AFFIDAVIT OF UNDERTAKING

I, BRYAN T. DELGADO, of the legal age, Filipino, and a resident of Capatan

Tuguegarao City, after having been sworn in accordance with the law, hereby
depose and state:

1. That I am a surrenderee during the anti-illegal drugs campaign of the City

Government and had gone into rehabilitation program and seminars
conducted by the City Social Welfare and Development Office (CSWDO
for brevity);

2. That on March 08, 2018, a drug test was conducted by the CSWDO along
with their Training on Community Based Rehabilitation Program at Casa
Angela, Tuguegarao City and I was found positive in using illegal drugs
despite of the rehabilitation programs and seminars I attended;

3. That in view of the foregoing, I hereby undertake that I will be always

present in Drug testing at City Health office (CHO) and to never use any
prohibited and/or illegal drugs;

4. That should I be found positive again, I will voluntarily surrender myself to

undergo Institutionalized treatment and rehabilitation;

5. That I execute this affidavit to attest the truth of foregoing facts and for
whatever purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 9th day of March
