The day of your appointment, you must have all the documents specified above with you.
Please note: if any of these documents are missing, this may lead to the non-issuance of the visa you have applied for.

Pre-requisites
 CERFA form n°14076*01 or 14571*05, dated and signed
 ID photograph
 If you are not a national of your country of residence, please provide proof of your legal residency in this country (e.g. residence permit,
etc.).

 Travel document, valid for at least three months after the planned date of return
Purpose of travel/stay
 Invitation from a company or an authority to participate in conferences or events of a commercial, industrial or professional nature
 Letter of introduction stating purpose of visit on company/organisation letterhead paper, signed by competent authority.
 Pre-booked return ticket
Travel health insurance
 Copy of travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalisation expenses for
the entire period of valid stay on Schengen territory as well as repatriation costs.

Accommodation
 Proof of accomodation (in case of accommodation at a private individual) or hotel reservation or sufficient means to cover the cost of
accommodation (travel provisions of 120 euros / day) or lease agreement.

Funds
 Supporting documents of personal income: copy of bank statements of the last 3 months, copy of income Tax Departement Return +
ITR V acknowledgment of the previous financial year, and : for employees : 3 last salary slips, for person owning a company : certificate of
registration of the company (Form B, rule 5(1) or Form ST3, rule 11) if aplicable, supporting documents of sponsoring and of personel
incom of the sponsor.

 Applicable price

On the day of your appointment, you must pay a application fee of: 60 €
