Re: Mr Patrick Kruger

I am writing to refer Mr Kruger, a 70 year old retired sailor, who is complaining of headache and
uncontrolled hypertension, his last blood pressure was 170/110.He lives alone and has not children.

He is an obese patient that his past medical history includes, Thyroidectomy 5 years ago, Arthritis,
Pneumonia, Hypertension, depression which occasionally associated with increased alcohol intake.

Mr Kruger’s first visit was 0 months ago when was discharged from hospital after recovering from
pneumonia. At that time he has been receiving attention for blood pressure monitoring once per
week ,managing of medications such as Beta blocker and NSAIDs and (as well as)for daily activities.

I would appreciate it if you could attend Mr Kruger at his home in regards of controlling his blood
pressure, managing his headache and assess his proteinuria.

Re: Mrs Patricia Jennings.

I am writing to refer Mrs Jennings, a 30 year old married and mother of two school age children, who
was admitted in our hospital on 5/05/08 for review and pain management. She is due to be
discharged on 11/05/08

Her past medical history includes Anterior Cruciate ligament repair in 1996, abdominal hysterectomy
as well as bilateral salpingo-oophorectomy in 2007 for ovarian cancer which has metastases to bone
and liver. She has received chemotherapy and radiotherapy and now is on oral medication and
subcutaneous morphine injection for pain reducing.

During hospitalisation, she was receiving subcutaneous morphine infusion for pain management, but
she was reluctant to continue morphine due to increased drowsiness. Oral analgesia has been
started for her with morphine contin on 7/05/08 and also she has been received attention by
palliative care Team on 9/05/08.

In regards of nursing management, she received infusion site dressing change on alternate days and
assisted for mobilization, showering and dressing.

Could you please encourage her for continuing Radiotherapy? It is important to know, her husband
is reluctant to accept her choice to discontinue radiotherapy treatment.

I would appreciate it if you could assist her for showering, dressing and palliative care support.

Dear Mrs Hyles,

Re: Ms Kyile GRESHAM

I am writing to refer Ms Gresham, a 19 year old single, lives with her mother in local area, who was
admitted on 1/10/07 with diagnosis of post-traumatic epilepsy following head injury after motor
vehicle accident. She is due to be discharged on 20/10/07 and her family notified on 18/10/07.

During hospitalization, she admitted in ICU for first 4 days, repeated EEG revealed epilepsy and that
is why she received dilantin.Her wound on the left cheek has been healed without scaring, any pain
has been treated by panadine forte which reduced in doses due to constipation and stopped after 4
days. She was assisted for mobilization by Pick up frame and Wheel chair on 4th day. She has
received attention by Physiotherapist every day and hydrotherapies twice per week.

It is important to know, some residual deficit of fine motor movement in left hand has remained,
and that may affect her personal care activities. Although she feels confident, but needs some
assistance for daily living activities.

Could you please arrange her to be visited by Occupational therapist because of stairs in front of
house? I would be grateful if you assess her rehabilitation therapy and arrange her counselling
programme for returning to work.
