RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, ISRAEL B. TORRES, Filipino, of legal age, and a resident of

_____________________________ and formerly employed with Ascentia Construction
Incorporated, (the “Company”) do hereby freely and voluntarily make the following
declarations and promises that:

1. I, acknowledge receipt of the sum of Php 4,218.35 (Four Thousand Two Hundred
Eighteen 35/100), Philippine Currency, from Ascentia Construction Incorporated in full
payment and final settlement consisting of my service incentive leave pay and unpaid
wages due to me or which may be due to me from ____________________ under the law
with respect thereto, as well as any and all claims of whatever kind and nature which I
have or may have against Ascentia Construction Incorporated arising from my
employment with Ascentia Construction Incorporated.

2. In consideration of said payment, I do hereby quitclaim, release, discharge and waive

any and all actions of whatever nature, expected, real or apparent, which I may have
against Ascentia Construction Incorporated, its directors, officers, employees, agents and
clients by reason of or arising from my employment with the company. I will institute no
action, whether civil, criminal, labor or administrative in any court, tribunal, quasi-
judicial bodies, government offices, agencies or commissions including without but not
limited to the NLRC, SSS or other similar government owned or controlled corporations
against Ascentia Construction Incorporated its directors, officers, employees, agents and
clients. Any and all actions which I may have commenced either solely in my name or
jointly with others before any office, board, bureau, court, or tribunal against Ascentia
Construction Incorporated, officers, employees, agents and clients are hereby deemed and
considered voluntary withdrawn by me and I will no longer testify or continue to
prosecute said action(s) , and this release, waiver and quitclaim shall serve as my motion
to dismiss any such case and I hereby irrevocably and unconditionally authorize the
Ascentia Construction Incorporated to cause the immediate dismissal thereof based on
this release, waiver and quitclaim.

3. I will not at any time, in any manner whatsoever, directly or indirectly engage in any
activity prejudicial to the interest of the Company its owners or officers.

4. I finally declare that I have read this document and that I have understood its contents
after these have been explained to me in Bisayan dialect which I understand. The entire
release and quitclaim hereby given are made by me willingly, irrevocably,
unconditionally and voluntarily, and with full knowledge of my rights under the law. (Sa
katapusan akong gimatuud nga ako nakasabut niining maong dokumento gi explicar
 ug gipasabot kanako sa bisaya nga sinultiang akong masabtan. Ako nasayod sa akong
mga katungod ubos sa bala-od. Kining pagbiya sa katungod akong gihimo sa akong
kaugalingong kabubut-on ug walay bisan kinsa nga nipugos kanako).
