OBSERVATIONS: Major Closure -

German Cockroaches - All life cycles

Observed 7 live cockroaches between rubber top set and wall next to 4-door reach-in refrigerator in back beer brewing area, 14 live
cockroaches on glue trap beneath 4-door reach-in refrigerator next to infested wall of back beer brewing area, 4 dead cockroaches
on glue trap beneath 4-door reach-in/2-door upright of back beer brewing area, 2 dead cockroaches on glue trap with dead rodents
near roll-up door of back brewing area, 7 live cockroaches in bottom corner just above cove tile base of dry storage area of kitchen,
3 live cockroaches on glue trap located on floor near corner of dry storage room in kitchen, 2 live cockroaches in corner area
beneath dishwash machine in kitchen, 2 live roaches in corner next to shelf of prep area of kitchen, 2 live on glue trap located
beneath ice machine in kitchen and 20+ live and dead cockroaches on glue trap located beneath bar equipment.

Rodent (mice)
20 dropping on top of open bulk bag of malt in back brewing area, 100+ droppings on floor along walls of back beer brewing area, 20
droppings on bottom shelf of prep sink in back brewing area, 10 droppings on top of lid of bulk container of masa inside dry storage
area of kitchen and two dead rodents in glue trap located next to roll-up door of back brewing area.

Must clean and remove all live and dead vermin and droppings immediately. Continue with professional pest service.

Minor
20+ insects (weevils) in masa of bulk container in dry storage room of kitchen. Discard immediately.

Dead Oriental cockroaches on glue traps in kitchen, back beer brewing area and front beer brewing area. Discard immediately.

NOTICE OF TEMPORARY SUSPENSION OF HEALTH PERMIT AND IMMEDIATE CLOSURE OF FOOD FACILITY

This notice is to inform you that as of this date the Environmental Health Permit for the above mentioned food facility is hereby
suspended and all operations therewith are ordered to cease. This action is taken in accordance with Section 114409 of the
California Health and Safety Code which states “If any imminent health hazard is found, unless the hazard is immediately corrected,
an enforcement officer may temporarily suspend the permit and order the food facility immediately closed.”

The violations for which this action is taken and associated sections of the Health and Safety Code (H&SC) are as follows:

Section 114259.1 H&SC: Food is subject to contamination from vermin.

You have the right to make a written request for a hearing to the Environmental Health Division located at 10590 Armstrong Ste B.,
Mather, CA 95655, within 15 days after receipt of this notice to show just cause why the permit suspension is not warranted. Failure
to request such a hearing within the 15-day period shall be deemed a waiver of the right to a hearing. After these violations have
been corrected, you must call Environmental Health for a reinspection to reinstate the permit to operate. Phone: (916) 875-8440.
