AFFIDAVIT

I EDMON A. DE LEON, of legal age, Filipino citizen, with address at Block 27B Lot3 Grand
Canon St. Grande Valle, Cainta Rizal, after having been duly sworn to in accordance with law,
hereby state that:

1. I am the son of MR. GREGORIO M. DE LEON and MRS. VALENTINA P. ABELARDO;

2. I have been using the name EDMUND ABELARDO DE LEON since childhood;

3. In all of my school and employment records, I have used the name EDMUND
ABELARDO DE LEON and I am publicly known under the same name;

4. When I was securing a certified true copy of my birth certificate in NSO, I found out
that my registered name is EDMON ABELARDO DE LEON and not EDMUND
ABELARDO DE LEON.

5. The names EDMUND ABELARDO DE LEON and EDMON ABELARDO DE LEON both
pertain to the same person;

6. I am executing this Affidavit to attest to the truth of the foregoing and for any legal
purpose it may serve;
