his back
1. Oral group of reflexes (5, 7, 12th CN)
a) Scout reflex (Kussmaul’s reflex): touching with a finger of a pacifier of an angle of the
mouth makes child to open his mouth and protrude his tongue trying to reach the
stimulus.
b) Sucking reflex: finger, pacifier or nipple put in the mouth are compressed by child’s lips
and sucking begins. Pathology of cranial nerves responsible for sucking cause
insufficient degree of vacuum within the oral cavity.
c) Palm-mouth reflex of Babkin: pressing on both thenars makes child to open his mouth,
slightly flex his head, sometimes protrude his tongue. Babkin’s reflex is sensitive and it
disappears when CNS damage is present, especially after intranatanl circulation
problems. Rapid return of this reflex is a good prognostic sign.
2. Hugging reflex (Moro’s reflex)
Vestibular portion of 8th nerve is responsible for this reflex. It is elicited by sharp blow to the table
20-30 cm from child’ head. Child should abduct his arms, extends his legs, which were
previously flexed and drawn to the abdomen. Moro’s reflex consists of two phases: first phase is
arm abduction, second phase is arm adduction with a tendency towards hugging himself. That is
why French scientist Moro called it a “hugging reflex”.

Normally both phases are present. Sometimes initially in first weeks of life only first phase is
present and the second phase appears later, this is also normal. Absence of Moro’s reflex or
absence of the second phase raise suspicion of pre- or intranatal pathology involving vestibular
nerve. Moro’s reflex is effected by low motor neuron at the level of cervical intumescences (arms
movement) and if traumatic low motor neuron palsies are present Moro’s reflex will not be
elicited in spite of the intact vestibular apparatus. Moro’s reflex is also absent in other low motor
palsies. Spastic paresis cause absence of Moro’s reflex initially later it becomes exaggerated
and persists.
Exaggeration of Moro’s reflex, spontaneous Moro’s reflex (to sound or with no reason) is a
reflexion of tectum of mesencephalonf “start-reflex” and speaks for low seizure threshold.
3. Cervical clonic asymmetrical reflex (Magnus-Klein’s reflex) reflects maturity of 8th and 9th
nerves and their connections to motor centers.
It is elicited by passively turning the child’s head. Extremities to which the head is turned extend,
arm is frequently elevated. The contralateral extremities flex. The child’s posture is called the
“swordsman posture”. Extenors’ tone is increased. Not infrequently children have torticollis (the
etiologies are listed above) in responce asymmetrical cervical clonic reflex develops leading to
redistribution of tone in extremities which is normal. Pre- and intranatal abnormalities can cause
late development of this reflex.
4. Grasping reflex
When examiner presses on child’s palm the child grasps the examiner’s finger. Sometimes it is
possible to lift the child that way. IT is important to estimate the symmetry of this reflex. Flaccid
paralysis causes absent or diminished reflex of the ipsilateral side. Different types of brain
circulation abnormalities causing motor insufficiency can lead to diminished reflex which can be
bilaterally (suspect cerebral palsy with bilateral hemiplegia) or unilateral (often these children
have hemiplegic form of cerebral palsy).
5. Plantar reflex is the analogue of the grasping reflex.
Pressing at the middle of the plantar side of the foot makes child to flex his toes. Symmetricity of
this reflex is also important.
6. Babinski’s reflex.
It is elicited by gentle tickling of the foot. In response child extends the big toe and puts his other
toes in a fan-like fashion. The examiner should note symmetricity and strength of this reflex.
When motor abnormality is present Babinsky can be spontaneous and be present earlier on the
involved side.

REFLEXES IN THE VERTICAL POSITION
Child is supported under armpits from the back, and thumbs of the examiner support the child’s
head.
1. Support reflex is elicited when child’s feet touch the examining table. It consists of 2 stages.
First stage: when child’s feet touch the table he flexes his legs. Second stage: child extends his
leg against the table planting his feet. If child puts only his toes on the table one should suspect
motor dysfunction. Planting of toe and crossing of legs speak for spastic paresis.
2. Automated ambulation can be viewed as a third stage of the support reflex. When child’s body
is slightly flexed (propulsion position) child begins to move his legs, making steps. If legs stuck
against each other, leaning on the toes (ballet dancer’s posture) is present or if automated
ambulation is absent then motor insufficiency is present with various degrees of spastic paresis.

PEMERIKSAAN DERAJAT KESADARAN, TANDA RANGSANG MENINGEAL,
REFLEKS BATANG OTAK
Untuk menilai derajat kesadaran secara kuantitatif, digunakan Glascgow Coma Scale (GCS) yang
terdiri dari komponen membuka mata (Eye=E), respon motorik (M), dan respon verbal (V). Cara
penilaian GCS:
E Buka mata spontan 4
Buka mata bila disuruh 3
Buka mata bila ada stimulus nyeri 2
Tidak ada respon buka mata 1

Kaku Kuduk :
- Pasien dalam kondisi berbaring
- Tangan kanan pemeriksa pada dada pasien di bawah dagu, tangan kiri di bawah kepala
pasien
- Dengan tangan kiri, kepala pasien dirotasikan ke kiri dan kanan kemudian difleksikan
sampai dagu menyentuh dada. Rasakan apakah terdapat tahanan atau tidak.
- Kaku kuduk positif bila terdapat tahanan saat kepala rotasi/difleksikan
- Perhatikan juga apakah saat kepala difleksikan terdapat fleksi pada panggul dan lutut (
Brudzinski I)
- Pemeriksaan ini tidak dilakukan pada pasien trauma cervical atau terdapat rheumatoid
arthritis.
Tanda Kernig
- Pasien dalam kondisi berbaring
- Tungkai kanan atas difleksikan pada sendi panggul membentuk sudut 90°, tungkai bawah
posisi fleksi pada sendi lutut
- Tungkai bawah digerakkan ekstensi pada sendi lutut
- Normal bila ekstensi tungkai bawah terhadap sendi lutut > 135° dari garis vertical, abnormal
bila timbul tahanan atau nyeri sebelum sudut 135°
- Lakukan pada tungkai kiri

Untuk membangkitkan suatu refleks regangan, Anda harus menyokong sendi yang diperiksa
sehingga ototnya rileks. Palu refleks dipegang di antara ibu jari dan jari telunjuk, dan diayunkan
dengan gerakan pada pergelangan tangan, bukan siku. Pada umumnya yang dipakai adalah ujung
runcing palu refleks berbentuk segitiga. Ketukan ringan pada tendo yang diperiksa seharusnya
menimbulkan kontraksi otot. Sering kali Anda juga harus mempalpasi, disamping mengamati otot itu
untuk menilai kontraksinya. Periksalah setiap refleks dan bandingkan dengan sisi lainnnya. Refleks
harus sama secara simetris.
Ada variasi yang luas dalam respons refleks. Hanya dengan pengalaman pemeriksa akan
dapat membuat penilaian yang baik tentang refleks normal.

Penderita dapat duduk dengan tungkai menjuntai (gambar 2 a), atau berbaring (gambar 2b),
atau dapat pula penderita berlutut dimana sebagian tungkai bawah dan kakinya terjulur diluar
meja pemeriksa.
Pada dasarnya pemeriksa sedikit meregangkan tendo Achiles dengan cara menahan ujung
kaki kearah dorsofleksi.
Tendo Achilles dipukul dengan gerakan fleksi kaki yang menyentak.
Bila perlu dapat dikerjakan reinforcement sebagaimana dilakukan pada pemeriksaan refleks
patella.

Pemeriksaan Refleks Abnormal atau Refleks Patologis
Tanda atau refleks Babinski adalah suatu refleks patologis. Biasanya kalau sisi lateral telapak kaki
digores dari tumit ke arah pangkal jari-jari kaki, melengkung kemedial melintasi kaput-kaput tulang
metatarsal, akan terjadi fleksi plantar pada ibu jari kaki. Prosedur ini menguji radiks saraf pada L5 –
S2. Kakinya harus digores dengan rangsang yang cukup kuat seperti sebuah kunci. Jangan
memakai peniti! Kalau ada penyakit traktus piramidalis, dan gerakan yang telah diuraikan diatas
dilakukan, akan terjadi dorsifleksi ibu jari kaki dengan penyebaran jari-jari lainnya. Refleks ini
adalah refleks Babinski. Karena tanda Babinski merupakan suatu refleks abnormal, kita hanya
menyatakan bahwa ada tanda Babinski. Adalah benar bila kita menuliskan refleks plantar sebagai
fleksi plantar (normal) atau dorsifleksi (abnormal, Babinski). Tehnik memeriksa refleks plantar
diperlihatkan dalam Gambar 45.
Dugaan akan adanya penyakit traktus piramidalis juga timbul kalau terjadi dorsifleksi ibu jari kaki
ketika sisi lateral kaki digores. Reaksi yang timbul merupakan tanda Chaddock. Kalau ada penyakit
traktus piramidalis, penekanan ke bawah sepanjang tulang kering juga akan menyebabkan
dorsifleksi ibu jari kaki. Reaksi ini dikenal sebagai tanda Oppenheim. Kedua tanda ini kurang
sensitive dibandingkan penggoresan permukaan plantar.

Refleks abnormal lainya yang berkaitan dengan penyakit traktus piramidalis adalah tanda Hoffman.
Untuk membangkitkan tanda ini, tangan pasien dipronasikan dan pemeriksa memegang falangs
terminal jari tengah pasien di antara ibu jari dan jari telunjuk. Falangs itu secara pasif difleksikan
dengan sentapan tajam dan tiba-tiba dilepaskan. Respons positif terdiri dari adukasi dan fleksi ibu
jari, dan juga fleksi jari-jari lain.

PEMERIKSAAN MOTORIK
Yang dinilai dalam pemeriksaan sistem motorik adalah :
Massa otot
Kekuatan otot
Tonus otot
Refleks
Cara pemeriksaan :
1. Inspeksi massa otot
Pemeriksaan motorik dimulai dengan inspeksi tiap daerah yang diperiksa. Bandingkan
kontur masa otot simetris. Inspeksi dipakai untuk menentukan atrofi otot dan adanya
fasikulasi.
2. Tonus
Tonus dapat didefenisikan sebagai sedikit ketegangan residual pada otot yang rileks secara
volunter. Tonus dinilai dengan resistensi terhadap gerak pasif. Mintalah pasien untuk rileks.
Lakukan gerak pasif pada otot itu. Bandingkanlah satu sisi dengan sisi lainya. Harus dingat
bahwa lesi neuron motorik atas menimbulkan spastisitas, hiperrefleksi, klonus, dan tanda
Babinski. Lesi neuron motorik bawah menimbulkan atrofi, fasikulasi, penurunan tonus, dan
hiporefleksi. Kedua macam lesi ini dapat menimbukan kelemahan. Fasikulasi dapat menjadi
lebih jelas kalau otot itu diketuk perlahan-lahan dengan palu refleks.
Menggerakkan ekstremitas pasien secara pasif. Perhatikan keadaan kontraksi atau regangan yang
terasa pada otot. Peningkatan tonus disebut Hipertonia, penurunan tonus disebut Hipotonia
3. Kekuatan
Periksa kekuatan otot dengan menyuruh pasien bergerak secara aktif melawan tahanan
Anda. Jika menemukan kelemahan otot, perbandingan kekuatan proksimal dan distal
penting. Pada umumnya kelemahan proksimal berkaitan dengan penyakit otot; kelemahan
distal berkaitan dengan penyakit neurologik.
Tingkat kekuatan menurut Medical Research Council (MRC):
0 : tidak ada kontraksi
1 : ada kontraksi otot tetapi tidak ada gerakan
2 : gerakan horisontal, dapat menggeser anggota gerak tetapi tidak dapat
mengangkat

3 : dapat menahan gaya berat/dapat mengangkat anggota gerak
4 : dapat melawan sejumlah tahanan
5 : dapat melawan tahanan maksimal
Salah satu cara mudah untuk menilai ada tidaknya kelemahan pada ekstremitas atas:
Pasien diminta merentangkan kedua lengannya ke depan dengan posisi kedua tangan supinasi
(telapak tangan menghadap ke atas), kedua mata ditutup, posisi dipertahankan sampai sekitar 1
menit, perhatikan posisi kedua lengan. Bila terdapat kelemahan pada satu sisi maka sisi yang lemah
perlahan-lahan akan turun dan cenderung ke posisi pronasi.

PEMERIKSAAN SARAF KRANIALIS
Pemeriksaan saraf kranialis harus dilakukan secara berurutan dan tidak perlu dilakukan
semua secara lengkap. Beberapa pemeriksaan hanya dilakukan sesuai indikasi. Tabel berikut
memuat daftar saraf/nervus kranialis, fungsinya, dan penemuan klinis kalau ada lesi.
N. I (Nervus Olfaktorius)

Nervus olfaktorius mensuplai ujung saraf ke konka nasal superior dan sepertiga atas septum
nasal. Nervus olfaktorius tidak rutin diperiksa. Tetapi harus diperiksa pada setiap pasien yang
dicurigai menderita gangguan pada lobus frontalis.
Cara Pemeriksaaan :
Periksa kemampuan pasien dalam persepsi, identifikasi dan menamakan zat yang akan dites
Gunakan zat pengetes yang dikenal sehari-hari (misalnya kopi, tembakau, jeruk), jangan
menggunakan zat yang merangsang mukosa hidung seperti mentol, amoniak, alcohol atau cuka.
Pasien diminta menutup mata, salah satu lubang hidung ditutup. Suruh pasien mencium . Tanyakan
apakah dia dapat mencium bau tersebut dan mengidentifikasinya.
Periksa lubang hidung yang lain dengan cara yang sama.
Tes ini merupakan tes yang subyektif berdasarkan penilaian kualitatif tergantung dari laporan yang
diberikan oleh pasien.
N. II ( Nervus Optikus)

Fungsi: Sensorik khusus melihat
Cara Pemeriksaan :
Mata diperiksa secara bergantian, pastikan bahwa bila satu mata diperiksa, mata yang lain ditutup.
Pemeriksaan akan lebih akurat bila pasien dalam keadaan sadar penuh dan kooperatif. Jika pasien tidak
mempunyai keluhan yang berhubungan dengan N.II dan pemeriksa juga tidak mencurigai adanya gangguan,
maka biasanya hanya dilakukan pemeriksaan N.II secara kasar (ketajaman penglihatan dan lapang
pandang).
Pemeriksaan N.II terdiri dari :
1. Pemeriksaan ketajaman penglihatan (visual acuity)
2. Pemeriksaan warna
3. Pemeriksaan Lapang pandang
4. Funduskopi
5. Pemeriksaan Reflex cahaya

memberitahukan bila dia melihat gerakan obyek pertama kali. Tes melalui berbagai meridian
penglihatan (temporal atas-bawah, medial atas-bawah), Ulangi untuk mata sebelah.
4. Pemeriksaan fundus (menggunakan alat ophtalmoskop)
- Pemeriksaan dilakukan dalam keadaan gelap
- Suruh pasien melihat jauh ke depan dalam jarak tertentu dan fiksasi matanya dengan
melihat benda di depannya. Pasien tidak boleh menggerakkan bola matanya, tetapi boleh
mengedipkan matanya.
- Pegang ophtalmoskop dengan tangan kanan. Lihat mata kanan pasien dengan
optalmoskop pada jarak 30 cm dan 15° horizontal dari garis fiksasi.
- Perlahan dekatkan optalmoskop 1-2 cm dari mata pasien
- Perhatikan optic disc, pembuluh darah, retina.
Gambaran fundus normal (Gambar 2): cupping (cekungan) berbatas tegas, berwarna jingga,
diameter pembuluh darah arteri dibanding vena = 2:3

- Minta pasien menatap objek tertentu di kejauhan
- Sinari satu pupil dengan cahaya, perhatikan ukuran pupil sebelum dan sesudah stimulus
cahaya. Pupil normal akan mengecil diameternya begitu diberi stimulus cahaya. Ini disebut
Refleks Cahaya Langsung. Perhatikan ukuran pupil mata sebelah sebelum dan sesudah
stimulus cahaya. Bila ukuran pupil mengecil bersamaan dengan mata yang diberi stimulus
cahaya, disebut Refleks Cahaya Tidak Langsung/Konsensual.
- Ulangi prosedur pada mata sebelah
N. V ( Nervus Trigeminus)

Fungsi :
- motorik : otot mengunyah, membuka dan menutup mulut dan menggerakkan rahang.
- sensorik : sensibilitas wajah, mukosa hidung, mulut dan gigi
Ada tiga cabang utama nervus trigeminus: oftalmik, maksilar, dan mandibular. Cabang-cabang ini
diperlihatkan dalam gambar di bawah ini:

Cabang oftalmik mensuplai sensasi sinus frontalis, konjungtiva, kornea, kelopak mata atas,
pangkal hidung, dahi dan kulit kepala sampai sejauh puncak kepala. Cabang maksilar mensuplai
sensasi pipi, sinus maksilaris, aspek lateral hidung, gigi atas, nasofaring, palatum durum, dan uvula.
Cabang mandibularis mensuplai sensasi dagu, rahang bawah, dua pertiga anterior lidah, gigi bawah,
gusi dan lantai mulut, dan mukosa bukal pipi. Cabang motoriknya mensuplai otot-otot untuk
mengunyah dan tensor timpani.
Cara Pemeriksaan :
1. Motorik :
- Inpeksi kening dan pipi, apakah ada atrofi dari otot-otot masseter dan
temporalis
- Palpasi otot-otot tersebut sambil pasien disuruh merapatkan giginya sekuat mungkin.
Perhatikan tonus dan kontraksinya.
- Suruh pasien untuk menggigit ‘tongue depressor’ . Bila tedapat paralysis otot, maka
pemeriksa dapat menarik tongue depressor tersebut dengan mudah pada sisi yang lemah

- Suruh pasien membuka mulutnya. Paralisis otot pterygoideus lateralis ditandai dengan
deviasi rahang ke arah sisi lesi. Adanya deviasi ditandai dengan cara pemeriksa
meletakkan pena atau penggaris di depan hidung pasien secara vertical, kemudian diambil
patokan gigi incissivus atas dan bawah, bila ada deviasi maka ada pergeseran dari posisi
gigi semula sewaktu membuka mulut. Jangan berpatokan pada posisi bibir.
- Suruh pasien untuk menggerakkan rahangnya dari satu sisi ke sisi lain melawan tahanan.
Paralisis otot di satu sisi, akan menyebabkan pasien tidak mampu menggerakkan
rahangnya ke sisi yang berlawanan arah dengan otot yang lumpuh.
2. Sensorik
Pemeriksaan sensorik dilakukan dalam berbagai modalitas sesuai dengan distribusi daerah seperti
yang ditunjukkan pada gambar
Refleks kornea
- Pasien diminta melirik ke arah yang berlawanan dengan arah yang akan dirangsang.
- Pemeriksa merangsang dengan mengadakan sentuhan ringan langsung ke kornea
menggunakan ujung kapas.
- Perhatikan refleks kornea langsung dan refleks konsensual pada mata kontralateral. Positif
jika terdapat kedipan palpebra.

N. VII ( Nervus Facialis)
Nervus fasialis mempersarafi otot-otot wajah dan pengecapan pada dua pertiga anterior
lidah.Sebagian sangat kecil mensuplai sensasi umum pada telinga luar.Nervus facialis juga
mengandung serabut motorik parasimpatis ke kelenjar ludah dan korda timpani.
Fungsi :
- menggerakan otot-otot wajah
- sekresi kelenjar submaksilaris, sublingual dan lakrimalis.
- pengecapan 2/3 anterior lidah
Cara Pemeriksaan:
1. Pemeriksaan otot-otot wajah
- Perhatikan muka penderita simetris atau tidak
- Perhatikan kerutan pada dahi, plika nasolabialis, sudut mulut, fisura (celah) palpebra
- Perhatikan gerakan spontan sewaktu tersenyum, mengedipkan mata.
- Suruh pasien mengangkat alis dan mengerutkan dahi, lihat kerutannya simetris atau tidak.
Pada sisi yang lumpuh kerutan berkurang atau menghilang.
- Suruh pasien memejamkan mata, kemudian pemeriksa mengangkat kelopak mata pasien
sementara pasien diminta untuk menahan kelopak matanya jangan sampai terbuka. Jika
terjadi kelumpuhan maka pasien tidak dapat mempertahankan pejamannya pada sisi yang
lumpuh.
- Suruh pasien menyeringai, mencucurkan bibir, menggembungkan pipi. Pada sisi yang
lumpuh sudut mulut tertinggal. Pada pasien yang tidak sadar, dapat diberikan rangsang
nyeri dengan menekan sudut rahangnya, lihat apakah simetris atau tidak.
3. Fungsi pengecapan
- Gunakan 4 macam rasa dalam larutan : gula, garam, acetic acid. Quinine

- Pasien disuruh untuk menjulurkan lidahnya keluar dan taruh pada lidahnya ke-4 macam
rasa tadi secara bergantian. Pastikan bahwa substansi itu ditaruh hanya 2/3 anterior lidah.
Gunakan cotton applicator untuk menaruh substansi tadi
- Apabila cairan ditaruh, penderita tidak boleh menarik lidahnya ke dalam mulut. Pasie
disuruh menyatakan pengecapan yang dirasakannya dengan isyarat atau dengan menulis
di atas kertas.
- Mulut harus dibersihkan dengan air antara tes rasa yang satu dengan lainnya. Rasa pahit
sebaiknya dites terakhir karena meninggalkan rasa yang lebih lama.
4. Reflex
- Chovstek sign : lakukan ketokan ringan di bagian depan telinga. Bila positif, terjadi
kontraksi otot yang dipersarafinya. (dapat normal atau tetani)
- Refleks orbikularis oris → Lakukan ketokan di atas bibir atas, terjadi kontraksi bibir atas
ipsilateral. Bila ketokan dilakukan pada mentalis, terjadi elevasi dan protrusi bibir bawah
dan kerutan kulit pada dagu = perioral, oral, bucal, nasomental reflex.

Kelemahan fasial
Ada dua macam kelemahan fasial.Lesi neuron motorik atas seperti stroke yang
melibatkan lintasan kortikobulbar akan menimbulkan kelemahan kontralateral pada bagian
bawah wajah dengan bagian atas wajah yang tetap berfungsi dengan normal.Pasien masih
dapat mengerutkan dahinya.Ini berkaitan dengan persarafan bilateral wajah atas oleh
serabut-serabut kortikobulbar.Wajah bawah hanya mempunyai persarafan unilateral dari
pusat kortikal kontralateral.Jenis lesi neuron motorik atas ini dilukiskan oleh lesi “A” dalam
Gambar 13. Jenis kedua kelemahan wajah akan menimbulkan gangguan total pada otot-otot
wajah ipsilateral tanpa ada daerah yang tidak terganggu.Gangguan ini dapat disebabkan
oleh lesi pada saraf itu ketika ia meninggalkan kepala atau oleh gangguan pada nucleus
fasialis di pons, seperti dilukiskan oleh lesi “B” pada Gambar 13.

N. VIII ( Nervus Vestibulo-koklearis)
Cara Pemeriksaan :
1. Pendengaran
- Lakukan tes secara kasar dahulu dengan menyuruh pasien mendengarkan bisikan pada
jarak tertentu atau gosokan ibu jari dan telunjuk. Bila terdapat perbedaan gunakan tes
Rinne, Weber, Schwabach, audiogram
- Gunakan garpu tala berfrekuensi 128, 256, 512 Hz
- Tes Rinne → untuk membandingkan konduksi tulang dan udara
Getarkan garpu tala, ujung garpu tala diletakkan di atas tulang mastoid, minta pasien
mengatakan saat getaran hilang, kemudian garpu tala didekatkan ke liang telinga, tanyakan
apakah suara getaran masih ada.
Tes Rinne bila konduksi udara lebih baik daripada tulang
bila konduksi tulang lebih baik daripada udara
- Tes Weber → lateralisasi suara ke arah telinga yang lebih keras
Getarkan garpu tala, ujung garpu tala di taruh di atas tulang vertex (pada garis tengah), lalu
tanyakan kepada pasien apakah terdengar bunyi yang simetris atau terdapat bunyi yang
lebih keras pada satu sisi telinga.

2. Keseimbangan, koordinasi
- Fukuda’s stepping test → pasien diminta berjaln di tempat dengan mata tertutup selama
± 1 menit (50x) . Normalnya rotasi badan < 30° dan pergerakan maju atau mundur < 1
meter
- Romberg test → Dengan mata terbuka pasien berdiri dengan merapatkan kedua kakinya,
perhatikan jatuh ke sisi mana. Setelah itu dengan mata tertutup, tangan dilipat pada dada
atau direntangkan ke depan. Jika terdapat kelainan akan jatuh.
- Berjalan tandem → pasien berjalan dengan tumit kaki yang satu di depan jari-jari kaki yang
lainnya. Perhatikan apakah ada penyimpangan.

N. IX ( Nervus Glosofaringeus)
N. X ( Nervus Vagus)
Nervus glosofaringeus mensuplai sensasi untuk faring, sepertiga posterior lidah, membrane
timpani, dan serabut-serabut sekresi ke kelenjar parotis. Sedangkan nervus vagus mensuplai
serabut-serabut parasimpatis ke visera dada dan abdomen, serabut-serabut motorik ke faring dan
laring, dan sensasi ke kanal telinga luar.
Cara Pemeriksaan :
1. Fungsi motorik:
- Pasien disuruh menyebutka “aaaa…..”Perhatikan kualitas suara pasien apakah disfonia
(suara sengau) atau afonia (tidak dapat bersuara)
- Penderita disuruh memakan makanan, apakah ada disfagia (gangguan menelan) terutama
bila menelan cairan
- Pasien diminta membuka mulutnya, perhatikan palatum molle, faring, arkus faring dan
uvula, apakah kedudukan simetris. Kemudian pasien disuruh untuk menyebut “aaaa….”
Bila terjasi kelumpuhan maka palatum molle, uvula dan arkus faring sisi yang lumpuh
letaknya lebih rendah dari yang sehat dan seolah-olah uvula tertarik ke bagian yang sehat.
- Fungsi viseromotorik: mengatur kelenjar untuk saliva
2. Fungsi sensorik
- Pemeriksaan somatosensorik sulit untuk dites secara spesifik, karena banyak persarafan
yang overlapping
- Fungsi viserosensorik → 1/3 posterior lidah. Tidak diperiksa secara rutin karena sukar
3. Refleks
Refleks faring (gag reflex) : berikan rangsangan ringan dinding faring atau tonsil atau lidah belakang
dengan tongue spatel. Refleks positif bila terjadi elevasi dan konstriksi otot faring, disertai dengan
retraksi lidah. Jika rangsang cukup kuat dapat membangkitkan refleks muntah. Bagian sensorik
lingkaran refleks ini adalah melalui nervus glosofaringeus; bagian motorik melalui nervus
vagus.

N. XI ( Nervus Aksesorius)
Otot Sternokleidomastoideus
- Lakukan inspeksi : perhatikan apakah ada atrofi
- Periksa kekuatan otot antara kedua sisi. Kita suruh pasien menoleh ke kanan, kemudian
kita tahan dengan tangan kita yang ditempatkan di dagu. Kita palpasi otot yang
berkontraksi. Lakukan juga pada sisi yang lain. Bandingkan kekuatan dan kontraksi otot
keduanya.
Otot Trapezius
- Pada isnpeksi : perhatikan apakah ada atrofi dan bagaimana kontur otot. Periksa apakah
posisi bahu simetris atau ada yang lebih rendah
- Kemudian lakukan palpasi untuk mengetahui konsistensinya
- Tempatkan tangan pemeriksa pada bahu pasien. Kemudian pasien disuruh mengangkat
kedua bahunya. Bandingkan kekuatan kiri dan kanan.

Cara Pemeriksaan:
- Suruh penderita membuka mulut dan perhatikan lidah dalam keadaan istirahat dalam mulut.
Perhatikan apakah terdapat atrofi lidah, fasikulasi dan tremor. Bila ada kelumpuhan, maka
lidah akan berdeviasi ke arah yang sehat.
- Suruh penderita untuk menjulurkan lidahnya. Jika terdapat kelumpuhan, lidah akan
mencong ke sisi yang lumpuh
- Kita juga menilai kekuatan lidah. Suruh penderita untuk menekankan lidahnya pada pipinya.
Kita nilai kekuatannya dengan menekankan jari kita pada pipi sebelah luar. Bila terdapat
kelumpuhan lidah bagian kiri, maka lidah tidak dapat ditekankan ke pipi sebelah kanan.
- Jika terdapat kelumpuhan bilateral (kedua sisi), lidah tidak dapat digerakkan atau dijulurkan.

PEMERIKSAAN SENSORIK, KESEIMBANGAN, KOORDINASI
PEMERIKSAAN SENSORIK
Pemeriksaan sensorik terdiri dari :
Sentuhan ringan
Sensasi nyeri
Sensasi getaran
Propriosepsi (sensasi posisi)
Lokalisasi taktil
Sensasi diskriminatif (termasuk diskriminasi dua titik, stereognosis, grafestesi, dan lokalisasi
titik). Pada pasien tanpa tanda atau gejala penyakit neurologist, pemeriksaan fungsi sensorik dapat
dilakukan dengan cepat, dengan memeriksa adanya sensasi normal pada ujung jari tangan dan kaki.
Pemeriksa dapat memilih apakah ia mau memeriksa sentuhan ringan, nyeri dan sensasi getaran.
Jika ini semuanya normal, pemeriksa sensorik lainnya tidak diperlukan. Jika ada tanda atau gejala
yang menunjukkan gangguan neurologik, harus dilakukan pemeriksaan yang lengkap.
Seperti pada pemeriksaan motorik, pemeriksa membandingkan sisi dengan sisi, dan proksimal
dengan distal. Gangguan neurologik biasanya menyebabkan gangguan sensorik yang mula-mula
terlihat di bagian lebih distal dibandingkan di bagian proksimal.

Pemeriksaan sensorik merupakan pemeriksaan yang bersifat subyektif, tergantung dari
respon pasien sehingga harus dilakukan pada pasien yang kooperatif atau dapat diajak bekerjasama
dan mengerti perintah kita. Saat pemeriksaan sebaiknya pasien diminta menutup matanya. Tes
dilakukan dengan membandingkan sisi kanan dan kiri.
Pemeriksaan sensorik terdiri dari :
1. Sentuhan lembut (light-touch)
Dinilai dengan seberkas kapas , dinilai kanan dan kiri, hindari bagian tubuh dengan rambut
banyak
2. Nyeri (pin prick), menggunakan jarum tumpul yang bersih
3. Suhu, menggunakan tabung reaksi berisi air hangat dan dingin
4. Sensasi posisi tubuh (joint position), dinilai dengan menahan sendi proksimal jari tangan
atau kaki, kemudian sendi distal digerakkan dan tanyakan pasien kea rah mana ujung jari
tersebut digerakkan.
5. Sensasi getaran (vibration), menggunakan garpu tala yang digetarkan kemudian ujung
garpu tala diletakkan

Tes Romberg
Pasien diminta untuk bediri dengan kedua kaki , pertama dengan mata terbuka kemudian
dengan mata tertutup.
Perhatikan apakah tubuh pasien cenderung jatuh ke satu sisi
Dikatakan positif (+) bila pasien tubuh pasien cenderung jatuh ke satu sisi
Pada ataksia sensorik (akibat gangguan proioseptif) maka pasien dapat mempertahankan
keseimbangan saat mata terbuka dan terganggu saat menutupkan mata. Sedangkan pada
gangguan serebelum, pasien mengalami gangguan keseimbangan pada saat mata tertutup
maupun terbuka.

Kaki diluncurkan dari lutut sampai ke ibu jari, kemudian kaki diangkat untuk mengulangi
gerakan tersebut.
Perhatikan apakah gerakan mulus atau tidak teratur ?
Jika pasien tidak dapat melakukan dengan baik maka terdapat gangguan pada system
koordinasi (serebelum)

Kedua tangan melakukan gerakan supinasi dan pronasi secara bergantian tapi dalam waktu

Banyak kelainan neurologist menimbulkan gaya berjalan yang menyolok dan khas. Pasien dengan
hemiplegia cenderung menyeret atau memutar tungkainya yang lemah dan dan spastik. Lengannya
seringkali difleksikan disiku, menyilang perut ketika pasien ini berjalan. Pasien dengan Penyakit
Parkinson berjalan dengan langkah-langkah pendek, diseret, dan tergesa-gesa. Kepalanya
menunduk, dengan punggung membungkuk. Pasien dengan ataksia serebelum berjalan dengan
langkah kaki berdasar lebar. Kedua kakinya sangat jauh terpisah ketika ia berjalan sempoyongan
dari sisi ke sisi. Pasien dengan foot drop mempunyai gaya berjalan seperti menampar yang khas,
yang disebabkan oleh kelemahan dorsifleksor pergelangan kaki. Pasien dengan ataksia sensoris
berjalan dengan langkah-langkah yang tinggi. Ia menampar dengan kakinya ke bawah seakan-akan
ia tidak merasa yakin akan lokasinya. Gambar 56 melukiskan gaya berjalan ini.

SCHIZOPHRENIA
A. Characteristic symptoms: two (or more) of the following, each present for a significant
portion of time during a 1 month period (or less if successfully treated):
1. delusions
2. hallucinations
3. disorganized speech (e.g., frequent derailment or incoherence)
4. grossly disorganized or catatonic behavior
5. negative symptoms, i.e., affective flattening, alogia, or avolition.
Note: Only one criterion A symptom is required if delusions are bizarre or hallucinations
consist of a voice keeping up a running commentary on the person’s behavior or thoughts,
or two or more voices conversing with each other.
B. Social/occupational dysfunction: for a significant portion of the time since the
onset of the disturbance, one or more major areas of functioning such as work,
interpersonal relations, or self care are markedly below the level achieved prior to
onset (or when the onset is in childhood or adolescence, failure to achieve
expected
level of international, academic, or occupational achievement.)
C. Duration: Continuous signs of the disturbance persist for at least 6 months. This 6-
month period must include at least I month of symptoms (or less if successfully
treated ) that meet criterion A (i.e., active-phase symptoms) and may include
periods
of prodromal or residual periods, the signs of the disturbance may be manifested by
only negative symptoms or two or more symptoms listed in criterion A present in
an attenuated form (e.g., odd beliefs, unusual perceptual experiences).
D. Schizoaffective and mood disorder with psychotic features have been ruled out
because either (i) no major depressive, manic, or mixed episodes have occurred
concurrently with the active-phase symptoms; or (2) if mood episodes have
occurred during active-phase symptoms, their total duration has been brief relative
to the duration of the active and residual periods.

E. Substance/general medical condition exclusion: The disturbance is not due to the
direct physiological effects of substance (e.g., a drug of abuse, a medication) or a
general medical condition.
F. Relationship to a pervasive developmental disorder. If there is a history of autistic
disorder or another pervasive developmental disorder, the additional diagnosis of
schizophrenia is made only if prominent delusions or hallucinations are also
present for at least a month (or less if successfully treated).
Classification of longitudinal course (can be applied only after at least 1 year has
elapsed since the initial onset of active-phase symptoms):
-Episodic with interepisode residual symptoms (episodes are defined by the
reemergence of prominent psychotic symptoms): also specify if: with prominent
negative symptoms.
Subtypes of Schizophrenia
Paranoid type
Type A of schizophrenia in which the following criteria are met
A. Preoccupation with one or more delusions or frequent auditory hallucinations
B. None of the following is prominent: disorganized speech, disorganized or catatonic
behavior, or flat or inappropriate affect
Disorganized type
Type A of schizophrenia in which the following criteria are met:

A. All of the following are prominent:
(1) disorganized speech
(2) disorganized behavior
(3) flat or inappropriate affect
B. The criteria are not met for catatonic type.
Catatonic type
Type A of schizophrenia in which the clinical picture is dominated by at least two of the
following:
(1) motoric immobility as evidence by catalepsy (including waxy flexibility) or stupor (2) excessive
motor activity (that is apparently purposeless and not influenced by
external stimuli)
(3) extreme negativism (an apparently motiveless resistance to all instructions or
maintenance of a rigid posture against attempts to be moved) or mutism
(4) peculiarities of voluntary movement as evidenced by posturing (voluntary assumption
of inappropriate or bizarre postures), stereotyped movements, prominent mannerisms,
or prominent grimacing
(5) echolalia or echopraxia
Undifferentiated type
Type A of schizophrenia in which symptoms that meet criterion A are present, but the
criteria are not met for the paranoid, disorganized, or catatonic type.
Residual type
Type A of schizophrenia in which the following criteria are met:
A. Absence of prominent delusions, hallucinations, disorganized speech, and grossly
disorganized or catatonic behavior
B. There is continuing evidence of the disturbance, as indicated by the presence of
negative symptoms are two or more symptoms listed in criterion A for schizophrenia,
present in an attenuated form (e.g., odd beliefs, unusual perceptual experiences).

DEPRESSION
A. Five (or more) of the following symptoms have been present during the same 2- week period
and represent a change from previous functioning; at least one of the symptoms is either (1)
(depressed mood or (2) loss of interest or pleasure.
Note: Don’t include symptoms that are clearly due to a general medical condition,
or mood- incongruent delusions or hallucinations.
(1) depressed mood most of the day, nearly every day, as indicated by either subjective
report (.e.g., appears tearful). Note: In children and adolescents, can be irritable
mood
(2) Markedly diminished interest or pleasure in all, or almost all, activities most of the
day, nearly every day (as indicated by either subjective account or observation
made by others)
(3) Significant weight loss when not dieting or weight gain (e.g., a change of more than
5% of body weight in month), or decrease or increase in appetite nearly every day.
Note: In children, consider failure to make expected weight gains.
(4) Insomnia or hypersomia nearly every day
(5) Psychomotor agitation or retarditation nearly every day (observable by others, not
merely subjective feelings of restlessness or being slowed down)
(6) Fatigue or loss of energy nearly every day
(7) Feelings of worthlessness or excessive or inappropriate guilt (which may be
delusional) nearly every day (not merely self-reproach or guilt about being sick)
(8) Diminished ability to think or concentrate, or indecisiveness, nearly every day (either
by subjective account or as observed by others)
(9) Recurrent thoughts of death (not just fear of dying), recurrent suicidal ideation
without a specific plan, or a suicide attempt or a specific plan for committing suicide.
