2.0. Introduction
o If it is about a SERVICE, WHERE will the service be available? HOW will the
service supposed to help the students?
State the problems that have been identified from NOT having the service or program by
only mentioning the aspects of it (e.g.: availability or maintenance of service OR food,
accommodation, transportation, location of the program. You can refer to the
questionnaire that you have made.)
Conclude your introduction by stating the purpose of writing your proposal report.
3.0 Methodology
State what method that you use in collecting information for this proposal report. Did you
conduct a survey? Interviews? Observation? DEFINE the method that you are using. (E.g.: If
you choose survey, explain what is survey. Cite the information if you get the definition from
somewhere else, using APA/IEEE format.) Justify WHY did you choose the method to collect
the data (Use citation again here.)
a. Sampling.
Based on the Demographic Profile section in your questionnaire, describe about your
respondents:
o Age range?
o Which school/program?
o Etc.
b. Questionnaire.
o How is the questionnaire being organized in the questionnaire? Did you divide it
into sections?
o The types of questions used. Did you use only ONE type OR did you COMBINE
several types of questions? Define each type of questions that you use. (Again,
include a proper citation if you cite any.)
4.0 Problems
Describe in detail the PROBLEMS that you have identified that inspired you to propose
the topic
Support with the finding that you obtained from the survey. Did the respondents agree
with the questions asked.
You can display the result in the form of graph/chart to support your statement.
Summarize the finding from the graph (apply what you have learn from Chapter 7: Data
Interpretation)
5.0 Solutions
After you have decided on your problems, brainstorm with your group members and
come up with THREE possible solutions to solve each problem.
Ensure that the solutions you suggested are logic and executable.
E.g.:
o Proposed solutions:
Set up a student learning hub where students can personally consult with
the tutors.
Support your statements with the data you have obtained from the survey. If can be
presented in the form of graph/chart.
Summarize the finding from the graph (apply what you have learn from Chapter 7: Data
Interpretation)
6.0 Benefits
From the SOLUTIONS that you have presented, list out the benefit that will be gained if
the solutions are implemented.
You can describe about how the solutions will benefit the UniMAP students particularly,
the staff, of UniMAP as a whole.
7.0 Management
Identify the departments or organizations that will be involved in organizing the program
or service.
You are encouraged to suggest for collaboration between the departments/ organizations.
How do you plan to promote about the program or service? What are the possible
channels that you can use to promote the program or service?
8.0 Cost
But, if there are no expenses involved, just write ‘no expenses are required’.

For any information that you have cited (e.g.: when you want to define what is survey,
advantage of doing survey, types of questions that you use in the questionnaire, etc.),
provide a reference list using either APA or IEEE style.
Please make sure you apply the in-text citation method as well (Refer to Chapter 4 in the
textbook). If there is no in-text citation made, the reference list will not be counted.
