Patients’ perceptions and experiences of living with a surgical wound


healing by secondary intention: A qualitative study

In the Journal expland surgical wound healing by secondary intention are open surgical
wounds that are left to heal from the base up. Is a widely used intervention, where by negative
pressure is applied to a wound via a gauze or foam dressing and theoretically promotes wound
healing by removing exudate and reducing infections ; however, evidence of its effectiveness is
limited.

Open surgical wounds are also managed through application of a variety ofwound dressin
gs, generally applied by hospital or community nurses, requiringpatients to undergo frequent dres
sing changes, often including packing of thewound cavity.Our qualitative study, embedded withi
n a large programme of research relatedto open surgical wounds, specifically aimed to explore pa
tients' perspectives ofliving with an open surgical wound, and to elicit their views regarding heali
ng oftheir wounds and their experiences of treatment.

Two published audit studies from the North of England, estimated that opensurgical wou
nds constitute approximately 28% of all prevalent acute woundsthat were receiving wound care p
rovision.Expectations of wound healing A lack of understanding regarding the process ofwound
healing by secondary intention and fear and anxiety that healing of theiropen surgical wound wo
uld never occur were common: ".

Treatments for open surgical wounds The main wound treatments experiencedby study p
articipants were negative pressure wound therapy; debridement; skingrafting; dressings.Systemat
ic reviews of negative pressure wound therapy for chronic woundshave concluded that there is n
o valid or reliable evidence that it accelerateschronic wound healing.

Our study yielded rich data, providing insights into how patients view andexperience surg
ical wounds healing by secondary intention and enhancingunderstanding of the impact of these w
ounds on patients' lives, as well assensitizing readers to some of the challenges associated with m
anagement ofthese wounds.The lack of a robust evidence base for costeffective wound managem
ent makesit difficult for clinicians to adopt an informed approach to the management ofnon-
healing, open surgical wounds.
