AFFIDAVIT OF EXPLANATION

I, MARY JANE SINAY, of legal age, Filipino, married and a resident of Claveria,
Cagayan, after being sworn to in accordance with law, do hereby depose and state:

1. That sometime on February 09, 2018, I went to the Philippine Consulate in

Toronto to process the renewal of my Philippine Passport thereat;

2. That after the processing of the renewal of my Philippine Passport, they

issued to me an E-Passport Claim Slip. A copy of which is hereto attached;

3. That one of the requirements which I submitted to them is the printed

confirmed flight schedule of my return to Canada which states that my
departure from Manila, Philippines to Toronto, Canada is on 14 March 2018. A
copy of which is hereto attached;

4. That after a careful perusal of the E-Passport Claim Slip, I was surprised to
discover that the date of release written therein is 17 April 2018;

5. That in view of the foregoing, it is most respectfully implored of this

Honorable Office to expedite the processing and renewal of my Philippine
Passport that I may pick up my passport before my scheduled date of return
to Canada.
