Root canal preparation In all selected cases (1 005 root canals) the same
method, instruments and materials were used for treat- ment. The root canals
were prepared by stainless-steel hand K files using the step-back technique.
The canal was cleaned and shaped to a size of at least three ISO sizes larger
than the master apical file (#25). In some
difficult cases, if the smaller #10 or #8 instruments could not reach the root
apex before enlarging the root canal, it was prepared as far as possible. During
preparation, root canals were irrigated with 2.5% NaClO with every change of
instrument. With re-treatment of previously root canal-filled teeth, the root
canal fillings were softened with chloroform and removed with H files.
Discussion
This is one of the few studies to evaluate the effect of canal calcification and
to determine the influence of several variables on working length accessibility
using multiple regression analysis. This study alerts clinical therapists to
anatomical factors influencing the working length accessibility, and allows for
a direct estimate of success rate given in situ measurements of tooth factors
during the root canal procedure.

FACTORES DE RIESGO DE LA ANATOMÍA DEL DIENTE QUE INFLUYEN EN
LA ACCESIBILIDAD A LA LONGITUD DE TRABAJO DEL CONDUCTO
RADICULAR
El objetivo de este estudio fue analizar la influencia específica de la anatomía del
conducto radicular sobre la accesibilidad de la longitud de trabajo durante la terapia
del conducto radicular. Se examinaron cuatrocientos setenta y seis casos de
tratamiento de conducto (totalizando 1.005 conductos radiculares). Los factores de
riesgo anatómicos evaluados en cada caso incluyeron: tipo de diente (ubicación del
diente), curvatura del conducto radicular y calcificación del conducto radicular, así
como retratamiento endodóntico. La investigación examinó la correlación entre cada
uno de estos factores anatómicos y la duración del trabajo, con un análisis
estadístico que consta de pruebas de Chi-cuadrado y análisis de regresión logística
múltiple. En un análisis factorial independiente, el tipo de diente (ubicación del
diente), la curvatura del conducto radicular, la calcificación del conducto y el
retratamiento endodóntico se determinaron como los principales factores de riesgo.
En un modelo de regresión de factores múltiples, se encontró que la curvatura de la
raíz y la calcificación del canal influyen más significativamente en la accesibilidad
de la longitud de trabajo del conducto radicular (P <0.05). La anatomía del conducto
radicular aumenta la dificultad de preparar el conducto radicular. La consideración
adecuada de la anatomía del diente ayudará a determinar con precisión la dificultad
de preparación antes de la instrumentación. Este estudio alerta a los terapeutas
clínicos sobre los factores anatómicos que influyen en la accesibilidad a la duración
del trabajo y permite una estimación directa de la tasa de éxito de las mediciones in

Introducción
La enfermedad endodóntica se asocia con infección bacteriana múltiple, donde
el tratamiento de conducto sirve como un método de tratamiento significativamente
eficaz [1-2]. Aunque la terapia exitosa depende de muchos factores, uno de los
pasos más importantes en cualquier tratamiento de conducto es la preparación del
canal [3]. La preparación del canal determina el grado de control sobre la eliminación
completa del contenido del conducto radicular, que es crucial para la infección por
desbridamiento [4-5]. Varios estudios han indicado que la capacidad de

El instrumento dental para acceder a la longitud total del conducto radicular (llamado
la accesibilidad a la longitud de trabajo) afecta significativamente el éxito del
tratamiento del conducto radicular. Negishi informó que la inaccesibilidad de la
anatomía apical afecta significativamente el éxito del tratamiento del conducto
radicular [6]. Consistentemente, los investigadores [7] demostraron que un
instrumento que alcanza la constricción apical proporciona el mejor pronóstico.
Estos resultados sugieren que establecer y mantener longitudes de trabajo
adecuadas es fundamental para el tratamiento del conducto radicular.
Desafortunadamente, la preparación del conducto se ve negativamente afectada
por la naturaleza altamente variable de la anatomía del conducto radicular [8-9].
Muchos problemas que ocurren durante la preparación del conducto radicular, como
la falta de canales, la perforación del piso de la pulpa o el transporte del canal, son
el resultado de un conocimiento insuficiente del conducto radicular.

RESULTADOS
Análisis de factor único los resultados estadísticos de la prueba de Chi-cuadrado,
que indicó que los factores de anatomía de: (i) tipo de diente (posición del diente),
(ii) calcificación del conducto radicular y (iii) curvatura fueron variables
estadísticamente significativas que contribuyeron adosado a la accesibilidad a la
longitud de trabajo del conducto radicular. Cuando aumentó la dificultad de
clasificación, disminuyó la tasa de éxito de la accesibilidad de la longitud de trabajo.
El tratamiento endodóntico previo también influyó significativamente en la
accesibilidad a la longitud de trabajo
ANÁLISIS DE FACTORES MÚLTIPLES
Los resultados del cribado del modelo de regresión logística para los factores de
riesgo de la anatomía se enumeran en la Tabla 4. En el modelo de factores
múltiples, se demostró que dos factores tenían
el impacto más significativo en los resultados de la preparación del conducto
radicular: (i) la curvatura del conducto radicular y (ii) la calcificación con P <0.05. El
tipo o ubicación del diente estaba menos relacionado con la accesibilidad a la
longitud de trabajo (P> 0.05).
DISCUSIÓN
Este es uno de los pocos estudios para evaluar el efecto de la calcificación del
conducto y para determinar la influencia de varias variables en la accesibilidad a la
longitud de trabajo mediante el análisis de regresión múltiple. Este estudio alerta a
los terapeutas clínicos sobre los factores anatómicos que influyen en la accesibilidad
a la longitud de trabajo, y permite una estimación directa de la tasa de éxito con
mediciones in situ de los factores dentales durante el procedimiento del conducto
radicular.

Summary: In the present in vivo study, the distance between the entrance orifices
to the root canals was evaluated in the chamber of the maxillary and mandibular first
molars. 60 first molars were used (30 maxillary and 30 mandibular), with vital pulp.
Once the opening and location of each input device to the ducts was made, with an
intraoral camera and its measurement computer program, photos of the floor of the
pulp chamber were obtained and on them, the measurements of the distances
between each one or the element of entrance to the root canals. The specimens
evaluated were grouped in cases with three and four entry holes. The statistical
results showed that the lower molars with three inlet holes were obtained in a
homogeneous group represented by the MV-ML, ML-DL and MV-DV distances,
which were significantly higher than the DV-DL distance. In the upper molars when
the entry holes were three, the different people were different. When four entry holes
were located, the means were grouped into three homogeneous groups
INTRODUCTION
The location of the entry holes to the root canals in the lower and upper first molars
is closely related to the shape of the pulp chamber. In superiors, its shape is
trapezoidal in 81% of cases1. In the lower ones, when the ducts are three, it is
triangular and when they are four, quadrangular2. When external agents, such as
the caries process, periodontal disease, atrictions, cavitary preparations and even
chemical substances, stimulate the cameral pulp, it reacts with the formation of
irritating or repairing dentin3 **. This situation modifies the anatomy of the cameral
pulp, being able to obliterate the entrance to the root canals, making its location
difficult. In our literature review, we have not observed previous studies that evaluate
the distance between each entrance to the root canals. Only the MB-2 canal of the
mesial vestibular root of the maxillary first molars has received special attention.
Stropko observed that said duct was present in 73.3% of the maxillary first molars
by the evaluated4. Recently, Görduysus et al. Observed that MB-2 was located 3.5
mm from the palatine duct and 2 mm from the mesiobuccal duct5 *. A previous
study6 on 60 mandibular first molars revealed that the mesial root had a conduit in
5%, two conduits in 78.3%, three in 13.3% and four in 3.3%. In the distal root they
observed a
pipeline in 58.3%, two in 40% and three in 1.7% of the cases evaluated. When there
is difficulty in locating any of the root canals, it may be of clinical importance, the
knowledge of the distance between them, as
reference point for your location. Therefore, we set the objective of this study to
evaluate the distance between the entrance holes to the root canals in the maxillary
and mandibular first molars.
MATERIAL AND METHODS
In the present in vivo study, a total of 30 maxillary first molars (15 right side and 15
left side) and 30 first mandibular molars were used, distributed in the same way
(table 1). The teeth were chosen at random, and corresponded to teeth with vital
pulp. After the coronary opening and elimination of the cameral pulp, the pulp
chamber was washed with 0.5% deodorized hypochlorite and dried with sterile
cotton. With an endodontic scanner (Dentsply-Maillefer, Ballaigues, Switzerland) the
entrance holes to each root canal were located. Then with a file type K # 15 of
stainless steel No. 15 (Dentsply-Maillefer) the probing of the cervical third was made.
The entrance to each duct was enlarged with a Gates Glidden # 2 strawberry
(Dentsply-Maillefer) proceeding to the irrigation and drying of the pulp chamber. Next
to the conduit entries, a lead sheet 1 cm wide was placed. With the use of an intraoral
camera (H. Schein-Madrid-Spain) a photographic image of the floor of the pulp
chamber was obtained in each tooth to be evaluated. The lens-floor distance of the
pulp chamber was in all cases ± 1 cm (Figure 1). To carry out the measurements,
the photographic images were processed with a computerized image processing
system, from the dental management program Natura Dental (H. Schein). For this,
the computer system rule was calibrated to 1 cm, taking as a reference the width of
the lead sheet previously placed. The measurements were made from the center of
the entrance hole to each root canal. The measurements to be evaluated depended
on the type of tooth, depending on whether it was superior or inferior and presented
3 or 4 entry holes to the root canals (Table 1). Of the first 15 mandibular molars with
four entries, due to an error in the calibration of the computer system, two cases
were eliminated.

STATISTIC ANALYSIS
The data obtained were analyzed statistically. First, the mean values and standard
deviations for the different distances considered were estimated. Then, in order to
determine the existence or not of significant differences between the means of the
distances examined, an analysis of the variance was made for each of the four
groups of measures. In all cases, each
molar examined as a block in order to isolate its effect on the response. Therefore,
the model considered to explain each distance observed was that of a one-way
design (type of distance) of fixed effects (each of the distances) with a blocking factor
(each molar). That is, if y (ijk) represents the k-th observed value of the ith distance
on the jth tooth, then: y (ijk) = m + a (i) + b (j) + e (ijk) ) where m represents the global
mean, a (i) represents the differentiating effect of the distance type (i = 1,2,3 when
there are three inputs oi = 1,2,3,4 when there are four inputs), b (j) ) denotes the
differentiating effect associated with the j-th tooth and (ijk) the random error not
explained by our model. It should be emphasized that in all cases the analyzes
carried out of the different estimated models overcame the relevant tests of
normality, randomness and heteroskedasticity, thus validating the structural
hypotheses of the model and the variance analysis technique used. Finally, and
given that the results of the ANOVA were significant (p <0.05), post-hoc contrasts of
multiple rank were carried out in order to determine the pairwise differences. In
particular, the Tukey test was used.
RESULTS
First madral molar (n = 28) Table 2 shows the estimates of the sample means and
the corresponding standard errors for each distance in the molars.
mandibular according to have three or four entries. Based on these descriptions, the
95% confidence intervals shown in Figure 2 were also calculated. The analysis of
the variance showed the suitability of the models considered in both cases since the
sum of squares of the blocks represents a high percentage of the global variability
(69% with three entries and 65% with four) and the interaction graph showed the
absence of interaction between the molars and the distances. The analysis of
variance showed the existence of significant differences between the different
average distances for the case of four entry holes (p <0.05), but no significant
differences were detected (p> 0.05) in the case of three entry holes. To determine
the pairwise differences in the case of four entries, a Tukey HSD test was performed,
resulting in two homogeneous groups with a 5% significance. One would be formed
by the three distances MV-ML, ML-DL and MV-DV and, the second would
correspond to the distance DV-DL, significantly lower in average to any of the first
group.
First Maxillary Molar (N = 30)
An analogous analysis was performed for the maxillary molars (see table 3 and
figure 3). Also in this case the analysis of the variance showed statistical significance
and the subsequent Tukey test differentiated the following homogeneous subgroups.

In the case of admission (23 cases), the three average distances were significantly
different (p <0.05) with the order VM-VD <P-VD <P-VM. In the case of four entry
holes, the Tukey test determined the existence of three homogeneous subgroups.
An intermediate subgroup consisting of VM-VD, MB-2-P and P-DV, a subgroup with
significantly higher mean (p <0.05) forming the VMP distance and, finally, the
subgroup formed by VM-MB-2 and VM-VD. In the latter case, however, it should be
noted that the significance has been only 0.059, so it would be advisable to have a
larger sample to defend this claim
DISCUSSION
The effective instrumentation of the root canals begins with their correct location and
approach. When the main anatomical references of the pulp chamber (pulp horns,
ceiling and floor of the pulp chamber, grooves of union of the entrances of the root
canals and the entrances themselves) have not been modified, the location of the
entrance orifices to the root canals are usually not difficult. In this circumstance the
holes are located in the axio-pulp angle of the floor of the cavity
In a normal tooth, the internal anatomy of the pulp chamber is mainly designed by
the formation of secondary dentin, the floor of the pulp cavity can take different
geometric aspects according to the amount and location of the entry holes to the root
canals (triangular or trapezoidal). These characteristics can be modified by
responses of the pulp tissue with the formation of calcification. The literature points
out that changes in pulpal calcification are often a response to stimuli from different
sources such as dental procedures, restorative materials, caries,
tooth abrasion, periodontal disease, pulpal inflammation, pulpal protection, and
trauma8 *, 9. In these situations, the pulp can react with the dentin formation of
irritation. This type of dentin produces calcifications in the pulp chamber (pulpal
stones, true or false) or in the root canals (diffuse or linear calcifications) 10 *. This
form of pulpal reaction modifies the anatomical characteristics of the normal pulp
chamber and can obliterate the entry holes to the root canals. In addition, depending
on the pulpal reaction, the entrance can be displaced from the original axio-pulp
angle.
If the canal is not detected, and can not be instrumented or sealed, there may be a
potential cause of endodontic therapy failure11. When the pulp chamber is calcified,
the risk of perforation during the location and access to the entrance holes to the
root canals is increased12, with the furcation in the molars, which occurs most
frequently10. The clinical experience shows us that in very few occasions the
entrance holes to the root canals are calcified. Finding some of them and knowing
the approximate distance to the supposed entrance area of the calcified orifice may
be of clinical utility, thus avoiding perforation of the chamber floor either for the
radicular furcation, or for the lateral zones of the root. Some root canals do not have
easy accessibility or are not visible from the pulp chamber; this is the case of the
additional ducts in the mesial root of the mandibular molars, which are often left
untreated13. When the first mandibular molars have two entrances in their distal
root, we have observed that the average distance between them is 2.429 mm (ET
0.169) with a confidence interval of 95%: (2.153 to 2.706). In our study, we observed
a distance between the entrance of the MV and palatal canal of 6.391 mm; between
palatal and MB-2 of 4,196 mm and between VM and MB-2 of 2,383 mm (table 2).
These results agree to a great extent with those observed by Görduysus et al5,
where the MB-2 input was observed at a distance of 3.5 mm from the
alatal canal and 2 mm from the MV canal. In this study, no reference is made to the
distance between VM and Palatine, which in our experience was 6.391 mm and
5.164 mm depending on whether the presence of MB-2 was observed or not. The
measurements obtained in the lower first molars corroborate that when the entry
holes are three, the arrangement between them forms a triangular geometric figure,
whose base is mesially oriented. When being four, this geometric figure changes to
trapezoidal with the greater side again in mesial. The distance between the entry
holes in the mesial roots was 3.372 mm when the tooth presented
three entrance holes and 3.529 mm, when the teeth had four entry holes. When the
upper molars were evaluated, our measurements showed that the entrance holes
are in a triangular geometric distribution with a vestibular base, in addition to the
number of entries it has. This shape can only be affected by the location of the MB-
2 entrance, which according to Görduysus et al7, and following an imaginary line
that joins the vestibule-mesial and palatal entrances, can deviate mesially up to 0.69
mm7. When we evaluate the upper molars with three entry holes
da, it was observed with a 5% significance that the three distances were different
(VM-VD <P-VD <P-VM). When the first upper molars had four entry holes, three
homogeneous subgroups were observed (Figure 3).
CONCLUSION
Given the difficulty of locating the entrance of any of the root canals in maxillary and
mandibular first molars, knowledge of the distance between them is an important
clinical fact to be considered for its location

resumen: En el presente estudio in vivo , se evaluó en la cámara pulpar de los
primeros molares maxilares y mandibulares, la distancia entre los orificios de
entrada a los conductos radiculares. Se emplearon 60 primeros molares (30
maxilares y 30 mandibulares), con pulpa vital. Efectuada la apertura y localización
de cada orificio de entrada a los conductos, con una cámara intraoral y su programa
informático de mediciones, se obtuvieron fotos del piso de la cámara pulpar y sobre
ellas, las medidas de las distancias entre cada orificio de entrada a los conductos
radiculares. Los especímenes evaluados se agruparon en casos con tres y cuatro
orificios de entrada. Los resultados estadísticos demostraron que en los molares
inferiores con tres orificios de entrada se obtuvo un grupo homogéneo representado
por las distancias MV-ML, ML-DL y MV-DV, las que resultaron significativamente
superiores a la distancia DV-DL. En los molares superiores cuando los orificios de
entrada fueron tres, las distancias obtenidas fueron diferentes. Cuando se
localizaron cuatro orificios de entrada, las medias se agruparon en tres grupos
homogéneos.
INTRODUCCIÓN
La ubicación de los orificios de entrada a los conductos radiculares en los primeros
molares inferiores y superiores está en estrecha relación con la forma de la cámara
pulpar. En los superiores su forma es trapezoidal en un 81% de los casos1 . En los
inferiores, cuando los conductos son tres es triangular y cuando son cuatro,
cuadrangular2. Cuando agentes externos, como el proceso de caries, la
enfermedad periodontal, atricciones, preparaciones cavitarias y hasta sustancias
químicas, estimulan a la pulpa cameral, esta reacciona con la formación de dentina
de irritación o reparadora3**. Esta situación modifica la anatomía de la pulpa
cameral, pudiendo llegar a obliterar la entrada a los conductos radiculares, haciendo
dificultosa su ubicación. En nuestra revisión bibliográfica, no hemos observado
estudios anteriores que evalúen la distancia existente entre cada entrada a los
conductos radiculares. Sólo ha merecido especial atención el conducto MB-2 de la
raíz vestíbulo mesial de los primeros molares maxilares. Stropko observó que dicho
conducto estuvo presente en el 73,3% de los primeros molares maxilares por el
evaluados4. Recientemente, Görduysus y cols, observaron que el MB-2 se ubicaba
a 3,5 mm del conducto palatino y a 2 mm del conducto mesiobucal5*. Un estudio
anterior6 sobre 60 primeros molares mandibulares, reveló que la raíz mesial tenía
un conducto en un 5%, dos conductos en el 78,3%, tres en un 13,3% y cuatro en
un 3,3%. En la raíz distal observaron un conducto en el 58,3%, dos en el 40% y tres
en el 1,7% de los casos evaluados. Cuando se presenta dificultad en la localización
de alguno de los conductos radiculares, puede ser de importancia clínica, el
conocimiento de la distancia existente entre ellos, como
punto de referencia para su ubicación. Por ello fijamos como objetivo del presente
estudio evaluar la distancia existente entre los orificios de entrada a los conductos
radiculares en los primeros molares maxilares y mandibulares.
MATERIAL Y MÉTODOS
En el presente estudio in vivo , se utilizaron un total de 30 primeros molares
maxilares (15 lado derecho y 15 del izquierdo) y 30 primeros molares mandibulares,
repartidos de idéntica forma (tabla 1). Los dientes fueron elegidas al azar, y
correspondieron a dientes con pulpa vital. Tras la apertura coronaria y eliminación
de la pulpa cameral, la cámara pulpar fue lavada con hipoclorito desodio al 0,5% y
secada con algodón estéril. Con un explorador endodóncico (Dentsply-Maillefer,
Ballaigues, Suiza) se procedió a la localización de los orificios de entrada a cada
conducto radicular. Luego con una lima tipo K#15 de acero inoxidable nº15
(Dentsply-Maillefer) se efectuó el sondaje del tercio cervical. La entrada a cada
conducto fue ampliada con una fresa Gates Glidden nº2 (Dentsply-Maillefer)
procediendo luego a la irrigación y secado de la cámara pulpar. Próximo a las
entradas de los conductos, se colocó una lámina de plomo de 1 cm de ancho. Con
la utilización de una cámara intraoral (H. Schein- Madrid-España) se obtuvo en cada
diente a evaluar, una imagen fotográfica del piso de la cámara pulpar. La distancia
lente-piso de la cámara pulpar, fue en todos los casos de ±1 cm (fig. 1). Para la
realización de las mediciones, las imágenes fotográficas fueron procesadas con un
sistema informático de tratamiento de imágenes, del programa de gestión dental
Natura Dental (H. Schein). Para ello, se calibró la regla del sistema informático a 1
cm, tomando como referencia el ancho de la lámina de plomo colocada
previamente. Las mediciones fueron realizadas desde el centro del orificio de
entrada a cada conducto radicular. Las mediciones a evaluar dependieron del tipo
de diente, según fuese superior o inferior y presente 3 o 4 orificios de entrada a los
conductos radiculares (tabla 1). De los 15 primeros molares mandibulares con
cuatro entradas, por un error en la calibración del sistema informático, se procedió
a la eliminación de dos casos.

ANÁLISIS ESTADÍSTICO
Los datos obtenidos fueron analizados estadísticamente. En primer lugar se
estimaron los valores medios y las desviaciones estándar para las distintas
distancias consideradas. Después, con objeto de determinar la existencia o no de
diferencias significativas entre las medias de las distancias examinadas, se realizó
un análisis de la varianza para cada uno de los cuatro grupos de medidas. En todos
los casos se consideró cada
molar examinado como un bloque al objeto de aislar su efecto en la respuesta. Por
lo tanto el modelo considerado para explicar cada distancia observada fue el propio
de un diseño de una vía (tipo de distancia) de efectos fijos (cada una de las
distancias) con un factor de bloqueo (cada molar). Es decir, si y(ijk) representa el
k-ésimo valor observado de la i-ésima distancia en el j-ésimo diente, entonces: y(ijk)
= m + a(i) + b(j) + e(ijk) donde m representa la media global, a(i) representa el efecto
diferenciador del tipo de distancia (i=1,2,3 cuando hay tres entradas o i=1,2,3,4
cuando hay cuatro entradas), b(j) denota el efecto diferenciador asociado al j-ésimo
diente y e(ijk) el error aleatorio no explicado por nuestro modelo. Conviene subrayar
que en todos los casos los análisis realizados de los distintos modelos estimados
superaron las pertinentes pruebas de normalidad, aleatoriedad y
heterocedasticidad, validando así las hipótesis estructurales del modelo y la técnica
del análisis de la varianza empleada. Finalmente y dado que los resultados de los
ANOVA fueron significativos (p<0,05), se realizaron contrastes post-hoc de rango
múltiple al objeto de determinar las diferencias pairwise. En particular se empleó la
prueba de Tukey.
RESULTADOS
Primer molar madibular (n=28) La tabla 2 recoge las estimaciones de las medias
muestrales y de los correspondientes errores estándar para cada distancia en los
molars mandibulares según tengan tres o cuatro entradas. En base a estos
descriptivos se calcularon también los intervalos de confianza al 95% que se
muestran en la figura 2. Los análisis de la varianza mostraron la conveniencia de
los modelos considerados en ambos casos ya que la suma de cuadrados de los
bloques representa un elevado porcentaje de la variabilidad global (69% con tres
entradas y 65% con cuatro) y el gráfico de interacciones mostró la ausencia de
interacción entre los molares y las distancias. El análisis de la varianza mostró la
existencia de diferencias significativas entre las distintas distancias medias para el
caso de cuatro orificios de entrada (p<0,05), pero en cambio no se detectaron
diferencias significativas (p>0,05) en el caso de tres orificios de entrada. Para
determinar las diferencias pairwise en el caso de cuatro entradas se realizó un test
HSD de Tukey resultando que con un 5% de significación se encuentran dos grupos
homogéneos. Uno estaría formado por las tres distancias MV-ML, ML-DL y MV-DV
y, el segundo correspondería a la distancia DV-DL, significativamente inferior en
promedio a cualquiera de las del primer grupo.
