I, NORMA LAPIDARIO QUINTANO, of legal age, married, Filipino citizen and

residing at No. 046 Quirino Street, Tondo, Manila, after being duly sworn to in
accordance with law, depose and say that:

1. That I am applying for Japan visa through Rajah Travel and Tours,
Ermita, Manila;
2. That one of the requirements is school record from my elementary
school which is Holy Infant, Tacloban City, Leyte or from my high school
and university/college which is Far Eastern University (FEU), Morayta
Street, Manila;
3. I can no longer secure my school records from Holy Infant because the
said school was burned 44 years ago or in March 1973;
4. Also, I can no longer secure my school records from Far Eastern
University (FEU) because my records in the said school can no longer be
found or traced since it was already eaten by termites and could no
longer be retrieved in the stock room of the school;
5. I am executing this affidavit to attest to the truth of the foregoing and
for the purpose of securing a Japan Visa and for whatever legal purpose
this may serve.
