Dear XXX Company:

Our Client, AAA, has referred to our law firm for

proper legal action his claim/demand for your
company to render full accounting and payment of his
SSS premium contributions, which from the
information given by AAA have been regularly
deducted from his salary but have remained
unpaid/unremitted to date by your company to the
Social Security System. Since his date of hire,
sometime on February 18, 2002 and to the present, the
SSS has no reported premium contributions under
AAA’s name and account that has evidently resulted to
the great damage and prejudice of our client.

In view whereof, formal demand is hereby made

upon you to render FULL ACCOUNTING and
thereafter FULL PAYMENT to our client of his SSS
premium contributions within seven (7) days from
your receipt/notice hereof, otherwise, our client,
without further notice to you, will be compelled to file
the proper criminal and civil cases, with damages,
against you.

Please be guided accordingly.
