 Performed head to toe assessments on patients including neurological, cardiovascular, respiratory, GI/GU,
integumentary, and psychosocial.
 Documented clinical assessments and findings electronically using EPIC, Medi Tech, and Paragon.
 Administered medications using the six rights of medication administration per doctor’s orders.
 Maintained patient confidentiality and safety at all times.
 Provided assistance with ADL’s for assigned patients throughout each clinical experience.
 Educated patients and families on individual care plan, medications, disease process, procedures, and
safety protocols.
 Observed procedures such as, caesarean section, wound debridement, thoracentesis, echo cardiogram,
epidural placement, central line placement, peripheral IV placement, blood draws, urinary catheter
placement/removal, and x-rays.

 Provided private duty total care for a patient with a Traumatic Brain Injury.
 Performed routine head to toe assessments including monitoring for seizures and reporting changes in
patient condition.
 Administered medications through a dual port G-Tube using standing orders and prn for symptom
management. Assessed effectiveness of medications and reported findings.
 Administered enteral nutrition through a dual port G-Tube as scheduled.
 Applied external urinary catheter, monitored urine output, emptied urine bag, and cleaned urine bag, as
needed.
 Utilized a Hoyer lift with a ceiling track system to move the patient to the bathroom for bathing and
toileting.
 Provided comfort measures including positioning, encouraging communication, range of motion, and
transferring from wheelchair to bed.
 Documented on patient condition, care given, medications administered, and intake and output.
