Thromboembolic complications in the nephrotic syndrome: Pathophysiology and clinical management

Introduction

The nephrotic syndrome, first described in 1827 [1], is defined by a urinary protein level exceeding 3.5
g/l.73 m2 /day occurring in association with edema, hypoalbuminemia, hyperlipidemia, and infectious and
thromboembolic complications [2]. Thromboembolic disease is an important complication in patients with
nephrotic syndrome, as both arterial and venous complications occur and the consequences are often severe. On
the other hand, many patients are asymptomatic, leading to diffi culties in establishing the diagnosis and with
controversy existing regarding the role of prophylactic anticoagulant therapy. Much evidence suggests that a
hypercoagulable state exists in the setting of the nephrotic syndrome, but the exact mechanisms are poorly
understood [2].

In this review, we shall discuss the epidemiology and clinical spectrum of thromboembolic disease
occurring in the nephrotic syndrome, the patho physiology of the hypercoagulable state observed in the nephrotic
syndrome, the diagnosis of renal vein thrombosis (RVT) in nephrotic syndrome, and the evidence supporting
prophylactic and thera- peutic anticoagulation for these thromboembolic disorders.

Epidemiology of thromboembolic disease and clinical manifestations

Venous thromboembolic complications in nephrotic syndrome include deep venous thrombosis,


pulmonary embolism and, in particular, RVT. Early observations led researchers to believe that renal vein
obstruction itself led to the development of nephrosis [3—5]. However, advanced roentgenographic techniques
and renal vein catheterization have made it clear that RVT is a consequence of the nephrotic syndrome [6—8].

Nephrotic syndrome of any etiology can be associated with RVT. Estimates of risk associated with different
glomerular diseases have been hampered by differences in disease distribution in patient populations, and
methodologic limitations in studies. Nonetheless, 5 observational studies evaluated patients with nephrotic
syndrome who underwent renal venography [7—11]. One of these studies specifically assessed patients with
membranous glomerulopathy [10], while another did not provide details on underlying disease distribution [11].
The findings from these 5 studies are summarized in Table 1. Taken together, these studies show that the
prevalence of RVT is highest, at 37%, in patients with membranous glomerulonephritis. However, the risk is still
clinically important in other primary glomerular diseases, particularly membranoproliferative glomerulonephritis
and minimal change disease. Furthermore, the risk of developing RVT may be underestimated in these largely
cross-sectional studies, since patients who initially were found not to have RVT may subsequently develop this
complication during the course of their disease. Other studies have attempted to measure the risk for RVT in the
nephrotic syndrome but have methodologic limitations, including a retrospective study design or lack of
standardized and accurate methods to detect RVT [12—15]. Preliminary data from one prospective study found a
prevalence of RVT in 54 patients with nephrotic syndrome of only 1.9% [16].

In the largest prospective study assessing RVT in 151 patients with nephrotic syndrome, RVT was identified
prospectively in 33 (22%) patients [7].In 4 of these patients, thrombosis occurred acutely, and presented with one
or more symptoms of acute flank pain, gross hematuria, and acute deterioration of renal function. The patients
with acute RVT tended to be young adults (mean age: 20 years), whereas patients with chronic RVT or without RVT
were older (mean age, 38 and 36 years, respectively) [7]. These findings suggest that the disease process
underlying the nephrotic syndrome may play a role in the pathogenesis of RVT [17].

The nephrotic syndrome and the risk for deep vein thrombosis and pulmonary embolism

Deep vein thrombosis of the lower extremities is also observed in the nephrotic syndrome and can occur in
isolation in up to 15% of patients [9], or in association with RVT [7]. RVT can also extend beyond the renal veins to
involve the inferior vena cava [8]. Pulmonary embolism may complicate deep vein thrombosis involving the lower
extremities, inferior vena cava or RVT. Thus, in a case series of 151 patients with nephrotic syndrome [7], in whom
94 patients underwent ventilation—perfusion lung scanning symptomatic pulmonary embolism was observed in 4
of 20 (20%) patients with chronic RVT, 1 of 4 (25%) patients with acute RVT, and 7 of 70 (10%) patients

scanning was 12.8%. One retrospective study assessed 102 patients with severe nephrotic syndrome (serum
albumin b2.0 g/dl), of whom 89 patients had complete clinical data and underwent ventilation—perfusion lung
scanning [18]. In this study, pulmonary angiography was performed in patients with a low- or intermediate-
probability ventilation—perfusion lung scan and a high pre-test probability for pulmonary embolism. Pulmonary
embolism was diagnosed in 32% of patients, based on a high-probability lung scan or a positive pulmonary
angiogram. It is noteworthy that decreased antithrombin and fibrinogen levels were the only laboratory markers
associated with pulmonary embolism. Similar findings have been described in pediatric patients with steroid-
responsive minimal change glomerulonephritis [19]. Other prospective case series reported a lower incidence of
pulmonary embolism in patients with nephrotic syndrome, but lung scanningwas done only in symptomatic
patients with clinically suspected pulmonary embolism, with the potential to underestimate the true incidence of
thromboembolic disease [8,9,16,20].

Many clinical studies have demonstrated an association between hypoalbuminemia and venous thromboembolism,
but serum albumin levels in patients with and without thromboembolic events were not significantly different
[7,9,17,21]. For example, in a large prospective study involving patients with the nephrotic syndrome [7], the mean
serum albumin concentration at presentation in patients with chronic RVT, with acute RVT, and without RVT was
2.2, 2.1, and 2.4 g/dL, respec- tively ( P N0.05). However, another study demon- strated that serum albumin b2.5
g/dL, at presentation, was a significant risk factor for the occurrence of venous thromboembolism (40.0% vs. 2.7%,
P b0.0l), and for combined arterial and venous thromboembolic events (28.6% vs. 5.5%, P b0.01) [22]. These data
suggest that hypoalbu- minemia is associated with, but not a prerequisite for, the development of RVT. As discussed
below, hypoalbuminemia may be linked to the development of RVT or may be a surrogate marker for abnormalities
in antithrombin and fibrinogen that predispose to thrombosis.

Summary

The nephrotic syndrome is complicated by the induction of a hypercoagulable state and various venous
and arterial thromboembolic phenomena. RVT is the most common of these findings, is usually asymptomatic, and
is most commonly identified in patients with membranous glomerulopathy. Untreated RVT can predispose to
serious sequelae such as pulmonary embolism, and once identified, should be treated with anticoagulation.
Although many derangements in the hemostatic system have been described in the nephrotic syndrome, clinical
association between specific features of coagulation and thromboembolic complications is not well established.
Platelet activation and aggregation, elevation in factor VIII and fibrinogen levels, hypoalbuminemia, and anti-
thrombin and PAI-1 deficiency appear to be some of the more relevant hemostatic abnormalities identified.

Despite newer roentgenographic techniques including CT and MRI, selective renal venography appears to
be the only accurate method of reliably detecting chronic RVT; these other modalities may be helpful in the
diagnosis of acute and RVT although this remains unproven. Nevertheless, routine venography in patients with the
nephrotic syndrome has not gained wide acceptance due its invasive nature. Therapy of diagnosed venous
thromboembolism in nephrotic syndrome is similar to that in the general population and includes heparin and
warfarin; treatment should probably be continued for as long as the patient remains nephrotic. Given the high
frequency of thromboembolic complications, patients at high risk for thromboembolic events, including patients
with membranous glomerulopathy and a serum albumin b2.0 g/dL, may warrant prophylactic anticoagulation.
Randomized controlled trials are needed to evaluate the role of prophylactic anticoagulation therapy in such
patients in the current treatment era, which includes anti-proteinuric agents such as angioten- sin-converting-
enzyme inhibitors, lipid-lowering therapy, and immunosuppressive treatment.
