AFFIDAVIT OF GUARDIANSHIP

I, ROWENA F. GATCHALIAN, of legal age, widow, Filipino citizen and

a resident of Osmena, Solano, Nueva Vizcaya, after having been duly sworn to in
accordance with law, hereby depose and state:

1. That I am the surviving spouse of the late PO1 Erwin A. Gatchalian of the
Talavera Police Station, Talavera, Nueva Ecija who died in a vehicular
accident last April 2, 2011 along the national highway in Carranglan,
Nueva Ecija;

2. That my late husband left with me the following minor children, namely:
Rowin John F. Gatchalian, and Kobe James F. Gatchalian, 9 and 2 years of
age, respectively;

3. That being their biological mother, the aforesaid minors are in my

custody as their guardian;

4. That I attest to the veracity of the foregoing statement.
