We, LLENA A. CALAGO and BEINVENIDA S. QUIÑANOLA, both of legal age,

married and residents of Bayawan City, Negros Oriental, after having been duly sworn
to in accordance with law, hereby depose and say that:

1. We have personally know the person of JOSEPHINE S. SARBODA since her

childhood because formerly we lived in the same locality for a long period of time,
and that her family is our close family friend;

2. We also personally know that JOSEPHINE S. SARBODA has been using that
name since she was a child;

3. When the above-stated person was living with her current husband, she was
using the name of JOSEPHINE S. DELA CRUZ;

4. We know for a fact that the person by the name of JOSEPHINE S. SARBODA
and the name JOSEPHINE S. DELA CRUZ, as appearing in her TEMPORARY
VOTER’S IDENTIFICATION CARD and other records, refer to one and the same
person;

5. We are not related in any way, either by consanguinity or affinity to the above-
named person;

6. As such, we execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve;

childhood because formerly we lived in the same locality for a long period of time,
and that her family is our close family friend;

2. We also personally know that JAINAH ASPAN TANEQUE has been using that
name since she was a child;

3. We know for a fact that the person by the name of JAENA TANIQUE appearing
in the Certificate of Baptism of her child Ricky Dohinog issued by Our Lady of
Lourdes Parish of Brgy. Granada, Bacolod City and the name JAINAH ASPAN
TANEQUE, as appearing in her Certificate of Live Birth and other records, refer
to one and the same person;

4. We are not related in any way, either by consanguinity or affinity to the above-
named person;

5. As such, we execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve;

6. Likewise, we execute this Affidavit in support to the request of Jainah A. Taneque

to change her name from JAENA TANIQUE to JAINAH ASPAN TANEQUE in
the Certificate of Baptism of her son.

1. We personally know the person of VERNIE DIONGCO GUANZON a.k.a JOSE

VERNIE BOY GUANZON since his childhood because his family is our close
family friend;

2. VERNIE DIONGCO GUANZON has been using the name JOSE VERNIE BOY
GUANZON since he was a child;

3. The name JOSE VERNIE BOY GUANZON was inadvertently recorded in his
Certificate of Baptism;

4. We know for a fact that the person by the name of JOSE VERNIE BOY
GUANZON appearing in the Certificate of Baptism issued by the Parish of St.
John Nepomuceno at Sum-ag, Bacolod City, and the name VERNIE DIONGCO
GUANZON, as appearing in his Certificate of Live Birth and other records, refer
to one and the same person;

5. We are not related in any way, either by consanguinity or affinity to the above-
named person;

6. As such, we execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve;
