AFFIDAVIT UNDERTAKING
I, OMAR A. BAUNTO, of legal age, Filipino, married and resident of #48,
Purok Bagong Pag-asa, Barangay Mahayahay, Iligan City. In my own capacity
as Proprietor/CEO of MArkbuilt Enterprises, which office is located at #48,
Purok Bagong Pag-asa, Barangay Mahayahay, Iligan City after having been
duly sworn to in accordance with law, do hereby depose and say, THAT:

1. I have contracted a Project Construction of Climate Change Phase I at

Sultan Kudarat State University (SKSU), Tacurong City, with project
duration from December 12, 2016 to May 5, 2017;
2. For failure to finish the project on time, I have hereunto appealed to the
Head Office of Sultan Kudarat State University to have an extension until
December 5, 2017;
3. I hereby undertake to increase the manpower in the project by adding at
least six (6) workers and five (5) laborers;
4. I hereby request the SKSU to closely monitor our actual implementation
to ensure quality output;
5. If I failed to finish the project on or before December 5, 2017, the same is
a ground for the blacklisting of the business enterprise/construction
firm;
6. I was not coerced, harassed, intimidated nor forced to execute this
affidavit of undertaking;
7. I am executing this affidavit to attest to the veracity of all the foregoing
facts and for all legal intents and purposes it may serve
