SAFE WORK PROCEDURE FOR LIFITNG OPERATION AND EQUIPMENT

1. PURPOSE
The purpose of this procedure is to establish general safe work practices with regard
to Lifting Operations and Lifting gears and Lifting equipment’s. All Schindler employees
and Schindler Sub-contractors’ workers are required to observe the following
requirements

2. SCOPE
This procedure is applicable to
 JEWEL Project work site
 Modernization and Major-Repair work sites
 Maintenance work sites

3. REFERENCE
 WSH (General Provisions) Regulations
 WSH (Construction) Regulations

4. PROCEDURE
A register of all lifting gears must be maintained containing all particulars such as
Equipment’s Number, Safe Working Load (SWL), Date of Test, Certificate Number
etc. for all the Lifting Gears, Lifting Appliances and Lifting Machines.

4.1 LIFTING GEARS (Wire Slings, Shackles, Lifting Chains etc…)


4.1.1 All lifting gears must be tested and examined by an authorized examiner.
4.1.2 All lifting gears must have a valid certificate signed by the authorized examiner
specifying the Safe Working Load for the gear.
4.1.3 All lifting gears must be thoroughly examined at-least once every 6 months and
fresh test certificates must be issued and maintained.
4.1.4 Lifting gears without a valid test certificate must not be used.
4.1.5 Lifting gears must have tags displaying the following details.
> Lifting Gear Number (LG number)
> Safe Working Load (SWL)
> Date of Test
4.1.6 Lifting gears must not be loaded beyond their Safe Working Load

4.1.7 Lifting Supervisor ensure all lifting gears must be maintained well, stored well
and used properly, weekly visual inspection to conduct for all lifting gears by lifting
supervisor.
4.1.8 Lifting Supervisor ensure to visually inspect the lifting gears every time before
use to check for any defects or damages.
4.1.9 Do not use any lifting gear that is damaged, ceased lifting gear and tag a label
“Damaged, Do not Use” and remove damaged lifting gears immediately from site

4.1.10 The test certificate of all lifting gears used at site must be available for
verification checks.
4.1.11 Do not load at the tip of the hook.

4.2 LIFTING APPLIANCE (Chain Block, Lever Block, Winch etc…)

4.2.1 All lifting appliances and machines must be tested and examined by an
authorized examiner

4.2.2 All lifting appliances and machines must have a valid certificate signed by the
authorized examiner specifying the Safe Working Load for the gear

4.2.3 All lifting appliances and machines must be thoroughly examined at-least once
every 6 months and fresh test certificates must be issued and maintained.

4.2.4 Every crane, crab and winch must have a good brake or safety device that shall
prevent the load from dropping when suspended and controls the load
effectively when operated (lowered or raised).

4.2.5 All hand winched must have a good pawl capable of sustaining the SWL.

4.2.6 All lifting appliances must have clear marking of its Safe Working Load and the
Equipment Number.

4.2.7 Lifting appliance must not be loaded beyond its Safe Working Load

4.2.8 The anchorage point for the lifting appliance must be safely accessible and
strong to bear the loading that is going to be subjected.

4.2.9 Only trained and competent personnel shall operate a lifting machine that have
successfully completed appropriate training
4.2.10 All lifting appliances must be visually inspected for any defect or damage
every time before use by the respective users.

4.2.11 Operators of Lifting machines and appliances must ensure that suspended
loads are not moved over or above any person at the worksite.

4.2.12 Loads being handled by lifting machines must have tag lines to control and
prevent swinging.

4.2.13 Do not ride on the lifting machines or loads at any point of time.

4.2.14 No load shall be left suspended on the lifting machine when not in use.

4.3 LIFTING ACTIVITY


Every lifting activity must have the following preparations:

4.3.1 Lifting Plan and Method Statement: Every lifting plan must include the
following
This must be a pictorial representation of the lifting sequence to scale considering the
site condition, reach of the crane (applicable only during delivery of material), SWL
and all critical factors that affect the safe completion of the activity.
Application of Permit-to-work for lifting operations by lifting supervisor during unloading
of materials using lorry crane (applicable during delivery of materials)
Step by Step procedure from mobilization to demobilization
Mechanical Integrity Records: Test certificates of all the lifting gears and appliances
and equipments used in the lifting activity. Verification of the test certificates must be
done.
Competency Records: Training certificates of involved personnel such as Riggers,
Signalmen, Lifting supervisors and Crane Operators.

4.3.2 Risk Assessment: Risk assessment must be carried out for lifting activity steps
and appropriate control measures must be developed to mitigate any medium or high
risks that could be encountered.
4.3.3 The area of the lifting activity must be cordoned off to prevent non-involved
people from entering the risk zone. Appropriate warning signboards must be displayed
as well.

5.SHIFTING OF ESCAVATOR USING 2 FORKLIFTS.

 Ensure ground are even and solid


 Put up Signage and temporary barricading.
 Banksman are to be at the area for a look out for temporary barricading to avoid
unauthorized personal from entering site area.
 Use plywood or metal plating for uneven ground.
 Ensure forklift are maintained with COS.
 Pre-test run before commence of work.
 Visual check on condition of forklift.
 Only licensed approved may operate the forklift.
 Clear communication with supervisor via walkie-talkie.
 Command are only to be given by only one-person in charge.

6. USING OF ‘A’ FRAME LADDER FOR RIGGING ESCALATOR TRUSS.

Permit to work at height


 Valid certificates
 Visual check on condition of ladder.
 Comply to P.T.W system.
 Ensure top 3 ranks are covered to avoid working on top.
 Maintain 3-point contact at all time.
 No carrying of materials when going up ladder.
 No carrying of tools when going up ladder.
 Maintain buddy system when going up ladder.
 Maintain buddy system while working on ladder.
 Only 1 man are allowed to work on each ladder.
 Ensure ground are even & firm.
 Visual inspection of ground condition before start.
 Avoid over stretching beyond standing point.

7. OPENING OF BARRICADE FOR ESCALATOR HOISTING.

 Ensure that main-con were informed before opening barricades.


 Appling of PTW for working at height if need.
 While not in use ensure barricade are placed back onto its original position.
 Put up Signage for temporary barricading.
 Banksman are to be at the area for a look out for temporary barricading to
avoid unauthorized personal from entering site area.

8. HOISTING OF ESCALATOR USING LORRY CRANE AND ELECTRICAL


CHAINBLOCKS.

 Apply permit to work for lifting.


 Ensure valid certificates for LG/LM/LA
 Visual inspection on the LG/LM/LA.
 Comply to PTW system.
 Ensure all worker are competent and trained personal.
 Cordon the areas with signages.
 No unauthorized personal are allowed to enter the working premises.
 Check on out rigger floor condition before commences of work.
 Rigger and signal man are to be on site when hoisting in commence.
 Only to be commanded by operation executer in charge.
 Clear communication to be given via walkie-talkie.
 Ensure to pre-test before use.

9. USING OF FORKLIFT FOR FINAL POSITIONING OF ESCALATOR (NOT FOR


HOISTING).

 Ensure ground are even and solid


 Put up Signage and temporary barricading.
 Banksman are to be at the area for a look out for temporary barricading to avoid
unauthorized personal from entering site area.
 Use plywood or metal plating for uneven ground.
 Ensure forklift are maintained with COS.
 Pre-test run before commence of work.
 Visual check on condition of forklift.
 Only licensed approved may operate the forklift.
 Clear communication with supervisor via walkie-talkie.
 Command are only to be given by only one-person in charge.
 Barricades are required to shift near to edge for forklift fork to access and to
final positioning of escalator.

9.1.1 RIGGING

 Practice good rigging procedures (Check rigging before lifting)


 Only certified rigger-man are allowed to rigged the load
 Use padding on load edges
 Ensure safety latch is in good conditions
 Use correct Rigging method, e.g. (Basket Hitch)

9.1.2 SIGNALLING

 Only certified signaler are allowed to give signal during lifting operations
 Practice good communication within the lifting team
 Give correct signal to crane operator
 Ensure rigging is safe before lifting

10. CONTINGENCY PLAN INCASE OF MALFUNCTIONAL OF ELECTRICAL


CHAIN BLOCK

 2 nos 5 tons manual chain blocks will be standby on site to replace


any malfunctional electrical chain block.
 Once any malfunctional of electrical chain hoist occur, the escalator
will be rest onto the slab. (to a safest region to ensure worker with
proper fall arrester protection can hook the manual chain blocks and
unhook the electrical chain block)
 Once proper rigging is done and replaced, competent person will
recheck and finalize it before re-hoisting operation begins.
 Lorry crane operator must monitor this transition and control
the boom/ crane winch if necessary.
 Ensure to pre-test run before officially start hoisting.

11. RECORDS

 Register of Lifting Gears (LG), Lifting Appliances (LA) and Lifting Equipments
(LE)
 Test Certificates of LG/LA/LE
 Training Records

12. FORKLIFT

 Ensure forklift maintain monthly inspection with records.


 Pre-check forklift condition prior to start work.
 Ensure forklift operator is licensed competent.
 Ensure forklift does not exceed SWL of the forklift.
 Sound horn near intersection and during reversing.
 While moving, carrying load with fork at axel level or at safe height.
 Banksman are to guide the forklift operator during movement.
 If load obstructs view, banksman are to give signal / guide operator at all times.
 Barricade are to put up at all times around the working area and the routes
taken by forklift operator.
 Ensure load of the materials that the forklift are carrying does not exceeds 3x
of the forklift’s fork.
 Worker working in the area are to use proper PPE (Safety shoes, safety helmet,
Luminous vest and hand gloves) at all times.

13. METHOD OF RIGGING

 Single leg method.
