Describe the instructional activities that will occur PRIOR to the SRT activity and how you will introduce
the SRT activity.
The students will engage in a fun discussion based lesson in the classroom where the content for this subject
will be delivered. They will use think-pair-share to come up with the 6 domains of health and wellness, an
example of what each domain includes, and at least one SMART goal that can improve one personal domain
area.
Describe the purpose of the SRT activity (check all that apply):
☒ Assess prior knowledge ☐ Anticipatory set (Create interest in a topic) ☐ To illuminate common
misconceptions ☐ Formative assessment of content knowledge (for purpose of differentiation and
mastery for ALL students) ☐ Summative assessment of content knowledge ☐ Test preparation
☐ Survey/Poll ☐ Discussion starter ☐ “Homework” collection ☐ Other (please explain):

Student Response and Assessment Tools
The SRT activity will be used purely as a studying tool in preparation for an upcoming exam. The students will
have access to the Quizzlet website on Introduction to Health where they can choose the best way they feel
they should study. They have the option of matching words with definitions, using flashcards to memorize
terms with definitions, take tests as many times as possible for practice, and even play games to help retain
the information learned from the classroom lesson.
If you are unable to provide a working sample of your questions, please list them below (8-10):
1. What are the 6 Components of Health?
- Environmental, Physical, Social, Emotional, Spiritual, and Intellectual Health
2. What is physical health?
- Everything ranging from the absence of disease to fitness level
3. What is social health?
- One's ability to form satisfying interpersonal relationships with others
4. What is intellectual health?
- Ability to think clearly and make responsible decisions
5. What is emotional health?
- Ability to express and control emotions
6. What is spiritual health?
- Having a sense of meaning and purpose in one's life
7. What is environmental health?
- Appreciation of one's external environment (in which you live, work & play can negatively or
positively affect you)
8. What does the "S" in SMART Goals Stand for?

9. What does the "M" in SMART Goals Stand for?
- Measurable
10. What does the "A" in SMART Goals Stand for?
- Achievable
11. What does the "R" in SMART Goals Stand for?
- Realistic
12. What does the "T" in SMART Goals Stand for?
- Time-Oriented
Right/Wrong answers: Will there be right/wrong answers to these questions?
☒ Yes ☐ No
☐ Mixed (Some will have correct answers, other will not.)
Immediate corrective feedback: Will you pre-select correct answers to some of all of the questions and
display correct response to the class after the SRT activity?
☒ Yes
☐ No
Why or why not?
Instead of confusing students with not providing the correct answers, Quizzlet will provide students with the
correct answer immediately so the student will know if they are in the right track, or if they are missing the
correct answers. By doing this, they students will know which area needs more studying, and which ones they
are strong in.

Describe what will happen AFTER the SRT activity?
After the SRT activity, the students will have access to the areas within the subject that needs help before the
exam, which will be happening during the following class. They can choose to continue using the SRT tool to
study some more, or find other resources to continue studying before the exam.
Reflective Practice:
I plan on using Quizzlet for future lesson, because I feel like it is a great studying tool that can help students
retain information for upcoming discussion and exams.
