How to fill in a Complaint Form
Before filing a complaint, a complainant should have to hand: 1. Name and address of alleged offender, if available. If this information is lacking, the ECC/PECC may not be able to notify the respondent or start the investigation. 2. Type of electoral offence committed by the alleged offender, if known. If the complainant cannot identify the offence, assistance should be requested. 3. Witnesses and reasonable evidence. The complaint must contain evidence in addition to details of witnesses who can corroborate the assertion by the complainant, if any witnesses are known. 4. Name and address of complainant. If this information is omitted, the ECC/PECC may not start the investigation. 5. Trust that the ECC/PECC will treat the complainant’s identifying details as confidential. Voter Registration Card number Give the number of your Voter Registration Card. 12. Are you filing this complaint on behalf of an organisation? If you are filing the complaint as a representative of an organisation, please tick the option YES and enter the name of the organisation. Otherwise answer NO. 13. Authorisation to use the information Please sign to allow the ECC/PECC to share the information you have given with other authorised bodies. Section 5: PROOF OF COMPLAINT RECEIPT FORM This is a separate page of the complaints form. It should be filled in by the official who receives the completed complaint form and should be given to you, the complainant. It must include: a) Docket number: This is the ID number of your complaint and will allow you to follow the outcome of your complaint on the ECC website or directly at the ECC/PECC office. b) Signature or mark of person receiving the complaint: Here must come the signature or mark of the ECC/PECC/IEC official who receives the complaint. c) Signature or mark of complainant and date: Finally, the signature or mark of the complainant and the date the complaint is filed are required here.
