WAIVER OF BANK SECRECY RIGHT

In accordance with the provisions of Section 6(F) of the
National Internal Revenue Code, as amended (NIRC). I
hereby voluntarily depose and say that
1. I have a pending internal revenue case described as
follows:
(INSERT A TABLE FOR BASIC DEFICIENCY TAX,
SURCHARGE, INTEREST AND PENALTY AMOUNTS)
2. I am requesting for a compromise settlement due to my
financial incapacity to pay the assessed tax
3. I have no bank deposit account with any bank as
date of this disposition
4. I waive my privilege under R.A. 1405 re: secrecy of
bank deposits and accordingly hereby authorize the
Commissioner of Internal Revenue or the Commissioner’s
duly authorized representative to inquire into my bank
deposit account with any bank purposes of determining
the veracity of my deposition and qualification for
compromise settlement, pursuant to the provisions of
Section 204, NIRC.
5. I accept the condition that the compromise settlement
forged between me and the Bureau of Internal Revenue
maybe nullified by the Commissioner or by his/her duly
authorized representative in the event the said inquiry
results in finding that I in fact have bank deposits of
sufficient amount to cover payment of the said tax
assessed.
6. I agree to waive my right to prescription concerning the
said tax assessment during the pendency of my
application for compromise agreement or within 180 days
from receipt hereof, whichever comes first.
7. I DECLARE UNDER PENALTIES OF PERJURY THAT
THE FOREGOING REPRESENTATIONS ARE TRUE
AND CORRECT AND THAT THE WAIVER OF RIGHTS
ARE GIVEN VOLUNTARILY AND KNOWINGLY, IN
ACCORDANCE WITH THE PROVISIONS OF THE
NATIONAL INTERNAL REVENUE CODE, AS AMENDED.
